    Capacity building of sea level and climate monitoring in the Pacific region: Fiji case study

    In order to enhance capacity building in the Pacific region, and to raise awareness on climate change and sea level issues, teaching and training modules were made available to the Pacific community through the “South pacific sea level and climate monitoring project” funded by AusAID. Numerous training workshops have been conducted through the project since its inception in 1991 and the project is now in its fourth and final phase. It was hoped that the goals of capacity building for the stakeholders on correct information of climate change and sea level have been understood and taken heed of. In addition, “The scientific educational resources and experience associated with the deployment of Argo” (SEREAD) project was also set up especially for ocean science in the Pacific island schools in 2001. However, it has been realized that the data from this project is more relevant to tertiary level rather than to secondary level students. Consequently, a survey was carried out to gauge the students’ outlook towards the physical side of marine science. The survey revealed that more than 80% of both tertiary level and high school non-physical science students decided not to take the physical aspect of marine science sighting reasons that it is either a difficult subject, boring, too hard to understand or difficult to pass in the examination. Even amongst students taking physical science, only about 50% believe that the physical aspect of marine science is enjoyable. A minority of students at USP and high school take science as a subject and a small proportion from them take the physical science. From this scenario, it can be predicted that there will be a shortage of physical science graduates in the future. The confidence of the Pacific community in the work of scientists is built on faith. They appreciate the effect of science on their lives and support it but are unaware of the scientific methods involved. Therefore, for the sustainability of physical aspect of marine science in the future for the Pacific region, public awareness of climate change and sea level is vitally important and it should be widely promoted in the community as a matter of urgency

    Outcomes of Induction vs Prelabor Cesarean Delivery at \u3c33 Weeks for Hypertensive Disorders of Pregnancy

    BACKGROUND: Hypertensive disorders of pregnancy are the leading cause of indicated preterm birth; however, the optimal delivery approach for pregnancies complicated by preterm hypertensive disorders of pregnancy remains uncertain. OBJECTIVE: This study aimed to compare maternal and neonatal morbidity in patients with hypertensive disorders of pregnancy who either went induction of labor or prelabor cesarean delivery at \u3c33 \u3eweeks\u27 gestation. In addition, we aimed to quantify the length of induction of labor and rate of vaginal delivery in those who underwent induction of labor. STUDY DESIGN: This is a secondary analysis of an observational study which included 115,502 patients in 25 hospitals in the United States from 2008 to 2011. Patients were included in the secondary analysis if they were delivered for pregnancy associated hypertension (gestational hypertension or preeclampsia) between 23 RESULTS: A total of 471 patients met inclusion criteria, of whom 271 (58%) underwent induction of labor and 200 (42%) underwent prelabor cesarean delivery. Composite maternal morbidity was 10.2% in the induction group and 21.1% in the cesarean delivery group (unadjusted odds ratio, 0.42 [0.25-0.72]; adjusted odds ratio, 0.44 [0.26-0.76]). Neonatal morbidity in the induction group vs the cesarean delivery was 51.9% and 63.8 %, respectively (unadjusted odds ratio, 0.61 [0.42-0.89]; adjusted odds ratio, 0.71 [0.48-1.06]). The frequency of vaginal delivery in the induction group was 53% (95% confidence interval, 46.8-58.7) and the median duration of labor was 13.9 hours (interquartile range, 8.7-22.2).     Short-Term Neonatal Outcomes of Pregnancies Complicated by Maternal Obesity

    BACKGROUND: Maternal obesity complicates a high number of pregnancies. The degree to which neonatal outcomes are adversely affected is unclear. OBJECTIVE: This study aimed to evaluate neonatal outcomes of pregnancies complicated by maternal obesity. STUDY DESIGN: This study was a secondary analysis of a cohort of deliveries occurring on randomly selected days at 25 hospitals from 2008 to 2011. Data were collected by certified abstractors. This analysis included singleton deliveries between 24 and 42 weeks of gestation. Body mass index was calculated on the basis of maternal height and most recent weight before delivery. Normal and overweight (reference group; body mass index, 18.5-29.9 kg/m RESULTS: Overall, 52,162 patients and their neonates were included after propensity score matching. Of these, 21,704 (41.6%) were obese, 3787 (7.3%) were morbidly obese, and 590 (1.1%) were super morbidly obese. A total of 2103 neonates (4.0%) had the composite outcome. Neonates born to pregnant people with morbid obesity had a 33% increased risk of composite neonatal morbidity compared with those in the reference group (adjusted odds ratio, 1.33; 95% confidence interval, 1.17-1.52), but no significant association was observed for persons with obesity (adjusted odds ratio, 1.05; 95% confidence interval, 0.97-1.14) or with super morbid obesity (adjusted odds ratio, 1.18; 95% confidence interval, 0.86-1.64). CONCLUSION: Compared with the reference group, gravidas with morbid obesity were at higher risk of composite neonatal morbidity

    Defining failed induction of labor

    BACKGROUND: While there are well-accepted standards for the diagnosis of arrested active-phase labor, the definition of a "failed" induction of labor remains less certain. One approach to diagnosing a failed induction is based on the duration of the latent phase. However, a standard for the minimum duration that the latent phase of a labor induction should continue, absent acute maternal or fetal indications for cesarean delivery, remains lacking. OBJECTIVE: The objective of this study was to determine the frequency of adverse maternal and perinatal outcomes as a function of the duration of the latent phase among nulliparous women undergoing labor induction. METHODS: This study is based on data from an obstetric cohort of women delivering at 25 U.S. hospitals from 2008-2011. Nulliparous women who had a term singleton gestation in the cephalic presentation were eligible for this analysis if they underwent a labor induction. Consistent with prior studies, the latent phase was determined to begin once cervical ripening had ended, oxytocin was initiated and rupture of membranes (ROM) had occurred, and was determined to end once 5 cm dilation was achieved. The frequencies of cesarean delivery, as well as of adverse maternal (e.g., cesarean delivery, postpartum hemorrhage, chorioamnionitis) and perinatal outcomes (e.g., a composite frequency of either seizures, sepsis, bone or nerve injury, encephalopathy, or death), were compared as a function of the duration of the latent phase (analyzed with time both as a continuous measure and categorized in 3-hour increments). RESULTS: A total of 10,677 women were available for analysis. In the vast majority (96.4%) of women, the active phase had been reached by 15 hours. The longer the duration of a woman's latent phase, the greater her chance of ultimately undergoing a cesarean delivery (P<0.001, for time both as a continuous and categorical independent variable), although more than forty percent of women whose latent phase lasted for 18 or more hours still had a vaginal delivery. Several maternal morbidities, such as postpartum hemorrhage (P < 0.001) and chorioamnionitis (P < 0.001), increased in frequency as the length of latent phase increased. Conversely, the frequencies of most adverse perinatal outcomes were statistically stable over time. CONCLUSION: The large majority of women undergoing labor induction will have entered the active phase by 15 hours after oxytocin has started and rupture of membranes has occurred. Maternal adverse outcomes become statistically more frequent with greater time in the latent phase, although the absolute increase in frequency is relatively small. These data suggest that cesarean delivery should not be undertaken during the latent phase prior to at least 15 hours after oxytocin and rupture of membranes have occurred. The decision to continue labor beyond this point should be individualized, and may take into account factors such as other evidence of labor progress

    Genetic Drivers of Heterogeneity in Type 2 Diabetes Pathophysiology

    Type 2 diabetes (T2D) is a heterogeneous disease that develops through diverse pathophysiological processes1,2 and molecular mechanisms that are often specific to cell type3,4. Here, to characterize the genetic contribution to these processes across ancestry groups, we aggregate genome-wide association study data from 2,535,601 individuals (39.7% not of European ancestry), including 428,452 cases of T2D. We identify 1,289 independent association signals at genome-wide significance (P \u3c 5 × 10-8) that map to 611 loci, of which 145 loci are, to our knowledge, previously unreported. We define eight non-overlapping clusters of T2D signals that are characterized by distinct profiles of cardiometabolic trait associations. These clusters are differentially enriched for cell-type-specific regions of open chromatin, including pancreatic islets, adipocytes, endothelial cells and enteroendocrine cells. We build cluster-specific partitioned polygenic scores5 in a further 279,552 individuals of diverse ancestry, including 30,288 cases of T2D, and test their association with T2D-related vascular outcomes. Cluster-specific partitioned polygenic scores are associated with coronary artery disease, peripheral artery disease and end-stage diabetic nephropathy across ancestry groups, highlighting the importance of obesity-related processes in the development of vascular outcomes. Our findings show the value of integrating multi-ancestry genome-wide association study data with single-cell epigenomics to disentangle the aetiological heterogeneity that drives the development and progression of T2D. This might offer a route to optimize global access to genetically informed diabetes care

    Sea level threat in Tuvalu

    Problem statement: Recently the impacts of climate change, in particular, sea level rise, had been a major concern for many Pacific island countries. In early 2000, there were a series of media coverage over sea level rise issues using Tuvalu as an example. The daily life of Tuvalu revolves around the ocean and the immediate threat on the islands people, economy, environment and its islands is of concern to the Tuvalu government. The Tuvalu government has concluded that Tuvalu was destined to become the first nation to be sunk by global warming because it is one of the smallest and lowest-lying countries in the world. Approach: In this study, sea level data from the Australian project will be focussed on despite the fact that the length of data is not sufficiently long. The AusAID funded South Pacific Sea Level and climate monitoring project was set up in response to concerns raised by Pacific island countries over the potential impacts of an enhanced greenhouse effect on climate and sea levels in the South Pacific for 20 years. Results: Based upon the 15Âœ years of sea level data from the project, the sea level rise rate in Tuvalu as at september 2008 was 5.9 mM year−1. This was about four times higher than the global average of 1-2 mm year−1. Sea level in the Tuvalu area had risen approximately 9.14 cm since the inception of the project 15Âœ years ago. However, it was to be noted that the land is quite stable and the rate of land sinking is -0.06 mM year−1 only. Accordingly, there was no significant impact on the sea level trends. Conclusion: Although the data length is just over 15 years, the sea level trend values do not fluctuate significantly since 1999. It simply indicated that the rate of sea level rise in the Tuvalu region was not accelerating as anticipated by the community
