    Impact of Cognitive Impairment on Functional Outcome in Stroke

    The aim of this study was to investigate the effect of the cognitive impairment on functional status in patients with subacute stroke. Fifty-two patients with subacute stroke were included in the study. Mini mental state examination (MMSE) test was used for the evaluation of cognitive status. Patients were separated into two groups according to their cognitive functions. Functional follow-up parameters were activities of daily living (ADL), global recovery and ambulation status. All patients were evaluated on admission to rehabilitation unit, at discharge and 6 months after discharge. Forty-four patients were completed the study. Mean age was 66 and 57 years; disease duration on admission was 4,8 and 3,5 months in the cognitively impaired and normal groups, respectively. Significant improvement was found in terms of functional follow-up parameters in both groups at discharge (P < .05). Functional follow-up parameters did not show statistically significant difference between the groups. But community ambulation rate was higher in cognitively normal group at the sixth month visit. As a result of this study, inpatient rehabilitation was effective both cognitively normal and impaired subacute stroke patients

    Impact of chylothorax on the post operative outcome after pediatric cardiovascular surgery

    BackgroundChylothorax is the accumulation of chyle in the pleural cavity, usually develops after disruption of the thoracic duct along its intra-thoracic route. In the majority of cases this rupture is secondary to trauma (including cardio thoracic surgeries). Chylothorax is a potentially serious complication after cardiovascular surgeries that require early diagnosis and adequate management.MethodsA retrospective study of all cases complicated with chylothorax after pediatric cardiac surgery in King Abdulaziz Cardiac Center between January 2007 and December 2009 was conducted. The study aim to determine the risk factors, and the impact on the post operative course.ResultsWe have 1135 cases operated during the study period, 57 cases (5%) were complicated with chylothorax in the post operative period. The most common surgeries complicated with chylothorax were the single ventricle repair surgeries (Glenn-Fontan) 15 cases (27%), followed by the arch repair cases 10 cases (18%), the ventricular septal defect cases 10 cases (18%), the Atrioventricular septal defect cases 7 cases (12%), the arterial switch cases 6 cases (11%), and others 8 cases (14%). The ICU stay, the length of hospital stay and the bypass time were significantly longer in the chylothorax group, also the ventilation time, the inotropes duration and number were higher in the chylothorax group.ConclusionChylothorax after pediatric open heart surgery is not an uncommon complication, it occurs more commonly with single ventricle repair and aortic arch repair surgeries, it has a significant impact on the post operative course and post operative morbidity

    The actor model eases the definition of concurrent programs with non uniform behaviors. Static analysis of such a model was previously done in a data-flow oriented way, with type systems. This approach was based on constraint set resolution and was not able to deal with precise properties for communications of behaviors. We present here a new approach, control-flow oriented, based on the abstract interpretation framework, able to deal with communication of behaviors. Within our new analyses, we are able to verify most of the previous properties we observed as well as new ones, principally based on occurrence counting

    Correlation Between Plasma Endothelin-1, Blood Glucose and Serum Calcium in Male Rats Instillated with Bleomycin

    Endothelin-1 is profibrotic by stimulating fibroblast replication, migration, contraction, collagen synthesis and secretion while decreasing collagen degradation.In the present study,20male rats weighing 240 grams and 10-12 weeks old were utilized.Pulmonary fibrosis was induced by a single instillation of bleomycin (90 mg/kg).After 7 days of instillation, elevated plasma ET-1 was considered as bleomycin induced pulmonary fibrosis. High dose of bleomycin instillation caused significant increase in plasma ET-1as compared with normal saline instillation. Blood glucose and serum calcium were also markedly elevated. Statistical analysis using Pearson coefficient of correlation revealed that there was a positive relationship between plasma ET-1 and blood sugar (r=0.7). The same correlation was detected between plasma ET-1 and serum calcium (r=0.7).The present study concluded thatthere is a strong correlation between plasma ET-1 and blood glucose and it may be due to decreaseof insulinsensitivity, and inhibits β-cells to release insulin .In addition,the role of calcium ions cannot be excluded in raising blood glucose by ET-1
