Systematization of reconstruction of the abdominal wall after reconstruction with TRAM
Introduction: Tram (transverse abdominal rectum flap) has remained the most used autologous breast reconstruction method over the last 30 years. First described by Holmström, the flap allows breast reconstruction with analogous tissue, providing natural appearance and consistency and lasting results. Reconstruction of the abdominal wall of the defect installed after flap transposition is a major challenge, and there is no consensus on the form for elevation or closure of the abdominal wall flap. The article aims to present a practical method for abdominal wall reconstructions to reduce morbidity in patients undergoing breast reconstruction with TRAM.
Methods: This is a descriptive work of a systematic abdominal wall reconstruction technique using propylene mesh.
Result: Once the technique is applied, we have an abdominal wall covered with polypropylene fabric, fully fixed and well adapted. The systematization of abdominal wall reconstruction after reconstruction with TRAM was performed, which is characterized by being easy to reproduce and applicable.
Conclusion: The technique is a good alternative in abdominal wall reconstructions for the surgeon, systematizing polypropylene mesh adaptation
Clinical And Surgical Anatomy Of Lumbar Hernia: A Review
Lumbar hernia is defined as the presence of failure in the transverse fascia or in the aponeurosis of the transverse abdominal muscle that results in the extrusion of intra or extra peritoneal organs through the discontinuity of the postero lateral abdominal wall. The aim of this study was to conduct a methodical review of the anatomy of the hernia form grynfelt dated from 2006 to 2017. For this, we performed a bibliographic review by means of electronic databases like SciELO, PubMed, Science Direct, LILACS and Bireme to get better approach to the subject. It has been found that the lumbar hernia is a disease little known by doctors whose diagnostics are often performed in the wrong way and for surgical correction needs a good anatomical knowledge. Lumbar hernias, although rare, must be taken into account, since ischemia of herniated intestinal segments can lead to the death of the patient, especially in the elderly. Knowledge about the anatomy of the lumbar region is of vital importance because it makes surgery safe and reduces risks of complications and recidivating of the hernia
Eletrocardiograma: um estudo acerca da sua utilização na triagem de atletas para práticas esportivas
Studies have shown that the inclusion of ECG in pre-participation screening for young athletes was effective in diagnosing cardiovascular conditions, such as arrhythmogenic dysplasias and hypertrophic cardiomyopathies. However, there are several limitations to the use of ECG. The objective of this review is to seek updates on the use of the ECG in the screening and evaluation of athletes, in order to elucidate the importance, impacts, benefits and limitations of performing the test in this group of people. With the results of this work, it is intended to contribute to Sports Medicine, as well as to the specialty of Cardiology. In order to investigate aspects of the use of electrocardiogram for the screening of athletes, a systematic review of the literature was carried out using the PubMed electronic database, a reliable source of information. It can be observed, with the analysis of the results of this study, that the ECG is an important test for the detection of cardiac alterations in athletes, in addition to its easy access and considerable cost-benefit. However, the lack of standardization for interpretation, the scarcity of databases in some countries and populations, and the lack of medical education on the interpretation of electrocardiograms are strong factors that limit the use of the test.Estudos mostraram que a inclusão do ECG na triagem de pré-participação para atletas jovens foi efetiva no diagnóstico de condições cardiovasculares, como displasias arritmogênicas e cardiomiopatias hipertróficas. No entanto, existem diversas limitações acerca da utilização do ECG. O objetivo desta revisão é buscar atualizações acerca da utilização do ECG na triagem e avaliação dos atletas, de forma a elucidar a importância, os impactos, os benefícios e as limitações da realização do exame neste grupo de pessoas. Com os resultados deste trabalho, pretende-se contribuir com a Medicina do Esporte, bem como com a especialidade da Cardiologia. Com o objetivo de investigar aspectos acerca da utilização do eletrocardiograma para a triagem dos atletas, realizou-se uma revisão sistemática da literatura, utilizando a base de dados eletrônica PubMed, uma fonte de informação confiável. Pode-se observar, com a análise dos resultados deste estudo, que o ECG é um importante exame para a detecção de alterações cardíacas em atletas, além do seu fácil acesso e considerável custo-benefício. No entanto, a falta de padronização para a interpretação, a escassez de base de dados de alguns países e populações e a deficiência do ensino médico acerca da interpretação dos eletrocardiogramas são fortes fatores que limitam a utilização do exame
Giant calyceal diverticulum with an extra-renal component: A case report
Calyceal diverticula are urine-filled non-secretory cavities that result from outpouchings of the kidney's calyx or pelvis. These cavities lie in the renal parenchyma and are linked to the kidney's collecting system through a narrow channel. They are generally small in size and present without symptoms. Here, we report the case of a middle-aged patient who was diagnosed, following imaging exams, with a giant calyceal diverticulum presenting with an extra-renal component, a remarkably rare finding. The patient's condition was successfully treated through excision by laparoscopic surgery
