Artificial light and nocturnal activity in gammarids
Artificial light is gaining attention as a potential stressor to aquatic
ecosystems. Artificial lights located near streams increase light levels
experienced by stream invertebrates and we hypothesized light would depress
night drift rates. We also hypothesized that the effect of light on drift
rates would decrease over time as the invertebrates acclimated to the new
light level over the course of one monthâs exposure. These hypotheses were
tested by placing Gammarus spp. in eight, 75 m Ă 1 m artificial flumes. One
flume was exposed to strong (416 lx) artificial light at night. This strong
light created a gradient between 4.19 and 0.04 lx over the neighboring six
artificial flumes, while a control flume was completely covered with black
plastic at night. Night-time light measurements taken in the Berlin area
confirm that half the flumes were at light levels experienced by urban aquatic
invertebrates. Surprisingly, no light treatment affected gammarid drift rates.
In contrast, physical activity measurements of in situ individually caged G.
roeseli showed they increased short-term activity levels in nights of complete
darkness and decreased activity levels in brightly lit flumes. Both nocturnal
and diurnal drift increased, and day drift rates were unexpectadly higher than
nocturnal drift
Prozessoptimierung in der Krankenhaussprechstunde: Erfahrungen und Ergebnisse unter BerĂŒcksichtigung der Möglichkeiten von Informationstechnologie
Einleitung: Prozessorientiertes Denken nimmt in der Medizin bei knapper werdenden Ressourcen einen immer gröĂeren Stellenwert ein. Diese Arbeit untersucht Möglichkeiten der Prozessoptimierung im Bereich der ambulanten Versorgung am Krankenhaus.
Fragestellung: Mit einer Interventionsstudie wird untersucht, ob ein aus der Softewareentwicklung abgeleitetes Vorgehensmodell erfolgreich fĂŒr ein Prozessoptimierungsprojekt in der ambulanten Sprechstunde eines Krankenhauses angewandt werden kann. Es wird die EDVUnterstĂŒtzung klinischer AblĂ€ufe der Sprechstunde eingefĂŒhrt. Das Gesamtzielkriterium ist die Steigerung der Patientenzufriedenheit. Als Teilaspekt werden die Erstellung und die
Anpassung eines in einem KIS abgebildeten Terminkalenders mittels Generatortools
untersucht. Können durch eine erfolgreiche Umsetzung verkĂŒrzte Wartezeiten der Patienten sowie eine verbesserte Dokumentenbereitstellung erreicht werden? Ein weiterer Teilaspekt ist die EinfĂŒhrung von âComputerized Physician Order Entryâ fĂŒr radiologische Untersuchungen in der ambulanten Sprechstunde. Ist dies technisch und organisatorisch möglich und können dadurch eine höhere ProzessqualitĂ€t und kĂŒrzere Prozesszeiten erreicht werden?
Material und Methoden: Untersucht wird die unfallchirurgische Sprechstunde fĂŒr ambulante und ehemals stationĂ€re Patienten an einem Krankenhaus der Maximalversorgung. Die Datenerhebungen fanden im Herbst 2002 und FrĂŒhjahr 2004 statt. Sie bestehen aus einer Patientenbefragung inklusive eines Zufriedenheitsfragebogens, Messungen der Warte- und Prozesszeiten
sowie Mitarbeiterbefragungen. Die zwischenzeitliche Prozessoptimierung erfolgt nach einem Vorgehensmodell, welches sich von dem Modell zur Softwareentwicklung im Krankenhaus nach (Kuhn KA, Lenz R et al. 2003) ableitet. Es besteht aus einer Erkundungs- und einer Analysephase zur Erfassung von Problembereichen und zur Datenerhebung. Es folgt eine Phase des Redesigns, in der ProzessÀnderungen und IT-Anwendungen entworfen werden. Diese werden in der Implementierungsphase organisatorisch und technisch umgesetzt.
Nach einer Phase der Schulung fĂŒr die Mitarbeiter folgt der Routinebetrieb. In der zweiten Analysephase wird abschlieĂend der der Erfolg des Projektes gemessen.
Ergebnisse: Die Prozessoptimierung nach dem genannten Vorgehensmodell konnte erfolgreich durchgefĂŒhrt werden. Ein Terminkalender fĂŒr die Sprechstunden und ambulante Operationen wurde erfolgreich im KIS eingefĂŒhrt, dieser beinhaltet die Terminvergabe bei der Arztbriefschreibung. Die Anmeldung von radiologischen Untersuchungen wurde auf ein CPOE-Verfahren umgestellt und ein Vorgehen zur direkten Einbestellung von Patienten zur Röntgenuntersuchung eingerichtet. Die Mitarbeiter der Sprechstunde akzeptieren diese ProzessĂ€nderungen, die eingefĂŒhrten IT-Anwendungen werden ĂŒberwiegend als positiv betrachtet.
Die Anzahl der vorhandenen TermineintrÀge der Patienten steigt signifikant auf
67,5% der FĂ€lle (p=0,000), die VerfĂŒgbarkeit von Akten bei der Anmeldung des Patienten steigt auf 47,8% (p=0,000). Die Ankunft der Patienten in der Sprechstunde erfolgt durchschnittlich zu einem spĂ€teren Zeitpunkt (p=0,000) und verteilt sich gleichmĂ€Ăiger ĂŒber die Sprechstundenzeit. Die Wartezeit der Patienten auf den ersten Aufruf in die Sprechstunde sinkt signifikant von 69min. (n=344) auf 43min. (n=317) (p=0,000). Ebenso sinkt die Gesamtdauer des Sprechstundenbesuches signifikant von 128min. auf 100min (p=0,000). Die Wartezeit auf die Röntgenuntersuchung verlĂ€ngert sich jedoch signifikant um 5min (p=0,045). Die Dauer der Röntgenuntersuchung verlĂ€ngert sich signifikant um durchschnittlich eine Minute (p=0,039). Weder steigt der Patientenanteil, der direkt zu einer Röntgenuntersuchung einbestellt wird, noch verringert sich die Wartezeit hierfĂŒr signifikant. Die Patientenzufriedenheit steigt zwischen den Messzeitpunkten signifikant an (p=0,001).
Diskussion: Mit dem Vorgehensmodell kann mit vertretbarem Aufwand erfolgreich eine Prozessoptimierung in einer ambulanten Sprechstunde am Krankenhaus unter EinfĂŒhrung von IT-Anwendungen durchgefĂŒhrt werden. Kurze Iterationen, ein offener Kommunikationsstil, die frĂŒhe Einbindung der Endnutzer und ein abteilungsĂŒbergreifendes Vorgehen sind wichtige Erfolgsfaktoren. Die Aufteilung des Gesamtprojektes in Unterschritte ist vorteilhaft. Bei ITgestĂŒtzten
Interventionen ist ein hochpartizipatorischer, iterativer Softewareentwicklungsprozess in Verbindung mit einem holistischen KIS der SchlĂŒssel zum Erfolg. Ein âGenerator Toolâ des KIS ermöglicht die schnelle Entwicklung klinischer Anwendungen. Durch die gewĂ€hlte Form der Prozessoptimierung kann die Patientenzufriedenheit signifikant steigen. Es
gibt starke Hinweise, dass Terminkalender im KIS die Wartezeit auf den ersten Aufruf in das Behandlungszimmer und die Gesamtaufenthaltsdauer der Patienten reduzieren sowie die AktenverfĂŒgbarkeit bei der Anmeldung erhöhen. Die Ăbernahme eines bestehenden CPOE fĂŒr radiologische Untersuchungen in eine ambulante Sprechstunde am Krankenhaus ist technisch und organisatorisch möglich. Sie fĂŒhrt zu einer höheren ProzessqualitĂ€t, jedoch entstehen dadurch eine zeitliche Mehrbelastung der anwendenden Ărzte und lĂ€ngere Wartezeiten der Patienten auf die Untersuchung. Die Einbestellung von Patienten direkt zu einer Röntgenuntersuchung unter Verwendung von CPOE ist umsetzbar, fĂŒhrt aber zu keiner Einsparung von Wartezeit fĂŒr den Patienten.

Deep brain stimulation for movement disorder treatment: Exploring frequency-dependent efficacy in a computational network model
A large scale computational model of the basal ganglia (BG) network is
proposed to describes movement disorder including deep brain stimulation (DBS).
The model of this complex network considers four areas of the basal ganglia
network: the subthalamic nucleus (STN) as target area of DBS, globus pallidus,
both pars externa and pars interna (GPe-GPi), and the thalamus (THA).
Parkinsonian conditions are simulated by assuming reduced dopaminergic input
and corresponding pronounced inhibitory or disinhibited projections to GPe and
GPi. Macroscopic quantities can be derived which correlate closely to thalamic
responses and hence motor programme fidelity. It can be demonstrated that
depending on different levels of striatal projections to the GPe and GPi, the
dynamics of these macroscopic quantities switch from normal conditions to
parkinsonian. Simulating DBS on the STN affects the dynamics of the entire
network, increasing the thalamic activity to levels close to normal, while
differing from both normal and parkinsonian dynamics. Using the mentioned
macroscopic quantities, the model proposes optimal DBS frequency ranges above

Referenceable mobile crowdsensing architecture: A healthcare use case
Smartphones have become an integral part in life of users, mainly because over the course of recent years, they have become extremely mainstream, cheap, flexible, and they pack high-end hardware that offers high computational capabilities. Many, if not all of todayâs smartphones are equipped with sophisticated sensors which enable smart mobile sensing. The programmable nature of these sensors in the smartphones enable a wide array of possibilities to achieve user-centric or environmental sensing. Even though there have been different approaches proposed to develop a smartphone app, platform, design frameworks, APIs, and even application-specific architectures, there is a lack of generalised referenceable architecture in the literature. In this paper, we propose a generic reference architecture, which can be derived to create more concrete mobile sensing or mobile app architectures.
Furthermore, we realise the proposed reference architecture in a healthcare use case, specifically in the context of applying smart mobile sensing to support tinnitus research
Towards Automated Smart Mobile Crowdsensing for Tinnitus Research
Tinnitus is a disorder that is not entirely understood, and many of its correlations are still unknown. On the other hand, smartphones became ubiquitous. Their modern versions provide high computational capabilities, reasonable battery size, and a bunch of embedded high-quality sensors, combined with an accepted user interface and an application ecosystem. For tinnitus, as for many other health problems, there are a number of apps trying to help patients, therapists, and researchers to get insights into personal characteristics but also into scientific correlations as such. In this paper, we present the first approach to an app in this context, called TinnituSense that does automatic sensing of related characteristics and enables correlations to the current condition of the patient by a combined participatory sensing, e.g., a questionnaire. For tinnitus, there is a strong hypothesis that weather conditions have some influence. Our proof-of-concept implementation records weather-related sensor data and correlates them to the standard Tinnitus Handicap Inventory (THI) questionnaire. Thus, TinnituSense enables therapists and researchers to collect evidence for unknown facts, as this is the first opportunity to correlate weather to patient conditions on a larger scale. Our concept as such is limited neither to tinnitus nor to built-in sensors, e.g., in the tinnitus domain, we are experimenting with mobile EEG sensors. Using Synthetic Aperture Radar to Define Spring Breakup on the Kuparuk River, Northern Alaska
Spring runoff measurements of Arctic watersheds are challenging given the remote location and the often dangerous field conditions. This study combines remote sensing techniques and field measurements to evaluate the applicability of synthetic aperture radar (SAR) to defining spring breakup of the braided lower Kuparuk River, North Slope, Alaska. A statistical analysis was carried out on a time series (2001â10) of SAR images acquired from the European Remote-Sensing Satellite (ERS-2) and the Canadian RADARSAT satellite, as well as on measured runoff. On the basis of field information, the SAR images were separated into pre-breakup, breakup, and post-breakup periods. Three variables were analyzed for their suitability to bracket the river breakup period: image brightness, variance in brightness over the river length, and a sum of rank order change analysis. Variance in brightness was found to be the most reliable indicator. A combined use of that variance and sum of rank order change appeared promising when enough images were available. The temporal resolution of imagery served as the major limitation in constraining the timing of the hydrologic event. Challenges associated with spring runoff monitoring and the sensitive nature of SAR likely resulted in an earlier detection of surficial changes by the remote sensing technique compared to the field runoff observations. Given a sufficient temporal resolution, SAR imagery has the potential to improve the spatiotemporal monitoring of Arctic watersheds for river breakup investigations.La mesure de lâĂ©coulement printanier des bassins hydrographiques de lâArctique nâest pas facile Ă rĂ©aliser en raison de lâĂ©loignement ainsi quâen raison des conditions souvent dangereuses qui ont cours sur le terrain. La variance de la luminance sâest avĂ©rĂ©e lâindicateur le plus fiable. Lâutilisation conjointe de cette variance et de la somme des changements de classement suivant le rang sâavĂ©raient prometteuse lorsque le nombre dâimages Ă©tait suffisant. La rĂ©solution temporelle de lâimagerie a constituĂ© la plus grande limitation pour contraindre la temporisation de lâĂ©vĂ©nement hydrologique. Les dĂ©fis liĂ©s Ă la surveillance de lâĂ©coulement printanier et la nature sensible du SAR ont vraisemblablement donnĂ© lieu Ă la dĂ©tection prĂ©coce des changements superficiels au moyen de la technique de tĂ©lĂ©dĂ©tection comparativement aux observations mĂȘmes de lâĂ©coulement printanier. Towards Incentive Management Mechanisms in the Context of Crowdsensing Technologies based on TrackYourTinnitus Insights
The increased use of mobile devices has led to an improvement in the public health care through participatory interventions. For example, patients were empowered to contribute in treatment processes with the help of mobile crowdsourcing and crowdsensing technologies. However, when using the latter technologies, one prominent challenge constitutes a continuous user engagement. Incentive management techniques can help to tackle this challenge by motivating users through rewards and recognition in exchange of task completion. For this purpose, we aim at developing a conceptual framework that can be integrated with existing mHealth mobile crowdsourcing and crowdsensing platforms. The development of this framework is based on insights we obtained from the TrackYourTinnitus (TYT) mobile crowdsensing platform. TYT, in turn, pursues the goal to reveal insights to the moment-to-moment variability of patients suffering from tinnitus. The work at hands presents evaluated data of TYT and illustrates how the results drive the idea of a conceptual framework for an incentive management in this context. Our results indicate that a proper incentive management should play an important role in the context of any mHealth platform that incorporates the idea of the crowd
Second-order polarizability of donor-acceptor substituted oligothiophenes : substituent variation and conjugation length dependence
Second-order polarizabilities and excited state dipole moments of a number of donor-acceptor substituted α-oligothiophenes have been investigated by use of electric field induced second harmonic generation (EFISH), solvatochromism and electrooptical absorption measurements (EOAM). The experimental results reveal that oligothiophenes are very effective conjugation units offering substantial enhancement of the second-order polarizability with increasing chain length
Functional Metaplasticity of Hippocampal Schaffer Collateral-CA1 Synapses Is Reversed in Chronically Epileptic Rats
Spatial learning and associating spatial information with individual experience are crucial for rodents and higher mammals. Hence, studying the cellular and molecular cascades involved in the key mechanism of information storage in the brain, synaptic plasticity, has led to enormous knowledge in this field. A major open question applies to the interdependence between synaptic plasticity and its behavioral correlates. In this context, it has become clear that behavioral aspects may impact subsequent synaptic plasticity, a phenomenon termed behavioral metaplasticity. Here, we trained control and pilocarpine-treated chronically epileptic rats of two different age groups (adolescent and adult) in a spatial memory task and subsequently tested long-term potentiation (LTP) in vitro at Schaffer collateralâCA1 synapses. As expected, memory acquisition in the behavioral task was significantly impaired both in pilocarpine-treated animals and in adult controls. Accordingly, these groups, without being tested in the behavioral training task, showed reduced CA1-LTP levels compared to untrained young controls. Spatial memory training significantly reduced subsequent CA1-LTP in vitro in the adolescent control group yet enhanced CA1-LTP in the adult pilocarpine-treated group. Such
training in the adolescent pilocarpine-treated and adult control groups resulted in intermediate changes. Our study demonstrates age-dependent functional metaplasticity following a spatial memory training task and its reversal under pathological conditions
