    Fluconazole resistance Candida albicans in females with recurrent vaginitis and Pir1 overexpression

    Background: Some genes may be associated with Candida albicans resistance to azoles. Pir1 gene is described as responsible to induce resistance in C. albicans. Objectives: The current study aimed to find the relationship between fluconazole resistance and Pir1 protein (Pir1p) overexpression in the females with recurrent C. albicans vaginitis requiring longer fluconazole therapy. Patients and Methods: A total of 52l vaginal samples were obtained from the females with C. albicans vaginitis. The azole susceptibility phenotype was determined according to the Clinical Laboratory for Standards Institute (CLSI) protocol for disk diffusion method and inhibition zone for fluconazole. Expression of pir1 gene and fluconazole -resistance were evaluated using polymerase chain reaction (PCR) in C. albicans. Results: In the 52 isolates, 49 (94) were resistant to fluconazole. Overexpression of Pir1 gene was detected in 47 (96) fluconazole-resistant C. albicans isolates. Conclusions: The findings show fluconazole -resistance in C. albicans isolates with overexpression of Pir1p. © 2015, Ahvaz Jundishapur University of Medical Sciences

    Feasibility study on Intensive polyculture of chainess and Indian carps in I.R.IRAN (Giulan province)

    500000 juvenile Indian majour carps (Labeo rohita,Cirhinus merigala, Catla catla) imported (2004) and transferred to the earthen ponds in Shahid Ansari Carp Breeding Center , In order to assess the viability of rearing under the climatic condition of north part of Iran and also achieve some growth parameters from the larval stage to market size.The fry with 300 mg weight, were released in to 8 ponds.specimens were separated to tree experimental categories: 1= (Labeo rohita) , 2= (Cirhinus merigala) , 3= (Catla catla). fishes were fed with artificial food and also with the natural products of the ponds which were enriched with organic fertilizers, after being equally stocked in the ponds .During the experiment, oxygen level ,PH, turbidity, and temperature were measured . Monthly fry samples were taken, for weight and length biometrical analysis and also determination of the weight gain percentage, average daily growth rate, specific growth rate. Results have indicated adaptations and considerable growth in this species, with an increase from the initial weight of 300 mg to an average of 13.5±1.6 mg in 50 days period until the end of November for fingerlings. Also the average weight of growing phase (second year of production) was 647g. For Labeo rohita ,420 g. for Cirhinus merigala and 734g. For Catla catla. The special growth rate(SGR)in Roho was 4.51 %.and 3.9 % for merigal and6.2 % for Catla while for Chinees carps SGR was 4.9, 6.7, 7.6 and 5.8 for silver carp,grass carp, common carp and big head respectively. Results revealed that the Indian majour carps compatibility with the condition in Gilan province was succesfull and the combination of species and optimization of their compatibility were remind for further studies

    Knowledge, attitude, and practice of Iranian health sciences students regarding hepatitis B and C virus infections: A national survey

    Background: The World Health Organization seeks to achieve the goal of viral hepatitis elimination by 2030 and lack of general knowledge about viral hepatitis seems to be a barrier to reaching this goal. This study was designed to evaluate the knowledge, attitude, and practice (KAP) regarding hepatitis B virus (HBV) and hepatitis C virus (HCV) infections among Iranian health sciences students in 12 Iranian medical sciences universities using a national survey. Methods: This survey was conducted during the second Hepatitis Awareness Campaign, which was held during the Iranian National Hepatitis Week (October 22-28, 2016). Students who visited our booths and were willing to participate in our survey were selected using convenience sampling and their HBV- and HCV-related KAP were evaluated. Results: Two thousand one hundred fifty-six health sciences students with mean age of 21.24 years participated in our survey. The mean KAP scores were 7.35 (out of 10), 4.88 (possible score, -20 to +20), and 5.67 (out of 9). Students with experience of accidental exposures to blood (21.6) had better KAP scores compared with the students without such experiences. The mean KAP scores were associated with subjects' year of education, field of study, university, and province (P < .05). Conclusions: Our study showed that HBV- and HCV-related KAP in a sample of Iranian health sciences students was not satisfying. The results also demonstrated priority of needing intervention regarding KAP in some subject areas, lower years of education, and some universities compared with others. © 2017 Association for Professionals in Infection Control and Epidemiology, Inc

    EULAR recommendations for the management of rheumatoid arthritis with synthetic and biological disease-modifying antirheumatic drugs: 2022 update

    ObjectivesTo provide an update of the EULAR rheumatoid arthritis (RA) management recommendations addressing the most recent developments in the field. MethodsAn international task force was formed and solicited three systematic literature research activities on safety and efficacy of disease-modifying antirheumatic drugs (DMARDs) and glucocorticoids (GCs). The new evidence was discussed in light of the last update from 2019. A predefined voting process was applied to each overarching principle and recommendation. Levels of evidence and strengths of recommendation were assigned to and participants finally voted on the level of agreement with each item. ResultsThe task force agreed on 5 overarching principles and 11 recommendations concerning use of conventional synthetic (cs) DMARDs (methotrexate (MTX), leflunomide, sulfasalazine); GCs; biological (b) DMARDs (tumour necrosis factor inhibitors (adalimumab, certolizumab pegol, etanercept, golimumab, infliximab including biosimilars), abatacept, rituximab, tocilizumab, sarilumab and targeted synthetic (ts) DMARDs, namely the Janus kinase inhibitors tofacitinib, baricitinib, filgotinib, upadacitinib. Guidance on monotherapy, combination therapy, treatment strategies (treat-to-target) and tapering in sustained clinical remission is provided. Safety aspects, including risk of major cardiovascular events (MACEs) and malignancies, costs and sequencing of b/tsDMARDs were all considered. Initially, MTX plus GCs is recommended and on insufficient response to this therapy within 3-6 months, treatment should be based on stratification according to risk factors; With poor prognostic factors (presence of autoantibodies, high disease activity, early erosions or failure of two csDMARDs), any bDMARD should be added to the csDMARD; after careful consideration of risks of MACEs, malignancies and/or thromboembolic events tsDMARDs may also be considered in this phase. If the first bDMARD (or tsDMARD) fails, any other bDMARD (from another or the same class) or tsDMARD (considering risks) is recommended. With sustained remission, DMARDs may be tapered but should not be stopped. Levels of evidence and levels of agreement were high for most recommendations. ConclusionsThese updated EULAR recommendations provide consensus on RA management including safety, effectiveness and cost.Pathophysiology and treatment of rheumatic disease
