    Design of a randomised controlled trial on immune effects of acidic and neutral oligosaccharides in the nutrition of preterm infants: carrot study

    <p>Abstract</p> <p>Background</p> <p>Prevention of serious infections in preterm infants is a challenge, since prematurity and low birth weight often requires many interventions and high utility of devices. Furthermore, the possibility to administer enteral nutrition is limited due to immaturity of the gastrointestinal tract in the presence of a developing immune system. In combination with delayed intestinal bacterial colonisation compared with term infants, this may increase the risk for serious infections. Acidic and neutral oligosaccharides play an important role in the development of the immune system, intestinal bacterial colonisation and functional integrity of the gut. This trial aims to determine the effect of enteral supplementation of acidic and neutral oligosaccharides on infectious morbidity (primary outcome), immune response to immunizations, feeding tolerance and short-term and long-term outcome in preterm infants. In addition, an attempt is made to elucidate the role of acidic and neutral oligosaccharides in postnatal modulation of the immune response and postnatal adaptation of the gut.</p> <p>Methods/Design</p> <p>In a double-blind placebo controlled randomised trial, 120 preterm infants (gestational age <32 weeks and/or birth weight <1500 gram) are randomly allocated to receive enteral acidic and neutral oligosaccharides supplementation (20%/80%) or placebo supplementation (maltodextrin) between day 3 and 30 of life. Primary outcome is infectious morbidity (defined as the incidence of serious infections). The role of acidic and neutral oligosaccharides in modulation of the immune response is investigated by determining the immune response to DTaP-IPV-Hib(-HBV)+PCV7 immunizations, plasma cytokine concentrations, faecal Calprotectin and IL-8. The effect of enteral acidic and neutral oligosaccharides supplementation on postnatal adaptation of the gut is investigated by measuring feeding tolerance, intestinal permeability, intestinal viscosity, and determining intestinal microflora. Furthermore, short-term and long-term outcome are evaluated.</p> <p>Discussion</p> <p>Especially preterm infants, who are at increased risk for serious infections, may benefit from supplementation of prebiotics. Most studies with prebiotics only focus on the colonisation of the intestinal microflora. However, the pathways how prebiotics may influence the immune system are not yet fully understood. Studying the immune modulatory effects is complex because of the multicausal risk of infections in preterm infants. The combination of neutral oligosaccharides with acidic oligosaccharides may have an increased beneficial effect on the immune system. Increased insight in the effects of prebiotics on the developing immune system may help to decrease the (infectious) morbidity and mortality in preterm infants.</p> <p>Trial registration</p> <p>Current Controlled Trials ISRCTN16211826.</p

    An Improved Biocompatible Probe for Photoacoustic Tumor Imaging Based on the Conjugation of Melanin to Bovine Serum Albumin

    A novel, highly biocompatible, well soluble melanin-based probe obtained from the conjugation of melanin macromolecule to bovine serum albumin (BSA) was tested as a contrast agent for photoacoustic tumor imaging. Five soluble conjugates (PheoBSA A-E) were synthesized by oxidation of dopamine (DA) in the presence of variable amounts of BSA. All systems showed the similar size and absorbance spectra, being PheoBSA D (DA:BSA ratio 1:2) the one showing the highest photoacoustic efficiency. This system was then selected for the investigations as it showed a marked enhancement of the photoacoustic (PA) contrast in the tumor region upon iv injection. Biodistribution studies confirmed the accumulation of PheoBSA D within the tumor region and showed fast renal elimination, lack of cell toxicity, and good hemocompatibility. A higher PA contrast enhancement was observed in the case of PC3 prostate tumor xenograft when compared to the TS/A breast one, likely reflecting different vascularization/extravasation properties between the two tumor murine models. The improved PA properties shown by PheoBSA D allowed to set up a 3D dynamic contrast-enhanced (DCE) approach that demonstrated a persistent increase of the PA signal in the tumor region for a long period. Overall, the herein reported results demonstrate that PheoBSA D is a promising blood pool contrast agent for in vivo PA imaging, particularly useful for the set-up of 3D DCE-PA approaches to monitor tumor vascular properties

    Advances in Geographic Information Systems and Remote Sensing technologies have the potential to revolutionize archaeological and paleontological fieldwork. Machine learning models have been effective in identifying conditions ideal for preservation, exposure, and discovery of fossils in a range of geographic contexts. Researchers working in the Koobi Fora Formation of northern Kenya have long inquired about the geographic patterning of extinct fauna and their respective paleoenvironments.     Persisting pan-institutional racism

    Which types of group-typing amounts to racism? The answer seemingly has to do with deeper physical or cultural traits over which an agent has no deliberate control but which are formative of the agent. In this article, I look to the cultural or ethnic bases of division of humans into races, albeit of a specific type: a basis that sees humanity climbing in a certain, presumably improving, direction. Those ethnicities that appear not to opt for this climb are commonly presumed – if tacitly – inferior. This outlook is so common, particularly in industrial, governmental, economic-developmental, academic and other intellectual and influential, high-profile institutional settings, it is hard to descry. This article aims to bring this racism, referred to as ‘pan-institutional’ because it extends from local to national and international institutions, into fuller light. The article argues that prominent pan-institutions, including government, academia and industry, work together to propagate this racism, even while these institutions aver they are spreading benevolence to less fortunate cultures, in the cause of cultural, economic and social globalization. In this way, these pan-institutions follow precisely in step with earlier colonialism and missionarism. The article ethically assesses this covert racist pan-institutional movement, reveals the quandaries that many indigenous cultures, especially foragers, face as this movement encroaches on them, and analyses how it is best understood as a refurbishing of colonialism in the name of benevolence. The argument ends by describing how such benevolent globalization builds upon misanthropy and it analyses the moral implications and why this most covert and widespread movement is, like more visible and standard racisms, wrong-headed, unethical, fatal, misanthropist and perhaps self-hating
