    Geochemistry and petrogenesis of early cretaceous sub-alkaline mafic dykes from Swangkre-Rongmil, East Garo Hills, Shillong plateau, Northeast India

    Numerous early Cretaceous mafic and alkaline dykes, mostly trending in N-S direction, are emplaced in the Archaean gneissic complex of the Shillong plateau, northeastern India. These dykes are spatially associated with the N-S trending deep-seated Nongchram fault and well exposed around the Swangkre-Rongmil region. The petrological and geochemical characteristics of mafic dykes from this area are presented. These mafic dykes show very sharp contact with the host rocks and do not show any signature of assimilation with them. Petrographically these mafic dykes vary from fine-grained basalt (samples from the dyke margin) to medium-grained dolerite (samples from the middle of the dyke) having very similar chemical compositions, which may be classified as basaltic-andesite/andesite. The geochemical characteristics of these mafic dykes suggest that these are genetically related to each other and probably derived from the same parental magma. Although, the high-field strength element (+rare-earth elements) compositions disallow the possibility of any crustal involvement in the genesis of these rocks, but Nb/La, La/Ta, and Ba/Ta ratios, and similarities of geochemical characteristics of present samples with the Elan Bank basalts and Rajmahal (Group II) mafic dyke samples, suggest minor contamination by assimilation with a small amount of upper crustal material. Chemistry, particularly REE, hints at an alkaline basaltic nature of melt. Trace element modelling suggests that the melt responsible for these mafic dykes had undergone extreme differentiation (~ 50%) before its emplacement. The basaltic-andesite nature of these rocks may be attributed to this differentiation. Chemistry of these rocks also indicates ~ 10-15% melting of the mantle source. The mafic dyke samples of the present investigation show very close geochemical similarities with the mafic rocks derived from the Kerguelen mantle plume. Perhaps the Swangkre-Rongmil mafic dykes are also derived from the Kerguelen mantle plume

    Geochemical characteristics of mesoproterozoic metabasite dykes from the Chhotanagpur Gneissic Terrain, eastern India: implications for their emplacement in a plate margin tectonic environment

    A number of mafic intrusive bodies (mostly dykes) are exposed in the Chhotanagpur Gneissic Terrain (CGT). Most dykes trend in ENE–WSW to E–W following major structural trends of the region. These metabasite dykes show granoblastic to grano-nematoblastic textures and contain hornblende, plagioclase, chlorite, quartz and epidote which suggest their metamorphism under amphibolite grade P–T conditions. Although no radiometric age is available for the metabasite dykes, field relationships with host rock and available geochronology on granitoids suggest their emplacement during Mesoproterozoic. Geochemical characteristics of these dykes classify them as low-K tholeiite to medium-K calc-alkaline type. At least two types of metabasite dykes are recognized on the basis of their HFSE contents; one group shows entirely calc-alkaline nature, whereas the other group has rocks of tholeiite-calc-alkaline series. High Mg# observed in a number of samples indicates their derivation from primary melt. Multi-element spidergrams and rare-earth element patterns observed in these samples also corroborate their derivation from different magma batches. Trace element patterns observed for Nb–Ta, Hf–Zr, Sr and Y suggesting involvement of subduction related processes in the genesis of CGT metabasite dykes. Perceived geochemical characteristics suggest that metamorphism did not affect much on the chemistry of metabasites but source region, responsible for the generation of CGT metabasites, was possibly modified during subduction process. This study suggests that magma generated in a destructive plate setting fed the Mesoproterozoic mafic dykes of the CGT

    Bilateral phyllodes tumor of the breast in a young nulliparous woman

    Cystosarcoma phyllodes is a rare breast tumor with incidence of 1% of all the mammary tumors. Bilateral occurrence is very rare. Median age of presentation is 40-50 years. We present a case of 24 years old nulliparous female with phyllodes tumour developing in both the breasts one after another with a gap of five years. Patient underwent simple mastectomy on both sides. Histopathology report confirmed benign variety of cystosarcoma phyllodes on both sides

    A study on drug prescribing pattern in psychiatry out-patient department from a tertiary care teaching hospital

    Background: The burden of illness resulting from psychiatric and behavioral disorders is enormous; although, it remains grossly under represented by conventional public health statistics, which focus on mortality rather than the morbidity or dysfunction. Therefore, the study was carried out to find out morbidity pattern of psychiatric diseases and prescribing trends of psychotropic drugs.Methods: A prospective observational study was conducted in psychiatry out-patient department of a tertiary care hospital for 3 months. Diagnosis was made according to ICD-10 criteria. Prescription pattern was analyzed using World Health Organization (WHO) drug indicators.Results: Of 520 patients analyzed, 52.31% were males and 47.69% were females, 72.31% were <45 years. Depression 42.88%, schizophrenia 23.08%, bipolar mood disorders 17.88%, and anxiety 9.04% accounted for 92.88% of cases. Among the total of 1092 psychotropic drugs prescribed, 1056 (96.7%) were oral formulations. Average number of psychotropic drugs per prescription was 2.1 ± 0.8. 20.58% of the prescriptions contained fixed dose combinations. About 28.75% drugs were prescribed by generic name. Utilization from the WHO and National essential medical list were 28.57% and 38.64%, respectively. The prescribing frequency of anxiolytics, anti-depressants, anti-psychotics, anti-cholinergics, and anti-mania drugs was 30.04% (328/1092), 25.46% (278/1092), 25.37% (277/1092), 11.54% (126/1092), and 7.6% (83/1092), respectively. Clonazepam, olanzapine, escitalopram, carbamazepine and trihexyphenidyl were the most commonly prescribed benzodiazepines (BZD), antipsychotic, antidepressants, mood stabilizer and anti-cholinergics, respectively.Conclusion: Utilization pattern of the psychotropic drugs were in accordance to the recommendations of various treatment guidelines. BZD were the most commonly prescribed psychotropic drugs

    Pharmacoepidemiological survey of schizophrenia in Central India

    Background:Schizophrenia is a chronic and debilitating psychiatric illness affecting around 0.3-0.7% of people at some point in their life. The rate of schizophrenia and related disorders is affected by some environmental factors and social variables. Therefore, pharmacoepidemiological survey of patients suffering from schizophrenia was carried out to analyze the sociodemographic profile and drug prescribing pattern.Methods:A prospective observational study was conducted in psychiatry OPD of a tertiary care hospital for nine months. Diagnosis of schizophrenia was made according to DSM IV-TR criteria. Prescriptions were analyzed for socio demographic details, distribution of subsets of disease and psychotropic drugs prescribed.Results:Amongst 196 cases analyzed, 55.61% were males, 69.9% were below 40 years, 52.04% unmarried, 61.22% belonged to low income group, 82.14% unemployed and 58.16% came from urban locality. Paranoid schizophrenia (79.59%) was the most common diagnosis and a total of 402 psychotropic drugs were prescribed. Average number of psychotropic drugs per prescription was 2.05. Atypical antipsychotics (80.09%) were prescribed more commonly than typical antipsychotics; olanzapine (42.48%) was the commonest antipsychotic drug followed by risperidone (21.68%), haloperidol (19.91%), quetiapine (7.96%), aripiprazole (4.42%) and clozapine (3.54%). As an adjunctive treatment escitalopram, clonazepam and carbamazepine were the commonly prescribed antidepressant, anxiolytic and antimanic agent respectively.Conclusion:Low socioeconomic status, unemployment, urban locality and living alone are the sociodemographic factors associated with schizophrenia. The treatment pattern observed correlates with the changing trends in the treatment of schizophrenia world over.

    Enhancement and modulation of cosmic noise absorption in the afternoon sector at subauroral location ( L  = 5) during the recovery phase of 17 March 2015 geomagnetic storm

    The present study has focused on the intense production of cosmic noise absorption (CNA) at Maitri, Antarctica (L = 5; CGM −62°S, 55°E) during the early recovery phase of the largest storm of the current solar cycle commenced on 17 March 2015 St. Patrick's Day. The enhancement of CNA during 15–18 UT (14–17 magnetic local time (MLT); MLT = UT − 1 at Maitri) was as large as the CNA enhancement occurred during the main phase of the storm. During this time the CNA pattern also exhibits oscillation in the Pc5 (2–7 mHz) range and is in simultaneity with geomagnetic pulsations in the same frequency range. We observed the amplitude of CNA pulsation is well correlated with the level of CNA production. High-amplitude Pc5 oscillations were observed in the vicinity of auroral oval near Maitri. Absence of electromagnetic ion cyclotron (EMIC) waves is marked suggesting the possible role of VLF waves in precipitation. The reason for the intense CNA production is found to be the precipitation caused mainly by hiss-driven subrelativistic electrons. The CNA enhancement event is located well inside the dusk plasmaspheric bulge region as suggested by Tsurutani et al. (2015). Signature of enhanced eastward electrojet at Maitri during 14–17 MLT could be an additional factor for such large CNA. In order to establish the cause and effect relationship between the geomagnetic and CNA oscillations at Maitri, transfer entropy method has been used, which confirmed the modulation of CNA by geomagnetic pulsations

    GLP-1 Receptor Agonists Critical Review: Revisiting Its Positioning for Type 2 Diabetes Mellitus in Routine Clinical Practice in India

    Objective: Despite the benefit–risk ratio favoring glucagon-like peptide-1 receptor agonists (GLP-1 RAs), knowledge and awareness is lacking among patients and physicians, particularly in India. The current review provides an overview of GLP-1 RAs and the opinion of a group of healthcare practitioners (HCPs) and independent consultants across India on the evidence for using GLP-1 RAs and its applicability to the Indian population. Materials and methods: A panel of eight HCPs met virtually on December 12–13, 2020 met as part of the Diabetes Research Society (DIABAID). They examined and critically discussed the current research on the use of GLP-1 RAs in the management of T2DM. Results: The panel observed that recent diabetes guidelines and recommendations have shifted toward a more individualised and CV risk-focused approach to T2DM management. They proposed that 1) GLP-1 RAs are ideal cardio-metabolic drugs that address multiple aspects of the T2DM; 2) to bring up GLP-1 RAs as early treatment option in discussions with patients; 3) in T2DM patients with a high CV risk or established ASCVD, CKD, or HF, GLP-1 RAs with proven CVD benefits should be initiated; 4) including oral semaglutide in international treatment recommendation guidelines to improve patient and HCP understanding and adaptability; and 5) patient-physician dialogues will be critical in incorporating GLP-1 RAs earlier in the treatment paradigm for effective T2DM management. Conclusions: The recommendations on using GLP-1 RAs and the associated benefits and risks of these drugs comprise essential considerations for using such medications in the Indian population

    Diaphragmatic rupture with right colon and small intestine herniation after blunt trauma: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Traumatic diaphragmatic hernias are an unusual presentation of trauma, and are observed in about 10% of diaphragmatic injuries. The diagnosis is often missed because of non-specific clinical signs, and the absence of additional intra-abdominal and thoracic injuries.</p> <p>Case presentation</p> <p>We report a case of a 59-year-old Italian man hospitalized for abdominal pain and vomiting. His medical history included a blunt trauma seven years previously. A chest X-ray showed right diaphragm elevation, and computed tomography revealed that the greater omentum, a portion of the colon and the small intestine had been transposed in the hemithorax through a diaphragm rupture. The patient underwent laparotomy, at which time the colon and small intestine were reduced back into the abdomen and the diaphragm was repaired.</p> <p>Conclusions</p> <p>This was a unusual case of traumatic right-sided diaphragmatic hernia. Diaphragmatic ruptures may be revealed many years after the initial trauma. The suspicion of diaphragmatic rupture in a patient with multiple traumas contributes to early diagnosis. Surgical repair remains the only curative treatment for diaphragmatic hernias. Prosthetic patches may be a good solution when the diaphragmatic defect is severe and too large for primary closure, whereas primary repair remains the gold standard for the closure of small to moderate sized diaphragmatic defects.</p
