Rising burden of Hepatitis C Virus in hemodialysis patients
<p>Abstract</p> <p>Aim</p> <p>High prevalence of Hepatitis C virus (HCV) has been reported among the dialysis patients throughout the world. No serious efforts were taken to investigate HCV in patients undergoing hemodialysis (HD) treatment who are at great increased risk to HCV. HCV genotypes are important in the study of epidemiology, pathogenesis and reaction to antiviral therapy. This study was performed to investigate the prevalence of active HCV infection, HCV genotypes and to assess risk factors associated with HCV genotype infection in HD patients of Khyber Pakhtunkhwa as well as comparing this prevalence data with past studies in Pakistan.</p> <p>Methods</p> <p>Polymerase chain reaction was performed for HCV RNA detection and genotyping in 384 HD patients. The data obtained was compared with available past studies from Pakistan.</p> <p>Results</p> <p>Anti HCV antibodies were observed in 112 (29.2%), of whom 90 (80.4%) were HCV RNA positive. In rest of the anti HCV negative patients, HCV RNA was detected in 16 (5.9%) patients. The dominant HCV genotypes in HCV infected HD patients were found to be 3a (n = 36), 3b (n = 20), 1a (n = 16), 2a (n = 10), 2b (n = 2), 1b (n = 4), 4a (n = 2), untypeable (n = 10) and mixed (n = 12) genotype.</p> <p>Conclusion</p> <p>This study suggesting that i) the prevalence of HCV does not differentiate between past and present infection and continued to be elevated ii) HD patients may be a risk for HCV due to the involvement of multiple routes of infections especially poor blood screening of transfused blood and low standard of dialysis procedures in Pakistan and iii) need to apply infection control practice.</p
Risk of Kaposi's sarcoma and of other cancers in Italian renal transplant patients
A follow-up study of 1844 renal transplant patients in Italy showed a 113-fold increased risk for Kaposi's sarcoma. Kaposi's sarcoma risk was higher in persons born in southern than in northern Italy. Significant increases were also observed for cancers of the lip, liver, kidney and for non-Hodgkin's lymphoma
Patterns of hepatitis C prevalence and seroconversion in hemodialysis units from three continents: The DOPPS
Background. Hepatitis C virus (HCV) remains a problem within hemodialysis units. This study measures HCV prevalence and seroconversion rates across seven countries and investigates associations with facility-level practice patterns.
Methods. The study sample was from the Dialysis Outcomes and Practice Patterns Study (DOPPS), a prospective, observational study of adult hemodialysis patients randomly selected from 308 representative dialysis facilities in France, Germany, Italy, Japan, Spain, the United Kingdom, and the United States. Logistic regression was used to model odds of HCV prevalence, and Cox regression was used to model time from study entry to HCV seroconversion.
Results. Mean HCV facility prevalence was 13.5% and varied among countries from 2.6% to 22.9%. Increased HCV prevalence was associated with longer time on dialysis, male gender, black race, diabetes, hepatitis B (HBV) infection prior renal transplant, and alcohol or substance abuse in the previous 12 months. Approximately half of the facilities (55.6%) had no seroconversions during the study period. HCV seroconversion was associated with longer time on dialysis, human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS), HBV infection, and recurrent cellulitis or gangrene. An increase in highly trained staff was associated with lower HCV prevalence (OR=0.93 per 10% increase, P=0.003) and risk of seroconversion (RR=0.92, P=0.07). Seroconversion was associated with an increase in facility HCV prevalence (RR=1.36, P<0.0001), but not with isolation of HCV-infected patients (RR=1.01, P=0.99).
Conclusion. There are differences in HCV prevalence and rate of seroconversion at the country and the hemodialysis facility level. The observed variation suggests opportunities for improved HCV outcomes
Hepatitis B and C infection in haemodialysis patients in Libya: prevalence, incidence and risk factors
Background: Patients receiving maintenance haemodialysis (HD) are at higher risk for acquiring Hepatitis B Virus
(HBV) and Hepatitis C Virus (HCV) infections than the general population. Strict infection control measures are
essential to prevent nosocomial transmission. We aimed to investigate the incidence and prevalence of HBV and
HCV infection in the HD population of Libya as well as risk factors for infection.
Methods: All adult patients receiving maintenance HD (n=2382) in Libyan dialysis centres (n=39) were studied
between May 2009 and October 2010. Testing for Hepatitis B surface antigen (HBsAg) and anti-HCV antibodies was
performed at initiation of dialysis and every 3–6 months thereafter. Patients who were sero-negative for HBV and
HCV (n=1160) were followed up for 1 year to detect sero-conversions.
Results: Participant median age was 49 years and 58% were male. 831 patients (34.9%) were sero-positive for HBV
and/or HCV (anti-HCV positive 31.1%; HBsAg positive 2.6%; both positive 1.2%). Of the sero-positive patients
4.7% were known to be infected before the initiation of HD. The prevalence of HBV±HCV infection varied widely
between HD centres from 0% to 75.9%. Sero-positive patients were younger, had longer time on dialysis and more
previous blood transfusions. Prospective follow-up revealed an incidence of sero-conversion of 7.7% during 1 year
(7.1% HCV; 0.6% HBV). Wide variation in rates of newly acquired infections was observed between dialysis centres.
All new HBV cases were referred from centres already treating HBV infected patients. New HCV infections were
reported in most centres but the rate of HCV sero-conversion varied widely from 1.5% to 31%. Duration of dialysis,
history of previous renal transplant and history of receiving HD in another centre in Libya were significantly
associated with sero-conversion.
Conclusion: Patients on maintenance HD in Libya have a high incidence and prevalence of HCV infection and
lower rates of HBV infection. The factors associated with HBV and HCV infection are highly suggestive of
nosocomial transmission within HD units. Urgent action is required to improve infection control measures in HD
centres and to reduce dependence on blood transfusions for the treatment of anaemia.
