    The clinical reasoning processes of extended scope physiotherapists assessing patients with low back pain

    IntroductionEmploying allied health professionals in extended scope roles has developed relatively recently in health-care. Within physiotherapy, the extended role has provided clinicians with autonomy to use knowledge and clinical acumen to request investigations such as Magnetic Resonance Imaging (MRI) and X-ray as part of the diagnostic process, a practice beyond the traditional scope of physiotherapy. In these advancing roles, little is written about the clinical reasoning processes that clinicians use in managing patients with musculoskeletal pain and knowledge of these processes would advance training for new recruits to this arena.StudyThis qualitative study has explored the processes by which extended scope physiotherapists (ESPs) clinically reason their decisions regarding patients reporting low back pain in a musculoskeletal outpatient setting. The study used a multiple case study design informed by grounded theory methodology, using focus groups (involving ESPs and non-ESPs/musculoskeletal physiotherapists) and semi-structured interviews with a think-aloud method (ESPs only) to investigate these processes.ConclusionsThe themes identified include: prior thinking; patient interaction; formal testing; time; safety and accountability; external and internal factors; and gut-feeling (which challenges current physiotherapy models of reasoning). Extended scope physiotherapists reported experiencing greater stress due to higher levels of perceived accountability, safety requirements and internal drivers for competence than non-ESPs. Further research is indicated to explore the role of gut-feeling in musculoskeletal physiotherapy clinical reasoning

    The role of clinician emotion in clinical reasoning: balancing the analytical process

    IntroductionThis review paper identifies and describes the role of clinicians' memory, emotions and physical responses in clinical reasoning processes. Clinical reasoning is complex and multi-factorial and key models of clinical reasoning within musculoskeletal physiotherapy are discussed, highlighting the omission of emotion and subsequent physical responses and how these can impact upon a clinician when making a decision.DiscussionIt is proposed that clinicians should consider the emotions associated with decision-making, especially when there is concern surrounding a presentation. Reflecting on practice in the clinical environment and subsequently applying this to a patient presentation should involve some acknowledgement of clinicians' physical responses, emotions and how they may play a part in any decision made. Presenting intuition and gut-feeling as separate reasoning methods and how these processes co-exist with other more accepted reasoning such as hypothetico-deductive is also discussed.ConclusionMusculoskeletal physiotherapy should consider the elements of feelings, emotions and physical responses when applying reflective practice principles. Furthermore, clinicians dealing with difficult and challenging presentations should look at the emotional as well as the analytical experience when justifying decisions and learning from practice

    Observing a Corpus Christi procession in post-socialist Hungary, this article uses a textual analysis to explore how the ritual mirrors post-socialist trends that affirm Hungarian identity. This article serves to both document an interesting ritual procession but also view it in light of growing ethno-nationalism that both unites a community yet also shows exclusion of others. It is like a mirror at a microcosmic level that reflects a kind of ritualization of ethno-nationalism

    Washing Machine Races, Gulag Pizza, and Mc Lenin: Kitsch-ification in Post Socialist Hungary

    During the cold war, material culture could symbolically mark conflicts between the East and West. Lenin, Marx, the sickle and hammer symbolized communist strength. Household items and electronics were marveled at as new and advanced in contrast to the Western capitalist exotic “Other.” In Hungary today these symbols become Communist kitsch that can undermine former tensions and illustrate a collective ambivalence towards the post-socialist condition by (1) reinterpreting time and space, (2) symbolizing the cold war ‘other,’ (3) creating fantasized spaces, (4) and by representing a generational divide. Making former symbols of Communist strength ‘kitschy’ illustrates a mockery of the past, which becomes fodder for humor much like Bahktin’s discussion of inverse of power. The kitschification of Communism evokes various understandings of nostalgia in material form that underscores reinvented commentaries on the past as well as a response to an uncertain present and in the process neutralizes political complexity

    Turkey Backbones and Chicken Gizzards: Women’s Roles and the Making of a Soup King in Post Socialist Hungary

    This article looks at soup making as a lens to view the impact of societal change for Hungarian pensioner women. Food as a practice illustrates agency: strategies and tactics used in time and space to communicate meaning for people in everyday life. During the Socialist period women endured frustrations of long lines and scarce resources. Post socialism, their heroic clout as food providers is diminished by the introduction of a market economy. However, the survival skills learned in the socialist era allow them to adapt to the new era of high inflation, and high unemployment. I use descriptions of one woman to connect personal experience to historical and political change. This paper makes two main points – first, to look at food practices developed during Socialism that carry over and adapt to the Post socialist condition; and second, to illustrate how this impacts gender and agency of elderly Hungarian women
