The Use of Turning and Repositioning Versus Pressure Redistributing Support Surfaces in the Prevention of Pressure Ulcers
Currently, 1.3-3 million adults in the United States are affected by pressure ulcers, costing 70,000 per ulcer (Smith, 2013). This costs the United States 11 billion dollars annually (Smith, 2013). This reviewâs PICO question is âIn hospitalized critically ill patients, how does turning and repositioning every two to four hours compared to the use of pressure redistributing support surfaces prevent the occurrence of pressure ulcers?â For this review, the articles found were rated as excellent (n=7), good (n=2), and fair (n=1). Appendix A shows the critical appraisal of all pertinent articles used. Findings suggest that there is minimal statistically significant evidence that the use of one intervention is more effective than another (pressure redistributing support surfaces versus turning and repositioning every two to four hours) (Bergstrom, 2013; Chou, 2013; Huang, 2013; Manzano, 2013; Manzano, 2014; Rich, 2011b; Smith, 2013). Furthermore, findings indicate that when both interventions are used together, pressure ulcer prevention is increased (Chou, 2013; Rich, 2011a; Smith, 2013)

Cavity Enhancement of Single Quantum Dot Emission in the Blue.
Cavity-enhanced single-photon emission in the blue spectral region was measured from single InGaN/GaN quantum dots. The low-Q microcavities used were characterized using micro-reflectance spectroscopy where the source was the enhanced blue output from a photonic crystal fibre. Micro-photoluminescence was observed from several cavities and found to be ~10 times stronger than typical InGaN quantum dot emission without a cavity. Influence of providers and nurses on completion of non-targeted HIV screening in an urgent care setting
Introduction: Despite recommendations by the Centers for Disease Control (CDC) that all adults be offered non-targeted HIV screening in all care settings, screening in acute-care settings remains unacceptably low. We performed an observational study to evaluate an HIV screening pilot in an academic-community partnership health center urgent care clinic. Methods: We collected visit data via encounter forms and demographic and laboratory data from electronic medical records. A post-pilot survey of perceptions of HIV screening was administered to providers and nurses. Multivariable analysis was used to identify factors associated with completion of testing. Results: Visit provider and triage nurse were highly associated with both acceptance of screening and completion of testing, as were younger age, male gender, and race/ethnicity. 23.5% of patients completed tests, although 36.0% requested screening; time constraints as well as risk perceptions by both the provider and patient were cited as limiting completion of screening. Post-pilot surveys showed mixed support for ongoing HIV screening in this setting by providers and little support by nurses. Conclusions: Visit provider and triage nurse were strongly associated with acceptance of testing, which may reflect variable opinions of HIV screening in this setting by clinical staff. Among patients accepting screening, visit provider remained strongly associated with completion of testing. Despite longstanding recommendations for non-targeted HIV screening, further changes to improve the testing and results process, as well as provider education and buy-in, are needed to improve screening rates

Summary Report for the 2005-2006 STATEMAP Project: Geologic Mapping to Support Improved Database Development and Understanding of Urban Corridors, Critical Acquifers. And Special Areas of Environmental Concern in Texas
Eleven geologic maps, 1:24,000 scale, have been constructed for Galveston and Mustang Barrier Islands and for part of the Brazos River Valley and its aquifers. The maps are intended to be used by professionals and laypersons as a source of general geologic information that relates to land and resource use and management. The geologic maps of the barrier islands include (a) Northern Mustang Island (Port Aransas quadrangle), (b) Southern Mustang Island (Crane Islands NW quadrangle), (c) Northeastern Galveston Island (Galveston quadrangle), (d) Central Galveston Island (Lake Como quadrangle), and (e) Southeastern Galveston Island (Sea Isle and San Luis Pass quadrangles). These maps display island wetland and upland geologic environments. Geologic maps of the Brazos River Valley study area include six quadrangles: Baileyville, Hammond, Maysfield, Calvert, Gause, and Hearne South. These maps and cross sections show the geologic framework of the Brazos alluvial aquifer in an area where it intersects three other Texas aquifers: the Carrizo-Wilcox, Queen City, and Sparta.Bureau of Economic Geolog
Environmental DNA captures elasmobranch diversity in a temperate marine ecosystem
Abstract: Many sharks, skates, and rays (elasmobranchs) are highly threatened by the activities of commercial fisheries, and a clear understanding of their distributions, diversity, and abundance can guide protective measures. However, surveying and monitoring elasmobranch species can be highly invasive or resourceâintensive, and utilization of nonâinvasive environmental DNAâbased methods may overcome these problems. Here, we studied spatial and seasonal variation in the elasmobranch community of the Western English Channel using environmental DNA (eDNA) collected from surface and bottom waters periodically over an annual cycle (2017â2018). In total we recovered 13 elasmobranch species within eDNA samples, and the number of transformed eDNA reads was positively associated with species (hourly) catch data resolved from 105âyear time series trawl data (1914â2018). These results demonstrate the ability of eDNA to detect and semiâquantitatively reflect the prevalence of historically dominant and rare elasmobranch species in this region. Notably, eDNA recorded a greater number of species per sampling event than a conventional trawl survey in the same area over the same sampling years (2017â2018). Several threatened species were recovered within the eDNA, including undulate ray, porbeagle shark, and thresher shark. Using eDNA, we found differences in elasmobranch communities among sampling stations and between seasons, but not between sampling depths. Collectively, our results suggest that nonâinvasive eDNAâbased methods can be used to study the spatial and seasonal changes in the diversity and abundance of whole elasmobranch communities within temperate shelf habitats. Given the threatened status of many elasmobranchs in humanâimpacted marine environments, eDNA analysis is poised to provide key information on their diversity and distributions to inform conservationâfocused monitoring and management
The HIKCUPS trial: a multi-site randomized controlled trial of a combined physical activity skill-development and dietary modification program in overweight and obese children
BACKGROUND: Childhood obesity is one of the most pressing health issues of our time. Key health organizations have recommended research be conducted on the effectiveness of well-designed interventions to combat childhood obesity that can be translated into a variety of settings. This paper describes the design and methods used in the Hunter Illawarra Kids Challenge Using Parent Support (HIKCUPS) trial, an ongoing multi-site randomized controlled trial, in overweight/obese children comparing the efficacy of three interventions: 1) a parent-centered dietary modification program; 2) a child-centered physical activity skill-development program; and 3) a program combining both 1 and 2 above. METHODS/DESIGN: Each intervention consists of three components: i) 10-weekly face-to-face group sessions; ii) a weekly homework component, completed between each face-to-face session and iii) three telephone calls at monthly intervals following completion of the 10-week program. Details of the programs' methodological aspects of recruitment, randomization and statistical analyses are described here a priori. DISCUSSION: Importantly this paper describes how HIKCUPS addresses some of the short falls in the current literature pertaining to the efficacy of child obesity interventions. KELT-7b: A hot Jupiter transiting a bright V=8.54 rapidly rotating F-star
We report the discovery of KELT-7b, a transiting hot Jupiter with a mass of
MJ, radius of RJ, and an orbital
period of days. The bright host star (HD33643;
KELT-7) is an F-star with , Teff K, [Fe/H]
, and . It has a mass of
Msun, a radius of Rsun, and
is the fifth most massive, fifth hottest, and the ninth brightest star known to
host a transiting planet. It is also the brightest star around which KELT has
discovered a transiting planet. Thus, KELT-7b is an ideal target for detailed
characterization given its relatively low surface gravity, high equilibrium
temperature, and bright host star. The rapid rotation of the star (
km/s) results in a Rossiter-McLaughlin effect with an unusually large amplitude
of several hundred m/s. We find that the orbit normal of the planet is likely
to be well-aligned with the stellar spin axis, with a projected spin-orbit
alignment of degrees. This is currently the second most
rapidly rotating star to have a reflex signal (and thus mass determination) due

Munc18-1 is a molecular chaperone for α-synuclein, controlling its self-replicating aggregation
Munc 18-1 is a key component of the exocytic machinery that controls neurotransmitter release. Munc 18-1 heterozygous mutations cause developmental defects and epileptic phenotypes, including infantile epileptic encephalopathy (EIEE), suggestive of a gain of pathological function. Here, we used single-molecule analysis, gene-edited cells, and neurons to demonstrate that Munc 18-1 EIEE-causing mutants form large polymers that coaggregate wild-type Munc 18-1 in vitro and in cells. Surprisingly, Munc 18-1 EIEE mutants also form Lewy body like structures that contain a-synuclein (alpha-Syn). We reveal that Munc 18-1 binds alpha-Syn, and its EIEE mutants coaggregate alpha-Syn. Likewise, removal of endogenous Munc 18-1 increases the aggregative propensity of alpha-Syn(wT) and that of the Parkinson's disease-causing a-Syn(A30P) mutant, an effect rescued by Munc18-1(WT) expression, indicative of chaperone activity. Coexpression of the alpha-Syn(A30P) mutant with Munc 18-1 reduced the number of alpha-Syn(A30P) aggregates. Munc 18-1 mutations and haploinsufficiency may therefore trigger a pathogenic gain of function through both the corruption of native Munc 18-1 and a perturbed chaperone activity for a-Syn leading to aggregation-induced neurodegeneration
KELT-3b: A Hot Jupiter Transiting a V=9.8 Late-F Star
We report the discovery of KELT-3b, a moderately inflated transiting hot
Jupiter with a mass of 1.477 (-0.067, +0.066) M_J, and radius of 1.345 +/-
0.072 R_J, with an orbital period of 2.7033904 +/- 0.000010 days. The host
star, KELT-3, is a V=9.8 late F star with M_* = 1.278 (-0.061, +0.063) M_sun,
R_* = 1.472 (-0.067, +0.065) R_sun, T_eff = 6306 (-49, +50) K, log(g) = 4.209
(-0.031, +0.033), and [Fe/H] = 0.044 (-0.082, +0.080), and has a likely proper
motion companion. KELT-3b is the third transiting exoplanet discovered by the
KELT survey, and is orbiting one of the 20 brightest known transiting planet
host stars, making it a promising candidate for detailed characterization
studies. Although we infer that KELT-3 is significantly evolved, a preliminary
analysis of the stellar and orbital evolution of the system suggests that the
planet has likely always received a level of incident flux above the
empirically-identified threshold for radius inflation suggested by Demory &

KELT-1b: A Strongly Irradiated, Highly Inflated, Short Period, 27 Jupiter-mass Companion Transiting a mid-F Star
We present the discovery of KELT-1b, the first transiting low-mass companion
from the wide-field Kilodegree Extremely Little Telescope-North (KELT-North)
survey. The V=10.7 primary is a mildly evolved, solar-metallicity, mid-F star.
The companion is a low-mass brown dwarf or super-massive planet with mass of
27.23+/-0.50 MJ and radius of 1.110+0.037-0.024 RJ, on a very short period
(P=1.21750007) circular orbit. KELT-1b receives a large amount of stellar
insolation, with an equilibrium temperature assuming zero albedo and perfect
redistribution of 2422 K. Upper limits on the secondary eclipse depth indicate
that either the companion must have a non-zero albedo, or it must experience
some energy redistribution. Comparison with standard evolutionary models for
brown dwarfs suggests that the radius of KELT-1b is significantly inflated.
Adaptive optics imaging reveals a candidate stellar companion to KELT-1, which
is consistent with an M dwarf if bound. The projected spin-orbit alignment
angle is consistent with zero stellar obliquity, and the vsini of the primary
is consistent with tidal synchronization. Given the extreme parameters of the
KELT-1 system, we expect it to provide an important testbed for theories of the
emplacement and evolution of short-period companions, and theories of tidal
