    Integrity of the arcuate fasciculus in patients with schizophrenia with auditory verbal hallucinations: A DTI-tractography study

    AbstractAuditory verbal hallucinations (AVH) of schizophrenia are associated with a disrupted connectivity between frontal and temporoparietal language areas. We hypothesized that this dysconnectivity is underpinned by white matter abnormalities in the left arcuate fasciculus, the main fiber bundle connecting speech production and perception areas. We therefore investigated the relationship between AVH severity and the integrity of the arcuate fasciculus measured by diffusion tensor imaging (DTI) tractography in patients with schizophrenia.Thirty-eight patients with treatment-resistant schizophrenia were included: 26 presented with daily severe treatment-resistant AVH, 12 reported prominent negative symptoms and no AVH. Fractional anisotropy (FA) was measured along the length of the left and right anterior arcuate fasciculi and severity of AVH was assessed using P3 PANSS item.FA values were significantly higher in the left arcuate fasciculus in patients with AVH than in no AVH patients (F(1,35)=3.86; p=0.05). No difference was observed in the right arcuate fasciculus. There was a significant positive correlation between FA value in the left arcuate fasciculus and the severity of AVH (r=0.36; p=0.02). No correlation was observed between FA values and PANSS total score suggesting a specific relationship between AVH severity and the left arcuate fasciculus integrity.These results support the hypothesis of a relationship between left frontotemporal connectivity and AVH in patients with schizophrenia and suggest that whilst a disruption of frontotemporal connectivity might be present to ensure the emergence of AVH, more severe anatomical alterations may prevent the occurrence of AVH in patients with schizophrenia

    ‘A Girl's Love’: Lord Alfred Douglas as Homoerotic Muse in the Poetry of Olive Custance

    This is an Accepted Manuscript of an article published by Taylor & Francis in Women: a Cultural Review on 15/09/2011, available online: http://dx.doi.org/10.1080/09574042.2011.585045.This article explores the relationship between the poet Olive Custance and her husband Lord Alfred Douglas, arguing that Custance constructed Douglas as a male muse figure in her poetry, particularly the sequence ‘Songs of a Fairy Princess’ (Rainbows 1902). The introduction sets out Custance's problematic historical positioning as a ‘decadent’ poet who published nothing following the Great War, but whose work came too late to fit into strictly ‘fin de siècle’ categories. I suggest, however, that Custance's oscillating constructions of gender and sexuality make her more relevant to the concerns of modernity than has previously been acknowledged and her work anticipates what is now termed ‘queer’. The first main section of the article traces the cultural background of the fin de siècle male muse, arguing that Custance's key influences—male homoerotic writers such as Wilde and Pater—meant it was logical that she should imagine the muse as male, despite the problems associated with gender-reversals of the muse-poet relationship which have been identified by several feminist critics. I then move on to focus specifically on how Shakespearean discourses of gender performance and cross-dressing played a key role in Custance and Douglas's courtship, as they exchanged the fluid roles of ‘Prince’, ‘Princess’ and ‘Page’. The penultimate section of the article focuses on discourses of fairy tale and fantasia in Custance's ‘Songs of a Fairy Princess’ sequence, in which these fantasy roles contribute to a construction of Douglas as a feminised object, and the relationship between the ‘Prince’ and ‘Princess’ is described in terms of narcissistic sameness. My paper concludes by tracing the demise of Custance and Douglas's relationship; as Douglas attempted to be more ‘manly’, he sought to escape the role of object, resulting in Custance losing her male muse. But her sexually-dissident constructions of the male muse remain important experiments worthy of critical attention

    Specific structural and functional cerebral alterations of resistant auditory hallucinations in patients with schizophrenia

    Les hallucinations auditives (HA) sont présentes chez 70% à 80% des patients atteints de schizophrénie et sont résistantes aux traitements pharmacologiques dans environs 25% cas. Ces symptômes induisent une détresse importante chez les sujets et grèvent le pronostic. Dans ce travail, nous avons mis en avant des altérations cérébrales spécifiques aux HA chez des patients atteints de schizophrénie. Dans une première étude, en utilisant la méthode de DTI, nous avons montré que les patients atteints de schizophrénie avec HA ont une intégrité du faisceau arqué, supérieure à celle mesurée chez des patients atteints de schizophrénie sans HA. Dans une seconde étude, en utilisant la méthode de SRM, nous avons montré une augmentation de NAA, reflet du métabolisme neuronal, dans le cortex préfrontal dorsolatéral de l'hémisphère droit chez les patients atteints de schizophrénie avec HA comparé au CPFDL gauche et à des patients atteints de schizophrénie sans HA. Dans ces deux études, le taux de NAA du CPFDL droit et l'intégrité du faisceau arqué ont été associés à la sévérité des HA présentes chez les patients. Dans notre dernière étude, nous avons montré une association entre le taux de BDNF périphérique, marqueur de la plasticité neuronale, et le taux de NAA mesuré dans le CPFDL droit, chez les patients atteints de schizophrénie résistante. Ces résultats montrent qu'il existe une pathophysiologie spécifique des HA chez les patients atteints de schizophrénie et mettent en avant l'importance de stratifier les patients sur la base de leurs symptômes prédominants dans les futures études pathophysiologiques de la schizophrénieAuditory hallucinations (AH) are present in 70% to 80% of patients with schizophrenia and are resistant to pharmacological treatments in 25% of cases. These symptoms induce significant distress in patients and predict a bad prognosis. In this work we have highlighted cerebral alterations specific to AH in patients suffer from schizophrenia. In a first study, using DTI method, we showed that patients with schizophrenia and AH have an arcuate fasciculus integrity, reflected by the measurement of fractional anisotropy (FA) greater than the one measured in patients with schizophrenia without AH. In a second study, using MRS method, we showed an increase of NAA level measured in the dorsolateral prefrontal cortex (DLPFC) in the right hemisphere in patients with schizophrenia and AH compared to the DLPFC in the left hemisphere and compared to patients with schizophrenia and without AH. Moreover, in these two studies we showed an association between AH severity and the arcuate fasciculus integrity in the left hemisphere and an association between AH severity and NAA levels in the right DLPFC. Finally, in our last study, we quantified BDNF levels using ELISA method and showed an association between peripheral BDNF level, a marker of neuronal plasticity, and NAA levels in the right DLPFC, marker of neuronal metabolism, in patients with treatment-resistant schizophrenia. These results show that there is a specific pathophysiology of AH in patients with schizophrenia and highlight the importance of stratifying patients on the basis of their predominant symptoms in future pathophysiological studies of schizophreni
