Embolization in post-traumatic epistaxis
Wst臋p. Krwawienie z nosa (epistaxis) cz臋sto wyst臋puje po urazach twarzoczaszki, kt贸re prowadz膮 do
uszkodzenia ga艂臋zi t臋tnicy szyjnej zewn臋trznej. W zale偶no艣ci od nasilenia krwawienia, jego miejsca oraz
do艣wiadczenia o艣rodka stosuje si臋 leczenie zachowawcze, chirurgiczne lub wewn膮trznaczyniowe. Celem
pracy jest ocena mo偶liwo艣ci i skuteczno艣ci embolizacji wewn膮trznaczyniowej u chorych z krwotokiem z nosa
w nast臋pstwie urazu twarzoczaszki.
Materia艂 i metody. Badaniem obj臋to 22 chorych (15 m臋偶czyzn i 7 kobiet) w wieku 21–64 lat (艣rednia
41 lat) z pourazowym krwotokiem z nosa. U 13 os贸b stwierdzono angiograficznie w obr臋bie ga艂臋zi t臋tnicy
szyjnej zewn臋trznej obecno艣膰 t臋tniaka rzekomego, u 6 wynaczynienie si臋 艣rodka cieniuj膮cego, a u 3 chorych
— przetok臋 szyjno-jamist膮. U 3 pacjent贸w po angiografii diagnostycznej odst膮piono od embolizacji
ze wzgl臋du na zbyt du偶e ryzyko zabiegu. U 19 pozosta艂ych zabieg embolizacji przeprowadzono przy
u偶yciu koreczk贸w z g膮bki 偶elatynowej, cz膮stek alkoholu poliwinylowego, cz膮stek polimeru akrylowego oraz
mikrospiral platynowych.
Wyniki. U wszystkich embolizowanych chorych natychmiastowo opanowano krwawienie. W 4 przypadkach
(20%) dosz艂o do nawrotu krwawienia, kt贸re w 2 przypadkach opanowano poprzez ponown膮 embolizacj臋.
U 9 chorych (42%) wyst膮pi艂y niekorzystne objawy w postaci b贸lu i obrz臋ku twarzy, parestezji, szcz臋ko艣cisku,
gor膮czki. Objawy te ust膮pi艂y w ci膮gu kilku do kilkunastu dni od zabiegu. U 3 chorych (15%) wyst膮pi艂y trwa艂e
zaburzenia czucia w obr臋bie policzka. U 1 pacjenta pojawi艂o si臋 niewielkie owrzodzenie podniebienia, kt贸re
skutecznie leczono zachowawczo. Nie zaobserwowano powa偶nych powik艂a艅 neurologicznych.
Wnioski. Embolizacja jest skuteczn膮 metod膮 leczenia krwotok贸w pourazowych z nosa i powinno si臋 j膮
uwzgl臋dnia膰 przy planowaniu post臋powania leczniczego.
Acta Angiol 2011; 17, 1: 89–97Background. Epistaxis is a common consequence of craniofacial injury involving damage to the external
carotid artery branches. Conservative treatment or surgical or endovascular procedures can be applied,
depending on the intensity and the site of bleeding as well as on the experience of the medical team. The
applicability of endovascular embolization and its efficacy in the treatment of epistaxis following craniofacial
injury are discussed in this paper.
Material and methods. The study group included 22 patients (15 men, 7 women) with mean age of 41 years
(range of 21–64 years), who suffered from post-traumatic epistaxis. Angiography disclosed pseudoaneurysms
of the external carotid branches in 13 patients, extravasation of contrast media in 6 persons, and cavernous
carotid fistulae in three patients. Based on diagnostic angiography findings, the risk related to endovascular
embolization procedure was considered too high in three patients. In the remaining 19 patients, embolization
was performed with the use of gelatine foam, polyvinyl alcohol, acrylic polymer, and platinum microcoils.
Results. Immediate arrest of epistaxis was achieved in all embolised patients. In four cases (20%) the bleeding
recurred, of which two cases were controlled with repeated embolization. Nine patients (42%) experienced
adverse effects in the form of transient facial pain and swelling, paresthesiae, trismus, and fever, which regressed
within several days. Three patients (15%) had permanent sensory disturbances in the cheek area.
One patient developed a small ulceration of the palate, treated conservatively with success. There were no
major neurological complications.
Conclusions. Endovascular embolization is an effective method for managing post-traumatic epistaxis and
should be considered when planning therapy.

Endovascular treatment of visceral artery aneurysms — a single-centre experience
Background. Visceral artery aneurysms (VAAs) are intra-abdominal aneurysms that occur in the coeliac trunk
and the superior and inferior mesenteric arteries or their branches. They are uncommon vascular anomalies but
can be life-threatening, with mortality ranging from 10 to 25% and up to 75% in pregnant women. Currently
available treatment options include transcatheter embolisation, percutaneous implantation of covered stent,
or surgical resection. The aim of our study was to demonstrate methods of minimally invasive endovascular
treatment of visceral artery aneurysms and to assess their applicability and efficacy.
Material and methods. Between January 2000 and September 2009, 34 patients with VAAs (aged 23–79
years) underwent endovascular treatment. Different techniques were used: 14 aneurysms were embolised
with coils, covered stents were implanted in 12 patients, and in 8 cases transcatheter direct thrombin injection
into the sack of the aneurysm was implemented.
Results. Almost all aneurysms (33/34) were successfully excluded from the circulation. Follow-up examinations
with Doppler USG or angio-CT were performed in 31 patients between 3 and 18 months after treatment.
No reperfusion of aneurysmal sac was observed in any of the followed-up patients. Satisfactory results were
observed in all 31 examined patients.
Conclusions. Our experience shows that percutaneous treatment of visceral artery aneurysms is both safe
and effective. Endovascular treatment of these lesions should be considered as the primary treatment option.
Good treatment results depend on proper assessment of the aneurysm’s morphology by means of angio-CT
or angiography as well as on selection of the appropriate vascular approach and endovascular technique.

tych naczy艅. Mimo 偶e s膮 rzadk膮 patologi膮 naczyniow膮, stanowi膮 powa偶ne zagro偶enie dla 偶ycia (wska藕nik
艣miertelno艣ci waha si臋 od 10% do 25%, nawet do 75% u kobiet ci臋偶arnych). W艣r贸d dost臋pnych metod
leczenia tych wad naczyniowych nale偶y wymieni膰: przezcewnikow膮 embolizacj臋, przezsk贸rn膮 implantacj臋 stent贸w pokrywanych (stentgraft贸w) lub chirurgiczne wy艂膮czenie t臋tniaka. Celem niniejszego badania by艂o
przedstawienie mo偶liwo艣ci leczenia wewn膮trznaczyniowego oraz ocena skuteczno艣ci tej metody leczenia.
Materia艂 i metody. W okresie od stycznia 2000 r. do wrze艣nia 2009 r. 34 chorych (w wieku 23–79 lat)
z t臋tniakami t臋tnic trzewnych poddano leczeniu wewn膮trznaczyniowemu. Zastosowano r贸偶ne metody:
14 t臋tniak贸w embolizowano za pomoc膮 spiral, u 12 chorych implantowano stenty pokrywane, a u pozosta艂ych
8 podano trombin臋 do worka t臋tniaka.
Wyniki. Prawie wszystkie t臋tniaki (33/34) skutecznie wy艂膮czono z kr膮偶enia. W okresie 3–18 miesi臋cy po
zabiegu u 31 chorych przeprowadzono badanie kontrolne: USG metod膮 doplerowsk膮 lub angio-CT. U 偶adnego
chorego nie stwierdzono nap艂ywu krwi do worka t臋tniaka.
Wnioski. Do艣wiadczenie autor贸w w leczeniu t臋tniak贸w t臋tnic trzewnych pokazuje, 偶e zabiegi przezsk贸rne
s膮 metod膮 zar贸wno skuteczn膮, jak i bezpieczn膮. Leczenie wewn膮trznaczyniowe t臋tniak贸w powinno si臋
rozwa偶a膰 jako pierwsz膮 z mo偶liwo艣ci terapeutycznych. Dobre wyniki leczenia zale偶膮 od poprawnie ocenionej
morfologii t臋tniaka na podstawie angio-CT lub angiografii oraz w艂a艣ciwie wybranej metody leczenia
wewn膮trznaczyniowego.

Angioplasty and stenting of stenosed carotid arteries in patients after radiotherapy for laryngeal cancer — a report of three cases
Pomimo rozwoju technik radioterapeutycznych w onkologii urazy popromienne naczy艅 krwiono艣nych stanowi膮
wa偶ne zagadnienie kliniczne. W przypadku stosowania takiego leczenia nowotwor贸w z艂o艣liwych szyi cz臋sto
dochodzi do wt贸rnych zw臋偶e艅 t臋tnic dom贸zgowych, co powoduje objawy niedokrwienia m贸zgu. Popromienne
ograniczenia dro偶no艣ci t臋tnic dom贸zgowych maj膮 inny charakter ni偶 typowe zmiany mia偶d偶ycowe, dotycz膮
d艂ugich odcink贸w t臋tnic i s膮 zlokalizowane w r贸偶nych (wielu) t臋tnicach, cz臋sto wielopoziomowo. W leczeniu tych
zw臋偶e艅 metod膮 z wyboru s膮 obecnie wewn膮trznaczyniowe zabiegi angioplastyki i stentowania ze wzgl臋du na du偶膮
skuteczno艣膰 i ma艂膮 inwazyjno艣膰, przy oczywistych ograniczeniach klasycznej chirurgii. W pracy zaprezentowano
trzy r贸偶ne (wybrane) przypadki stentowania zw臋偶e艅 t臋tnic dom贸zgowych u chorych po radioterapii raka krtani.
Acta Angiol 2010; 16, 4: 172–180Despite constant improving radiotherapy technique In oncology radiation-induced vascular injuries are still of
an important clinical issue. When radiotherapy is concerned with neck malignancies it is relatively frequent
that patient may present obstructions in scope of antebrain arteries and finally, brain ischemia. Post-radiation
arterial stenoses are different when compared to ordinary atherosclerotic changes — they may affect long
arterial segments and they are localized in many (different) arteries, commonly multilevel. Endovascular
angioplasty and stenting is nowadays a treatment method of choice in these pathologies, because of high
efficacy, non-invasiveness — at obvious limitations of classic surgery. The paper presents three different
(selected) cases of endovascular stenting in patients who underwent radiotherapy due to laryngeal cancer.
