    Evaluation of the implementation of the smoking area banning regulations on smoking behavior of government officials in Boalemo, GorontaloPurposeThis study aimed to investigate implementation of regional regulations for banning smoking on employees smoking behavior in a government office in Boalemo. MethodsThis research used a qualitative approach with a case study design involving 23 subjects. Data collection used systematic observation, regulatory documents, in-depth interviews and focus group discussions. ResultsBackground process of drafting rules were, 1) idea of regent, 2) translation of indicators of clean and health living behavior, 3) Ministry of Health, 4) illness and death data caused by smoking, and 5) impact of smoking. Effectiveness of policy implementation for area smoking ban was largely determined by, 1) input: availability of facilities dedicated to smoking area, effort control, labeling, guidance system and form of communication channel, 2) process: leadership support, stakeholder involvement, social norm, legal regulation regent, monitoring, and evaluation. Rules indirectly provide positive effect for output of small portion of employees in terms of, 1) intention to quit smoking, 2) frequency of smoking in room reduced, and 3) there were some employees who quit smoking. ConclusionImplementation of a policy was determined by: 1) role of leader as indicator of example and role model for subordinates, 2) support and role of all relevant stakeholder, and 3) social norms that encouraged positive behavioral changes in smoking

    Unbiased bases (Hadamards) for 6-level systems: Four ways from Fourier

    In quantum mechanics some properties are maximally incompatible, such as the position and momentum of a particle or the vertical and horizontal projections of a 2-level spin. Given any definite state of one property the other property is completely random, or unbiased. For N-level systems, the 6-level ones are the smallest for which a tomographically efficient set of N+1 mutually unbiased bases (MUBs) has not been found. To facilitate the search, we numerically extend the classification of unbiased bases, or Hadamards, by incrementally adjusting relative phases in a standard basis. We consider the non-unitarity caused by small adjustments with a second order Taylor expansion, and choose incremental steps within the 4-dimensional nullspace of the curvature. In this way we prescribe a numerical integration of a 4-parameter set of Hadamards of order 6.Comment: 5 pages, 2 figure

    Group measure space decomposition of II_1 factors and W*-superrigidity

    We prove a "unique crossed product decomposition" result for group measure space II_1 factors arising from arbitrary free ergodic probability measure preserving (p.m.p.) actions of groups \Gamma in a fairly large family G, which contains all free products of a Kazhdan group and a non-trivial group, as well as certain amalgamated free products over an amenable subgroup. We deduce that if T_n denotes the group of upper triangular matrices in PSL(n,Z), then any free, mixing p.m.p. action of the amalgamated free product of PSL(n,Z) with itself over T_n, is W*-superrigid, i.e. any isomorphism between L^\infty(X) \rtimes \Gamma and an arbitrary group measure space factor L^\infty(Y) \rtimes \Lambda, comes from a conjugacy of the actions.     Exploration of finite dimensional Kac algebras and lattices of intermediate subfactors of irreducible inclusions

    We study the four infinite families KA(n), KB(n), KD(n), KQ(n) of finite dimensional Hopf (in fact Kac) algebras constructed respectively by A. Masuoka and L. Vainerman: isomorphisms, automorphism groups, self-duality, lattices of coideal subalgebras. We reduce the study to KD(n) by proving that the others are isomorphic to KD(n), its dual, or an index 2 subalgebra of KD(2n). We derive many examples of lattices of intermediate subfactors of the inclusions of depth 2 associated to those Kac algebras, as well as the corresponding principal graphs, which is the original motivation. Along the way, we extend some general results on the Galois correspondence for depth 2 inclusions, and develop some tools and algorithms for the study of twisted group algebras and their lattices of coideal subalgebras.     Recurrent vulvar carcinoma: complex surgical treatment via perineal excision and reconstruction with musculocutaneous flap

    Vulvar cancer is a malignant disease having a low frequency and with well-established surgical and oncological treatments based on the stage of the disease. The most important therapeutic problem encountered is represented by cases of perineal local regional recurrence, which are common in patients with large primary tumors and can occur even if the margins of the resection had no tumoral invasion. We present a case study of a 64-year-old patient diagnosed one year ago with squamous vulvar carcinoma (G3) for which a vulvectomy was performed after neoadjuvant radiotherapy. The patient later developed local recurrence with invasion of the anal sphincter, creating a delicate problem regarding a surgical approach. The size and the extent of the recurrent tumor required a complex surgical intervention using a mixed surgical team of general surgeons and plastic surgeons. Surgical intervention with a large excision of the recurrent cancer along with amputation of the inferior rectum via perineal route, and creation of a left iliac anus was performed. The perineal defect was covered via a musculocutaneous flap using the gracilis muscle. The immediate post-operative evolution was favorable
