    Resumo I (PrĂĄtica PedagĂłgica) Este RelatĂłrio de EstĂĄgio Ă© composto por duas secçÔes. Na primeira descreve-se o estĂĄgio realizado, na Classe de PercussĂŁo do Professor Carlos Voss (CV), na Escola de MĂș-sica do ConservatĂłrio Nacional (EMCN), ao longo do ano letivo de 2015/2016, com a su-pervisĂŁo do Professor Doutor Eduardo JosĂ© Tavares Lopes. ApĂłs uma contextualização da EMCN e dos alunos acompanhados no estĂĄgio, descreve-se o desempenho de cada um deles, ao longo do ano letivo, bem como as prĂĄticas educativas desenvolvidas pelo Professor Coo-perante, nas aulas assistidas e pelo estagiĂĄrio. Resumo II (Investigação) Na segunda parte, descreve-se o projeto de investigação, que consiste na elaboração de um Programa Curricular de Iniciação de Bateria pretendendo organizar e estruturar a in-formação necessĂĄria na ĂĄrea do ensino de Bateria para os quatro anos da Iniciação Musical. A escolha de um modelo programĂĄtico assente em objetivos comportamentais, dos quais a motricidade global, motricidade fina, consciĂȘncia corporal, autonomia e principalmente a linguagem da Bateria, tendo em consideração as competĂȘncias envolvidas na aprendizagem musical, bem como, os processos envolvidos na aquisição dessas mesmas competĂȘncias; Abstract: Supervised teaching Practice report held at the National Conservatory music school: teaching Drums in initiation courses: Curriculum, skills and goals. Abstract I (Teaching) This Training Report consists of two sections. The first describes the stage conducted by master's degree in Teacher Battery Class Carlos Voss (CV), the School of the National Conservatory Music (EMCN) throughout the academic year of 2015/2016, under the super-vision of Professor Eduardo JosĂ© Tavares Lopes. After a contextualization of EMCN and students accompanied on stage, we describe the performance of each of them, throughout the school year as well as the educational practices developed by Professor Cooperative, in assisted classes, and by the trainee. Abstract II (Research) Title - “The Battery Teaching in Initiation courses: Program Curriculum, competencies and objectives” In the second part, the research project is described, which consists of the elaboration of a Curricular Initiation Program for Drums intended to organize and structure the necessary training in the area of teaching the Drum for the four years of Music Initiation. The choice of a programmatic model based on behavioral objectives, of which the global motricity, fine motor, body awareness, autonomy and mainly the language of the Drum, taking into account the competences involved in musical learning, as well as the processes involved in acquiring these same skills

    Central autonomic nervous system response to autonomic challenges is altered in patients with a previous episode of Takotsubo Cardiomyopathy

    Aims: Takotsubo cardiomyopathy is an intriguing disease characterized by acute transient left ventricular dysfunction usually triggered by an episode of severe stress. The excessive levels of catecholamines and the overactivation of the sympathetic system are believed to be the main pathophysiologic mechanisms of Takotsubo cardiomyopathy, but it is unclear whether there is a structural or functional signature of the disease. In this sense, our aim was to characterize the central autonomic system response to autonomic challenges in patients with a previous episode of Takotsubo cardiomyopathy when compared with a control group of healthy volunteers. Methods and results: Functional magnetic resonance imaging (fMRI) was performed in four patients with a previous episode of Takotsubo cardiomyopathy (average age of 6712 years) and in eight healthy volunteers (average age of 66 +/- 5 years) while being submitted to different autonomic challenges (cold exposure and Valsalva manoeuvre). The fMRI analysis revealed a significant variation of the blood oxygen level dependent signal triggered by the Valsalva manoeuvre in specific areas of the brain involved in the cortical control of the autonomic system and significant differences in the pattern of activation of the insular cortex, amygdala and the right hippocampus between patients with Takotsubo cardiomyopathy and controls, even though these regions did not present significant volumetric changes. Conclusion: The central autonomic response to autonomic challenges is altered in patients with Takotsubo cardiomyopathy, thus suggesting a dysregulation of the central autonomic nervous system network.     Caregivers of elderly people: Burden and Quality of Life

    Objetivo: Identificar os fatores que se relacionam Ă  sobrecarga produzida sobre cuidador no cuidado de idosos dependentes e sua correlação com a qualidade de vida. MĂ©todos: Estudo transversal, que avaliou variĂĄveis sĂłcio-demogrĂĄficas (sexo, faixa etĂĄria, escolaridade, grau de parentesco com o idoso e remuneração da atividade cuidador), a sobrecarga (Zarit Burden Interview) e a qualidade de vida (WHOQOL- Abbreviated) dos cuidadores de pacientes idosos do Hospital Dia do Idoso de AnĂĄpolis. Foram obtidas informaçÔes sobre o perfil do idoso dependente: o nĂ­vel de dependĂȘncia (Ă­ndice de Katz) e variĂĄveis demogrĂĄficas (sexo e faixa etĂĄria). As mĂ©dias das variĂĄveis categĂłricas foram analisadas estatisticamente pelo teste T-Student e ANOVA e entre as variĂĄveis quantitativas foi calculada correlação Pearson. Resultados: De 35 cuidadores incluĂ­dos, 83% apresentaram grau de sobrecarga no mĂ­nimo moderada. Cuidadores do sexo feminino, alfabetizadas, sem grau de parentesco com idoso obtiveram maiores mĂ©dias de sobrecarga. Idosos muito dependentes estiveram relacionados tambĂ©m a maiores Ă­ndices de sobrecarga. Evidenciou-se associação entre altos nĂ­veis de sobrecarga e baixos escores na percepção da qualidade de vida. ConclusĂ”es: Conclui-se que sobrecarga vivenciada pelos cuidadores de idosos com maior dependĂȘncia influencia na pior percepção de qualidade de vida e que hĂĄ um perfil de cuidadores mais suscetĂ­vel Ă  sobrecarga, contribuindo para conscientização da equipe de saĂșde sobre a importĂąncia de formular medidas de apoio ao cuidador e atĂ© de tratamento para que o cuidado seja exercido de forma eficiente no Hospital Dia do Idoso de AnĂĄpolis

    Pulmonary Embolism and Intracardiac Type A Thrombus with an Unexpected Outcome

    Detection of right heart thrombi (RHT) in the context of pulmonary thromboembolism (PE) is uncommon (4–18%) and increases the risk of mortality beyond the presence of PE alone. Type A thrombi are serpiginous and highly mobile and are thought to be originated from large veins and captured in-transit within the right heart. Optimal management of RHT is still uncertain. A 79-year-old woman, with a history of recent total hysterectomy with adnexectomy and a Wells procedure, presented to the emergency department following an episode of syncope. Computed tomography revealed bilateral PE and the presence of a right atrial thrombus. Transthoracic echocardiography demonstrated a free-floating type A thrombus in the right atrium, protruding into the right ventricle, and signs of pulmonary hypertension and right ventricle dysfunction. Considering the recent surgery and clinical stability, treatment with heparin alone was decided. Subsequent clinical improvement was observed and echocardiographic follow-up revealed complete thrombus dissolution and complete recovery of right ventricle function. Most authors recommend treatment of PE with RHT with thrombolysis or embolectomy followed by anticoagulation, although evidence is scarce. Individual risk of hemorrhage and operatory-related mortality should be taken into account when defining the treatment strategy especially when benefit is not firmly established

    Cancer mortality trends in Brazilian adults aged 80 and over from 2000 to 2017

    INTRODUCTION: Adults aged 80 and over represent the fastest growing segment of the population in emerging countries. Studies of cancer mortality trends in the oldest old population are scarce in Brazil. OBJECTIVE: To describe trends in cancer mortality in the Brazilian oldest old, by gender and cancer type, from 2000 to 2017. METHODS: This was a descriptive study with a time trend design, based on data from the Mortality Information System (of the Informatics Department of the Unified Health System). The variables analyzed were year of death, sex and cancer site. The five most common types of cancer were identified, and mortality rates and trends were calculated for each one. Trends were determined using joinpoint regression. In all cases where one or more joinpoints were statistically significant the average annual percent change (AAPC) was calculated based on the arithmetic mean of the annual percent change (APC), weighted by the length of each segment. The statistical significance of the APC and AAPC was estimated by calculating 95% confidence intervals (CI) with an alpha level of 0.05. RESULTS: Mortality rates increased over time (AAPC = 1.50; 95%CI, 1.20 - 1.70) in both males (AAPC = 1.90; 95%CI, 1.70 - 2.10) and females (AAPC = 1.30; 95%CI, 1.00 - 1.50). Men had higher mortality rates than women. The most common causes of cancer-related death were prostate cancer (AAPC = 1.70; 95%CI, 1.10 - 2.30) in men, and breast cancer (AAPC = 1.90; 95%CI, 1.50 - 2.20) in women, followed by cancers of the lung and bronchus, stomach and colon. All rates increased over time, except in the case of stomach cancer. CONCLUSION: The study revealed increasing mortality rates for screenable and/or preventable cancers, alerting to the need for preventive measures.</p
