    Viewpoint: filovirus haemorrhagic fever outbreaks: much ado about nothing?

    The recent outbreak of Marburg haemorrhagic fever in the Democratic Republic of Congo has put the filovirus threat back on the international health agenda. This paper gives an overview of Marburg and Ebola outbreaks so far observed and puts them in a public health perspective. Damage on the local level has been devastating at times, but was marginal on the international level despite the considerable media attention these outbreaks received. The potential hazard of outbreaks, however, after export of filovirus from its natural environment into metropolitan areas, is argued to be considerable. Some avenues for future research and intervention are explored. Beyond the obvious need to find the reservoir and study the natural history, public health strategies for a more timely and efficient response are urgently needed

    Effects of random alloy disorder, shape deformation, and substrate misorientation on the exciton lifetime and fine structure splitting of GaAs/AlxGa1-xAs(111) quantum dots

    Using atomistic, million-atom screened pseudopotential theory together with configuration interaction, as well as atomically resolved structures based on experimental characterization, we perform numerical calculations on self-assembled GaAs/AlxGa1-xAs(111) quantum dots that we compare with our experimental data. We show that random alloy disorder in the barrier can cause a symmetry breaking at the single-particle level (distortions of wave functions and lifting of degeneracies) which translates into the appearance of a nonzero exciton fine structure splitting (FSS) at the many-body level. Nevertheless, our results indicate that varying the concentration of aluminum in the random alloyed barrier allows simultaneous tuning of the exciton fine structure splitting and emission wavelength without altering its radiative lifetime tau approximate to 200 ps. Additionally, the optical properties of these quantum dots are predicted to be very robust against both symmetric and asymmetric shape elongation (with FSS 2.2 mu eV), rendering postselection less essential under well-controlled growth conditions. On the other hand, the growth on miscut substrates introduces a structural anisotropy along the quantization axis to which the system is very sensitive: the FSS ranges between 5 and 50 mu eV while the radiative lifetime of the transition is increased up to tau = 400 ps. The numerical results for the FSS are in perfect agreement with our experimental measurements which give FSS = 10 +/- 9 mu eV for 2 degrees miscut angle at x = 0.15

    Characterisation of the porous structure of Gilsocarbon graphite using pycnometry, cyclic porosimetry and void-network modeling

    file: :C:/pdf/1-s2.0-S000862231400164X-main.pdf:pdfThe cores of the fourteen Advanced Gas-cooled nuclear Reactors (AGRs) within the UK comprise Gilsocarbon graphite, a manufactured material surrounded predominantly by CO2 at high pressure and temperature to provide heat exchange. The intense ionising radiation within the reactors causes radiolytic oxidation, and the resulting mass loss is a primary factor in determining reactor lifetime. The void structure of the porous Gilsocarbon graphite affects the permeability and diffusion of the carbon dioxide, and the sites of oxidation. To model this void structure, the porosities and densities of ten virgin Gilsocarbon graphite samples have been measured by powder and helium pycnometry. For comparison, results are also presented for highly ordered pyrolytic graphite (HOPG), and a fine-grained Ringsdorff graphite. Samples have been examined at a range of magnifications by electron microscopy. Total porosities and percolation characteristics have been measured by standard and cyclic mercury porosimetry up to an applied mercury pressure of 400MPa. Inverse modelling of the cyclic intrusion curves produces simulated void structures with characteristics which closely match those of experiment. Void size distributions of the structures are presented, together with much Supplementary Information. The simulated void networks provide the bases for future simulations of the radiolytic oxidation process itself

    Deep near-infrared imaging of the HE0450-2958 system

    The QSO HE0450-2958 and the companion galaxy with which it is interacting, both ultra luminous in the infrared, have been the subject of much attention in recent years, as the quasar host galaxy remained undetected. This led to various interpretations on QSO and galaxy formation and co-evolution, such as black hole ejection, jet induced star formation, dust obscured galaxy, or normal host below the detection limit. We carried out deep observations in the near-IR in order to solve the puzzle concerning the existence of any host. The object was observed with the ESO VLT and HAWK-I in the near-IR J-band for 8 hours. The images have been processed with the MCS deconvolution method (Magain, Courbin & Sohy, 1998), permitting accurate subtraction of the QSO light from the observations. The compact emission region situated close to the QSO, called the blob, which previously showed only gas emission lines in the optical spectra, is now detected in our near-IR images. Its high brightness implies that stars likely contribute to the near-IR emission. The blob might thus be interpreted as an off-centre, bright and very compact host galaxy, involved in a violent collision with its companion.Comment: 4 pages, 3 figures, accepted for publication in A&

    Marburg hemorrhagic fever in Durba and Watsa, Democratic Republic of the Congo: clinical documentation, features of illness, and treatment

    The objective of the present study was to describe day of onset and duration of symptoms of Marburg hemorrhagic fever (MHF), to summarize the treatments applied, and to assess the quality of clinical documentation. Surveillance and clinical records of 77 patients with MHF cases were reviewed. Initial symptoms included fever, headache, general pain, nausea, vomiting, and anorexia (median day of onset, day 1-2), followed by hemorrhagic manifestations (day 5-8+), and terminal symptoms included confusion, agitation, coma, anuria, and shock. Treatment in isolation wards was acceptable, but the quality of clinical documentation was unsatisfactory. Improved clinical documentation is necessary for a basic evaluation of supportive treatment

    Administration of single-dose GnRH agonist in the luteal phase in ICSI cycles: a meta-analysis

    <p>Abstract</p> <p>Background</p> <p>The effects of gonadotrophin-releasing hormone agonist (GnRH-a) administered in the luteal phase remains controversial. This meta-analysis aimed to evaluate the effect of the administration of a single-dose of GnRH-a in the luteal phase on ICSI clinical outcomes.</p> <p>Methods</p> <p>The research strategy included the online search of databases. Only randomized studies were included. The outcomes analyzed were implantation rate, clinical pregnancy rate (CPR) per transfer and ongoing pregnancy rate. The fixed effects model was used for odds ratio. In all trials, a single dose of GnRH-a was administered at day 5/6 after ICSI procedures.</p> <p>Results</p> <p>All cycles presented statistically significantly higher rates of implantation (P < 0.0001), CPR per transfer (P = 0.006) and ongoing pregnancy (P = 0.02) in the group that received luteal-phase GnRH-a administration than in the control group (without luteal-phase-GnRH-a administration). When meta-analysis was carried out only in trials that had used long GnRH-a ovarian stimulation protocol, CPR per transfer (P = 0.06) and ongoing pregnancy (P = 0.23) rates were not significantly different between the groups, but implantation rate was significant higher (P = 0.02) in the group that received luteal-phase-GnRH-a administration. On the other hand, the results from trials that had used GnRH antagonist multi-dose ovarian stimulation protocol showed statistically significantly higher implantation (P = 0.0002), CPR per transfer (P = 0.04) and ongoing pregnancy rate (P = 0.04) in the luteal-phase-GnRH-a administration group. The majority of the results presented heterogeneity.</p> <p>Conclusions</p> <p>These findings demonstrate that the luteal-phase single-dose GnRH-a administration can increase implantation rate in all cycles and CPR per transfer and ongoing pregnancy rate in cycles with GnRH antagonist ovarian stimulation protocol. Nevertheless, by considering the heterogeneity between the trials, it seems premature to recommend the use of GnRH-a in the luteal phase. Additional randomized controlled trials are necessary before evidence-based recommendations can be provided.</p

    Patient expression of emotions and neurologist responses in first multiple sclerosis consultations

    Background: Anxiety and depression are common in people with multiple sclerosis (MS), but data on emotional communication during MS consultations are lacking. We assessed patient expressions of emotion and neurologist responses during first-ever MS consultations using the Verona Coding Definitions of Emotional Sequences (VR-CoDES). Methods: We applied VR-CoDES to recordings/transcripts of 88 outpatient consultations (10 neurologists, four MS Italian centers). Before consultation, patients completed the Hospital Anxiety and Depression Scale (HADS). Multilevel sequential analysis was performed on the number of cues/concerns expressed by patients, and the proportion of reduce space responses by neurologists. Results: Patients expressed 492 cues and 45 concerns (median 4 cues and 1 concern per consultation). The commonest cues were verbal hints of hidden worries (cue type b, 41%) and references to stressful life events (type d, 26%). Variables independently associated with number of cues/concerns were: anxiety (HADS-Anxiety score &gt;8) (incidence risk ratio, IRR 1.08, 95% CI 1.06-1.09; p&lt;0.001); patient age (IRR 0.98, 95% CI 0.98-0.99; p&lt;0.001); neurologist age (IRR 0.94, 95% CI 0.92-0.96; p=0.03); and second opinion consultation (IRR 0.72, 95% CI 0.60-0.86; p=0.007). Neurologists reacted to patient emotions by reducing space (changing subject, taking no notice, giving medical advice) for 58% of cues and 76% of concerns. Anxiety was the only variable significantly associated with 'reduce space' responses (odds ratio 2.17, 95% CI 1.32-3.57; p=0.003). Conclusions: Patient emotional expressions varied widely, but VR-CoDES cues b and d were expressed most often. Patient anxiety was directly associated with emotional expressions; older age of patients and neurologists, and second opinion consultations were inversely associated with patient emotional expression. In over 50% of instances, neurologists responded to these expressions by reducing space, more so in anxious patients. These findings suggest that neurologists need to improve their skills in dealing with patient emotions

    HAWK-I imaging of the X-ray luminous galaxy cluster XMMU J2235.3-2557: The red sequence at z=1.39

    We use HAWK-I, the recently-commissioned near-IR imager on Yepun (VLT-UT4), to obtain wide-field, high-resolution images of the X-ray luminous galaxy cluster XMMU J2235.3-2557 in the J and Ks bands, and we use these images to build a colour-magnitude diagram of cluster galaxies. Galaxies in the core of the cluster form a tight red sequence with a mean J-Ks colour of 1.9 (Vega system). The intrinsic scatter in the colour of galaxies that lie on the red sequence is similar to that measured for galaxies on the red sequence of the Coma cluster. The slope and location of the red sequence can be modelled by passively evolving the red sequence of the Coma cluster backwards in time. Using simple stellar population (SSP) models, we find that galaxies in the core of XMMU J2235.3-2557 are, even at z=1.39, already 3 Gyr old, corresponding to a formation redshift of z ~ 4. Outside the core, the intrinsic scatter and the fraction of galaxies actively forming stars increase substantially. Using SSP models, we find that most of these galaxies will join the red sequence within 1.5 Gyr. The contrast between galaxies in the cluster core and galaxies in the cluster outskirts indicates that the red sequence of XMMU J2235.3-2557 is being built from the dense cluster core outwards.Comment: 9 pages, 2 figures.
