Edukacja terapeutyczna chorych na cukrzycę
Pojęcie edukacji terapeutycznej ma długą historię,
chociaż prawdziwe znaczenie tej metody postępowania
zostało upowszechnione dopiero w ostatnich
latach. Światowa Organizacja Zdrowia (WHO, World
Health Organization) włączyła edukację leczniczą do
modeli postępowania w chorobach przewlekłych.
Wiedza pacjenta o chorobie poprawia bowiem jego
współpracę z personelem medycznym. Edukacja jest
szczególnie ważna w cukrzycy, ponieważ stosowanie
się chorych do zaleceń medycznych i zapobieganie
powstawaniu ostrych i przewlekłych powikłań
cukrzycy wpływa na jakość życia. Edukacja terapeutyczna
powinna być prowadzona przez zawodowych
edukatorów w małych grupach. Kursy powinny dotyczyć
wszystkich zagadnień związanych z leczeniem
tej choroby. Przy doborze osób do poszczególnych
grup należy uwzględnić środowisko oraz problemy
chorych.
INTRODUCTION. Education of diabetic patients is
a key component of diabetes management which
significantly influences the risk of acute and longterm
diabetic complications. The aim of the study
was to determine the patients’ knowledge of diabetes,
to develop a health education plan and to
establish what methods and forms of education
would be more suitable for patients.
MATERIAL AND METHODS. The study included
100 type 2 diabetic patients of Municipal Diabetes
Center of Bialystok. Authors used an anonymous
questionnaire as an assessment tool.
RESULTS. Almost 37% of patients did not recognize
the symptoms of hypoglycemia, and 31% did not
know the risk factors of hypoglycemia. 52% of patients
lacked sufficient knowledge about prevention
of diabetic foot. More than 90% of participants confirmed
that education improves the patients’ mood
and helps them to become more motivated to manage
diabetes.
CONCLUSIONS. The study demonstrated that the
patients’ knowledge about self-control and self-management is insufficient and health education
is the key activity to increase awareness of the possibility
of patients’ condition and quality of life improvement.
The aim of education is to provide patients
with the knowledge of their illness, self-care
skills, and psycho-emotional support and motivation
to cope with practical and emotional problems.
Methods and forms of health education should be
adjusted to individual needs of each patient.WSTĘP. Edukacja chorych na cukrzycę stanowi istotne
uzupełnienie leczenia i znacząco wpływa na obniżenie
ryzyka ostrych oraz późnych powikłań cukrzycy.
Celem pracy było określenie wiedzy pacjentów
na temat cukrzycy, zaplanowanie edukacji zdrowotnej
oraz ustalenie, jakie metody i formy szkolenia
najbardziej odpowiadają chorym.
MATERIAŁ I METODY. Badaniem objęto 100 chorych
na cukrzycę typu 2 leczonych w Miejskim Ośrodku
Diabetologicznym w Białymstoku. Narzędziem badawczym
była anonimowa ankieta.
WYNIKI. Prawie 37% ankietowanych nie znało objawów
hiperglikemii, a 31% przyczyn hipoglikemii.
Istotne braki w wiedzy na temat profilaktyki stopy
cukrzycowej stwierdzono u 52% respondentów. Powyżej
90% badanych potwierdziło, że edukacja pozytywnie
wpływa na samopoczucie chorych, a także
zwiększa motywację do leczenia.
WNIOSKI. Wykazano, że u chorych poziom wiedzy
dotyczącej samokontroli i samoopieki jest niewystarczający,
a edukacja zdrowotna jest podstawowym
działaniem zwiększającym świadomość na temat
możliwości poprawy zdrowia i jakości życia. Działania
edukacyjne muszą nie tylko umożliwiać chorym
zdobycie wiedzy, umiejętności oraz zwiększać motywację
psychoemocjonalną, ale także powinny kreować
nowe wartości osobowe i moralne w walce z chorobą.
Metody i formy edukacji zdrowotnej należy dostosować
do indywidualnych potrzeb każdego pacjenta

Mental Mapping Method in Anthropological Studies on Cultural Landscape
This paper presents the method of mental mapping, adapted for the purposes of anthropological research on perception and assessment of urbanised spaces. Its application is also discussed in the evaluation of cultural landscape, taking into account the aspect of human experience. The method allows for examination of the inhabited environment’s perceptions and their externalisation in the form of mental maps constructed on the basis of residents’ relations of examined locations. The analysis of mental maps allows both for identification and description of elementsthat create the structure of the space and the evaluation of their role in the life of the community and in the formation of a harmonious landscape. The source basis of the article, apart from works referred to in the bibliography, are ethnographic field studies conducted between 2014 and 2017 in ten villages in the Kielce Voivodeship and in Kielce
Nyckelharpa z Wolina. Przyczynek do historii instrumentów strunowych w średniowiecznej Europie
The text presents the results of a functional analysis and interpretation of a wooden object discovered during archaeological works in Wolin. According to the authors of the article, it is the top plate of the soundboard of a stringed instrument called the nyckelharpa. The results of a radiocarbon test indicate that the Wolin instrument is now the oldest known instrument of this type
TMPRSS6 rs855791 Polymorphism Status in Children with Celiac Disease and Anemia
Celiac disease (CD) is an autoimmune chronic inflammatory disease occurring in genetically predisposed individuals in response to the intake of gluten. Clinical presentation can be heterogeneous. Iron-deficient anemia (IDA) is one of the most common extra-intestinal manifestations of CD. Although IDA usually reverts with a gluten-free diet (GFD), some patients show persistent IDA, the mechanisms of which are poorly understood. Recent studies suggest an association between the rs855791 polymorphism in the TMPRSS6 gene and persistent IDA in adults with CD. The current study aimed to assess the potential link between rs855791 and persistent IDA in pediatric patients with CD. The study included 106 children diagnosed with CD between 2015 and 2019. Clinical and blood parameters (including blood count, serum iron) were collected at diagnosis and after ≥12 months of GFD, and the rs855791 genotype was assessed for each patient. IDA was present at diagnosis in 25 patients (23.6%); only three (3%) had persistent IDA after GFD. The prevalence of rs855791 genotypes was 9% (n = 10) for TT, 53% (n = 56) for CT, and 38% (n = 40) for CC. There was a tendency toward a higher proportion of the T allele in patients with IDA and lower hemoglobin in the TT genotype but without statistical significance. An association between rs855791 and persistent IDA was not observed. These findings suggest that persistent IDA is uncommon in pediatric patients with CD. The prevalence of rs855791 in children with CD is reported for the first time
A rare CD5-positive subgroup of diffuse large B-cell lymphoma – clinical, morphological and immunophenotypic features in Polish patients
The aim of the study was to assess the incidence of CD5-positive diffuse large B-cell lymphoma (DLBCL) in the Polish population and to describe its morphologic and clinical characteristics. The study included 36 patients with CD5-positive DLBCL, diagnosed and treated in the Maria Skłodowska-Curie Institute and Oncology Centre, Warsaw, Poland and the Medical University of Warsaw, Poland in the years 2002-2013. The control group consisted of 28 patients with CD5-negative DLBCL. CD5-positive DLBCL accounted for 6.26% of all DLBCL cases diagnosed in the Maria Skłodowska-Curie Institute and Oncology Centre in the years 2008-2012. The incidence is comparable to other European countries, lower than noted in Japan and higher than in the US. Patients with CD5-positive DLBCL, in comparison to the CD5-negative group, were characterized by: (1) older age (≥ 60 vs. younger) and worse general status (ECOG ≥ 2 vs. < 2), (2) lower frequency of complete remission (CR), (3) higher expression of unfavorable prognostic factors (BCL2, FOXP1, CD44) and MMP-9, and (4) lower expression of favorable prognostic factors (CD30, cyclin D1, cyclin D3) and TIMP-2
Atherosclerotic Risk Factors in Children with Celiac Disease
Introduction and Objectives. Celiac disease (CD) is a complex autoimmune disorder occurring in genetically susceptible individuals. There is limited data on the impact of gluten-free diet (GFD) on the risk of developing cardiovascular diseases. Hereby, we present our study regarding the impact of treatment with GFD on the biochemical risk factors of atherosclerosis. Material and Methods. 277 patients with CD from 7 Polish clinics were enrolled in the study (210 children treated for at least 5 years and 67 children included in the study on the day of CD diagnosis and observed for 1 year on a GFD). We obtained selected clinical data, and we assessed lipid profile, apolipoproteins (A1, B, and E), lipoprotein (a), homocysteine, as well as antioxidants (folic and uric acid), and high-sensitivity CRP (hsCRP) for all patients. The compliance to GFD was verified using anti-transglutaminase antibodies and deamidated gliadin peptide antibodies. As a reference group, the data of 95 healthy children recruited for another project was used, for which we had the results of selected parameters. Results. We found significantly lower concentrations of total cholesterol, lipoprotein LDL-C, apolipoproteins A1 and B, as well as hCRP in all children with CD. We showed decreased level (5 years. Moreover, we showed significant decrease of folic acid level already after 1 year of a GFD (12 vs. 5.6 ng/mL; p<0.001). We also found significant negative correlation of z-score body mass index (BMI) with HDL and APOA1 level (r=−0.33; p=0.015 and r=−0.28; p=0.038, respectively) and modest positive correlation of z-score BMI with atherogenic factor of total cholesterol-HDL ratio and LDL-HDL ratio (r=0.40; p=0.002 and r=0.36; p=0.006, respectively). Analysis of physical activity showed an increase in the insulin levels with inactivity (r=0.36; p=0.0025). We also found positive correlation of the sleep duration with the adiponectin level (r=0.41; p=0.011). Conclusions. In children with CD treated with a GFD, decreased level of folic acid together with increased BMI, sedentary behavior, and an improper lipid profile may predispose them to atherosclerosis in the long run. This data suggests the need of further studies to determine the need for metabolic cardiovascular risk screening in children with CD
