    Ytterbium-doped tantalum pentoxide waveguide lasers

    We have demonstrated a Yb:Ta2O5 waveguide laser fabricated by RF magnetron sputtering on oxidised silicon. The waveguide laser was end-pumped with a laser diode at 977 nm and lasing was observed between 1015 and 1020 nm. The launched pump power threshold and slope efficiency were measured to be ~25 mW and 1.78 %, respectively

    Competence of Biopesticide and Neem in Agriculture

    Neem plant is considered as the most useful traditional plant in India. The various properties of different parts of neem tree are used as insecticide, antifeedant, hormonal, antifungal, antiallergic, antidermatic, anti-inflammatory, antiscabic larvicidal and spermaticidal activity etc. In recent era the major challenge is to increase the food production without harming the environment and can control the pest. Since, last decades pesticides have become an integral component in sustainable agriculture and the modern day cultivation practices uses of chemical pesticides and fertilizers are in eliminable. Enormous use of different chemical pesticides in agricultural fields is resistant to most of the pests. So that the natural pesticides from A. indica are considered to be less harmful, biodegradability, least persistence, lest toxic to non-target organism, economic and used to combat insects, pests are not ruled out. Neem derived products play an essential role in the pest management, in agricultural fields such as crop and stored grains. Fruitful results of application of formulated neem based products in agriculture field will provide a cost effective technology to the farmers

    Clinicoetiological profile of neonatal seizures in a tertiary care hospital

    Background: Seizure is one of the most frequently observed clinical sign in neonatal period and may be the only sign of a central nervous system disorder. Its recognition is of paramount importance and a newborn with seizures is in a true medical emergency requiring prompt intervention. Objective: To determine the clinical and etiological profile of seizures among neonates admitted to neonatal unit in a tertiary care hospital. Materials and Methods: It was a prospective, observational, hospital-based study conducted over a period of 1 year (December 2014-November 2015). Nearly, 63 babies either admitted with neonatal seizures or who developed seizures after admission were studied to find the clinical and etiological profile. Detailed history including gestational age, mode of delivery, birth events, Apgar scores and antenatal problems, age of onset, and frequency of seizures was also noted. Seizures were classified on the basis of detailed description by pediatric resident on duty in a predesigned pro forma. Informed written consent of parents was taken before enrolling a baby in study. Results: Sepsis was the most common cause of neonatal seizures (64.9%), followed by birth asphyxia, i.e., hypoxic ischemic encephalopathy (HIE) (48.3%). Hypoglycemia was seen in 21.6% and hypocalcemia in 18.3% cases. Subtle seizures were the most common type of seizures seen in 65% babies, followed by tonic (31.6%), myoclonic (16.6%%), and clonic (6.67%) type. Subtle seizures were commonly seen in cases of HIE. Conclusion: The recognition of clinicoetiology of neonatal seizures is often helpful with respect to prognosis and treatment. The most common etiology for neonatal seizure is sepsis. Onset of seizures during first 3 days of life has significant correlation with HIE as etiology. Subtle seizures are the most common type of clinical seizures, which is difficult to identify; therefore, careful observation of at risk newborns is necessary

    Role of ultrasound guided fine needle aspiration cytology of right hypochondrial masses

    Background: Patients having right hypochondrial intra-abdominal masses are commonly encountered in clinical practice. The study was done to know the role of ultrasound guided fine needle aspiration in diagnosing right hypochondrial masses and its most common cause.Methods: 112 cases were collected from department of surgery, SVBP hospital meerut. FNAC was done using 22-23 G disposable lumbar puncture needle with trochar fitted with 20 ml syringe, introduced under radiological guidance and aspiration is done under negative pressure. Smears were stained with Leishman’s stain, May Grunwald Geimsa (MGG) and Papnicolou stain.Results: Out of total 112 cases, 12 cases excluded from study as only blood was aspirated. Therefore, out of 100 cases, 83% (83/100) cases were malignant, 7% (7/100) benign and 10% (10/100) inconclusive/ due to low cellularity. Among the malignant masses, majority 52 (52.0%) cases were of liver secondaries followed by 24 (24.0%) cases of adenocarcinoma gall bladder, 5 (5.0%) cases of primary hepatocellular carcinoma (HCC) and single case (1%), each of cholangiocarcinoma GB and squamous cell carcinoma GB. Among the benign lesions, 3 (3.0%) cases of liver abscess, 2 (2.0%) cases of hydatid disease followed by single case (1.0%) of hepatic adenoma and cysticercosis liver. In this study, overall accuracy of USG guided FNAC was 96.66%. Sensitivity, specificity, positive predictive value, negative predictive value and efficacy of USG guided FNAC in right hypochondrial masses were 96.66%, 100%, 100%, 66.67% and 96.87% respectively.Conclusions: USG guided FNAC is simple, quick, safe, reliable and economical tool without any significant complication in diagnosing right hypochondrial masses

    Role of apoptotic index, mitotic index and MIB-1 antibody expression as biomarkers in preneoplastic and neoplastic lesions of uterine cervix

    Background: In India, cervical cancer ranks as the 2nd most frequent cancer among women. This study was carried out to study the role of apoptotic index (AI), mitotic index (MI) and MIB-1 labelling index (LI) as biomarkers in preneoplastic and neoplastic lesions of uterine cervix.Methods: Total of 90 cases was studied, over a period of two years, in the Department of Pathology as well as Obstetrics and Gynaecology, LLRM Medical College and associated SVBP Hospital, Meerut, India. Apoptotic Index and Mitotic Index were calculated on H & E stained sections in all the 90 cases while ki-67 immunostaining was done only on 44.44% (40/90) cases.Results: Apoptotic Index, Mitotic Index and MIB-1 expression were found to increase as the grade of lesion increased from cervical dysplasia to invasive cervical carcinoma. In CIN-I, MIB-1 positive cells were confined to lower one-third of ectocervix and full thickness was involved in CIN-III.Conclusion: We found proliferative indices (Mitotic index and MIB-1 labelling index) and Apoptotic index useful in determining the grading of dysplasia and cervical carcinoma.Key words: Cervical dysplasia, Cervical carcinoma, Apoptotic index, Mitotic index, MIB-1 Labelling inde

    A Comparative study between conventional method and the Bethesda system for reporting thyroid cytopathology (TBSRTC)

    Background: FNAC is widely used for the diagnosis of thyroid lesions. TBSRTC was introduced to streamline the reporting of thyroid aspirates. Objectives was to study the spectrum of various thyroid lesions in and around Meerut and comparison of conventional method and TBSRTC.Methods: A total of 464 patients were evaluated both by conventional system and TBSRTC.Results: As per conventional system of reporting, maximum number of cases were diagnosed as benign (87%), followed by malignant (6.4%), follicular neoplasia (2.5%), equivocal (2.1%) and inadequate (1.7%). As per TBSRTC, maximum number of cases were reported as category 2, 86.2% followed by category 6, 6%, category 1, 2.5% and cat 3,2.5%, category 4, 1.5% and category 5, 1%. Among benign category, maximum number of cases were of colloid goiter/multinodular goiter (MNG) (65%) followed by Hashimoto`s thyroiditis (15%), Grave`s disease (6%). Among the malignant category, maximum number of cases were of papillary carcinoma thyroid (PCT) (5.3%) followed by medullary carcinoma thyroid (MCT) (0.43%), non-Hodgkin`s lymphoma NHL (0.21%), metastatic carcinoma (0.21%), and anaplastic carcinoma (0.21%) of each.Conclusions: In present study it was found that there is an excellent agreement between the conventional system and TBSRTC. Results of both systems of reporting are comparable. In some cases, TBSRTC is almost equivalent to the conventional system in the pattern of reporting (viz. benign and malignant cat) while in some cases, TBSRTC is better in reporting (viz. FN and equivocal cat). The sensitivity of TBSRTC (93%) is slightly more as compared to the conventional system (90%) while its specificity (86%) slightly less as compared to conventional system (88%). Thus, as compared to the conventional system, TBSRTC may be viewed as a better screening test for thyroid lesions, though at the cost of specificity

    Hapi: A Robust Pseudo-3D Calibration-Free WiFi-based Indoor Localization System

    In this paper, we present Hapi, a novel system that uses off-the-shelf standard WiFi to provide pseudo-3D indoor localization. It estimates the user's floor and her 2D location on that floor. Hapi is calibration-free, only requiring the building's floorplans and its WiFi APs' installation location for deployment. Our analysis shows that while a user can hear APs from nearby floors as well as her floor, she will typically only receive signals from spatially closer APs in distant floors, as compared to APs in her floor. This is due to signal attenuation by floors/ceilings along with the 3D distance between the APs and the user. Hapi leverages this observation to achieve accurate and robust location estimates. A deep-learning based method is proposed to identify the user's floor. Then, the identified floor along with the user's visible APs from all floors are used to estimate her 2D location through a novel RSS-Rank Gaussian-based method. Additionally, we present a regression based method to predict Hapi's location estimates' quality and employ it within a Kalman Filter to further refine the accuracy. Our evaluation results, from deployment on various android devices over 6 months with 13 subjects in 5 different up to 9 floors multistory buildings, show that Hapi can identify the user's exact floor up to 95.2% of the time and her 2D location with a median accuracy of 3.5m, achieving 52.1% and 76.0% improvement over related calibration-free state-of-the-art systems respectively.Comment: Accepted for publication in MobiQuitous 2018 - the 15th International Conference on Mobile and Ubiquitous Systems: Computing, Networking and Service

    A comparative study for efficacy and safety of low doses of clonidine for hemodynamic stability in patients undergoing laparoscopic cholecystectomy

    Background: This randomized prospective double-blind study was designed to evaluate the efficacy and side effects of low doses clonidine for perioperative haemodynamic stability and postoperative recovery.Methods: Patient’s with ASA grade I–II undergoing laparoscopic cholecystectomy were randomized into three groups of 30 patients each. All patients received either normal saline 10 ml (Group I) or 0.8 µg/kg (Group II) or 1 µg/kg (Group III) over duration of 180 seconds, 10 min prior to laryngoscopy and intubation. Anaesthesia was induced with 1% propofol (2 mg/kg) and maintained with nitrous oxide 60% in oxygen and isoflurane. The parameters assessed at various time intervals were heart rate, systolic blood pressure, diastolic blood pressure, mean arterial pressure and sedation score.Results: Both doses of clonidine proved to be effective in perioperative haemodynamic stability. Clonidine 0.8 mcg/kg was as effectve and safer to Clonidine 1 mcg/kg for attenuatíon of the hemodynamíc responses to laparoscopy. There were no significant differences in the parameters of recovery between groups.Conclusions: Significant hemodynamic derangements can occur during laproscopic cholecystectomy at intubation, pneumoperitoneum and extubation. These were effectively attenuated by premedication with 0.8 mcg/kg and 1 mcg/kg of intravenous clonidine. Dose of 1 mcg/kg though found to be effective but produced adverse effects in form of hypotension and bradycardia
