    Risk and protective factors at age 10: Psychological adjustment in children with a cleft lip and/or Palate

Objective: To explore psychological functioning in children with a cleft at age 10 from a broad perspective, including cognitive, emotional, behavioral, appearance-related, and social adjustment. High-risk groups were identified within each area of adjustment to investigate whether vulnerable children were found across domains or whether risk was limited to specific areas of adjustment. Methods: Retrospective chart review from psychological assessments at age 10 (N = 845). The effects of gender, cleft visibility, and the presence of an additional condition were investigated. Results were compared with large national samples. Measures: Personality Inventory for Children, Child Experience Questionnaire, Strengths and Difficulties Questionnaire, Satisfaction With Appearance scale. Results: The factor affecting psychological adjustment on most domains was the presence of an associated condition in addition to the cleft. As expected, no support was found for cleft visibility as a risk factor, while there were some gender differences related to emotional difficulties and attention. Correlation analyses of risk groups pointed to an association between social experiences and emotional adjustment and between social and behavioral adjustment; whereas, dissatisfaction with appearance was not related to any other domains of risk at age 10. Conclusions: The results point to the importance of early screening and assessment of children born with a cleft to identify possible associated conditions and offer adapted and appropriate treatment and care. Future research should investigate how protective factors could counteract potential risk in children with a cleft

    Continuous twin screw rheo-extrusion of an AZ91D magnesium alloy

    © The Minerals, Metals & Materials Society and ASM International 2012The twin screw rheo-extrusion (TSRE) is designed to take advantage of the nondendritc microstructure and thixotropic characterization of semisolid-metal slurries and produce simple metal profiles directly from melts. The extrusion equipment consists of a rotor-stator high shear slurry maker, a twin screw extruder, and a die assembly. The process is continuous and has a potential for significantly saving energy, manufacturing cost, and enhancing efficiency. The present investigation was carried out to study the process performance for processing rods of an AZ91D magnesium alloy and the microstructure evolution during processing. The semisolid slurry prepared by the process was characterized by uniformly distributed nondendritic granular primary phase particles. AZ91D rods with uniform and fine microstructures and moderate mechanical properties were produced. For the given slurry making parameters, decreasing extrusion temperature was found to improve microstructures and properties.     Theoretical overview on high-energy emission in microquasars

    Microquasar (MQ) jets are sites of particle acceleration and synchrotron emission. Such synchrotron radiation has been detected coming from jet regions of different spatial scales, which for the instruments at work nowadays appear as compact radio cores, slightly resolved radio jets, or (very) extended structures. Because of the presence of relativistic particles and dense photon, magnetic and matter fields, these outflows are also the best candidates to generate the very high-energy (VHE) gamma-rays detected coming from two of these objects, LS 5039 and LS I +61 303, and may be contributing significantly to the X-rays emitted from the MQ core. In addition, beside electromagnetic radiation, jets at different scales are producing some amount of leptonic and hadronic cosmic rays (CR), and evidences of neutrino production in these objects may be eventually found. In this work, we review on the different physical processes that may be at work in or related to MQ jets. The jet regions capable to produce significant amounts of emission at different wavelengths have been reduced to the jet base, the jet at scales of the order of the size of the system orbital semi-major axis, the jet middle scales (the resolved radio jets), and the jet termination point. The surroundings of the jet could be sites of multiwavelegnth emission as well, deserving also an insight. We focus on those scenarios, either hadronic or leptonic, in which it seems more plausible to generate both photons from radio to VHE and high-energy neutrinos.     Dynamic Vortex Phases and Pinning in Superconductors with Twin Boundaries

    We investigate the pinning and driven dynamics of vortices interacting with twin boundaries using large scale molecular dynamics simulations on samples with near one million pinning sites. For low applied driving forces, the vortex lattice orients itself parallel to the twin boundary and we observe the creation of a flux gradient and vortex free region near the edges of the twin boundary. For increasing drive, we find evidence for several distinct dynamical flow phases which we characterize by the density of defects in the vortex lattice, the microscopic vortex flow patterns, and orientation of the vortex lattice. We show that these different dynamical phases can be directly related to microscopically measurable voltage - current V(I) curves and voltage noise. By conducting a series of simulations for various twin boundary parameters we derive several vortex dynamic phase diagrams.Comment: 5 figures, to appear in Phys. Rev.

    The Green, Green Grass of Home: an archaeo-ecological approach to pastoralist settlement in central Kenya

This paper considers the ecological residues of pastoralist occupation at the site of Maili Sita in Laikipia, central Kenya, drawing links with the archaeological record so as to contribute a fresh approach to the ephemeral settlement sites of mobile herding communities, a methodological aspect of African archaeology that remains problematic. Variations in the geochemical and micromorphological composition of soils along transects across the site are compared with vegetation distributions and satellite imagery to propose an occupation pattern not dissimilar to contemporary Cushitic-speaking groups further north.     Cancer care for people with dementia: literature overview and recommendations for practice and research

    As many countries experience population aging, patients with cancer are becoming older and have more preexisting comorbidities, which include prevalent, age-related, chronic conditions such as dementia. People living with dementia (PLWD) are vulnerable to health disparities, and dementia has high potential to complicate and adversely affect care and outcomes across the cancer trajectory. This report offers an overview of dementia and its prevalence among patients with cancer and a summary of the research literature examining cancer care for PLWD. The reviewed research indicates that PLWD are more likely to have cancer diagnosed at an advanced stage, receive no or less extensive cancer treatment, and have poorer survival after a cancer diagnosis. These cancer disparities do not necessarily signify inappropriately later diagnosis or lower treatment of people with dementia as a group, and they are arguably less feasible and appropriate targets for care optimization. The reviewed research indicates that PLWD also have an increased risk of cancer-related emergency presentations, lower quality processes of cancer-related decision making, accessibility-related barriers to cancer investigations and treatment, higher experienced treatment burden and higher caregiver burden for families, and undertreated cancer-related pain. The authors propose that optimal cancer care for PLWD should focus on proactively minimizing these risk areas and thus must be highly person-centered, with holistic decision making, individualized reasonable adjustments to practice, and strong inclusion and support of family carers. Comprehensive recommendations are made for clinical practice and future research to help clinicians and providers deliver best and equitable cancer care for PLWD and their families

    Search for the Decay τ4pi3π+(π0)ντ\tau^{-}\to 4pi^{-}3\pi^{+}(\pi^{0})\nu_{\tau}

    We have searched for the decay of the tau lepton into seven charged particles and zero or one pi0. The data used in the search were collected with the CLEO II detector at the Cornell Electron Storage Ring (CESR) and correspond to an integrated luminosity of 4.61 fb^(-1). No evidence for a signal is found. Assuming all the charged particles are pions, we set an upper limit on the branching fraction, B(tau- -> 4pi- 3pi+ (pi0) nu_tau) < 2.4 x 10^(-6) at the 90% confidence level.     Planck 2015 results. XXVII. The Second Planck Catalogue of Sunyaev-Zeldovich Sources

    We present the all-sky Planck catalogue of Sunyaev-Zeldovich (SZ) sources detected from the 29 month full-mission data. The catalogue (PSZ2) is the largest SZ-selected sample of galaxy clusters yet produced and the deepest all-sky catalogue of galaxy clusters. It contains 1653 detections, of which 1203 are confirmed clusters with identified counterparts in external data-sets, and is the first SZ-selected cluster survey containing > 10310^3 confirmed clusters. We present a detailed analysis of the survey selection function in terms of its completeness and statistical reliability, placing a lower limit of 83% on the purity. Using simulations, we find that the Y5R500 estimates are robust to pressure-profile variation and beam systematics, but accurate conversion to Y500 requires. the use of prior information on the cluster extent. We describe the multi-wavelength search for counterparts in ancillary data, which makes use of radio, microwave, infra-red, optical and X-ray data-sets, and which places emphasis on the robustness of the counterpart match. We discuss the physical properties of the new sample and identify a population of low-redshift X-ray under- luminous clusters revealed by SZ selection. These objects appear in optical and SZ surveys with consistent properties for their mass, but are almost absent from ROSAT X-ray selected samples
