Traditional Chinese medicine in patients with osteoarthritis of the knee
AbstractTo evaluate whether the use of traditional Chinese medicine (TCM; 中醫 zhōng yī) influences symptoms or functional outcomes in patients with osteoarthritis (OA) of the knee (膝關節炎 xī guān jié yán).A systematic review of randomized control trials was conducted. Searches for studies in PubMed that were performed between 1965 and August 2013, and retrieved studies were subjected to reference screening. The types of studies included in our review were 1) placebo-based or comparative studies; 2) open label, single-blinded or double-blinded studies; 3) studies evaluating the efficacy of TCM for treating OA of the knee; and 4) studies evaluating only TCM or combination preparations. Trials were conducted with participants over 18 years of age with knee pain and at least three of the following characteristics: 1) an age greater than 50 years; 2) morning stiffness lasting for fewer than 30 min; 3) a crackling or grating sensation; 4) bony tenderness of the knee; 5) bony enlargement of the knee; or 6) no detectable warmth of the joint to the touch. Studies were rated for risk of bias and graded for quality.After screening, 104 studies that satisfied the eligibility requirements were identified, and only 18 randomized control trials were included in the quantitative and qualitative synthesis. Upon review, we found “moderate-quality” evidence of effects from acupuncture (針灸 zhēn jiǔ) on pain, which was measured using a visual analogue scale, and physical function, which was measured using qigong (氣功 qì gōng) with motion. “Low-quality” evidence was found regarding the effects of acupuncture on physical function, and no evidence was found regarding the effects of herbal medicine on pain or physical function. Herbal patches (藥布 yào bù) appeared to affect pain and physical and function, but these effects were not found to be significant.The initial findings included in this review suggest that acupuncture is a promising intervention according to the primary outcome measure, pain, and qigong with motion is an effective method for treating physical function. However, according to the Grades of Recommendation, Assessment, Development, and Evaluation criteria, only moderate-quality evidence was found in these studies. Metrology Camera System of Prime Focus Spectrograph for Subaru Telescope
The Prime Focus Spectrograph (PFS) is a new optical/near-infrared multi-fiber
spectrograph designed for the prime focus of the 8.2m Subaru telescope. PFS
will cover a 1.3 degree diameter field with 2394 fibers to complement the
imaging capabilities of Hyper SuprimeCam. To retain high throughput, the final
positioning accuracy between the fibers and observing targets of PFS is
required to be less than 10um. The metrology camera system (MCS) serves as the
optical encoder of the fiber motors for the configuring of fibers. MCS provides
the fiber positions within a 5um error over the 45 cm focal plane. The
information from MCS will be fed into the fiber positioner control system for
the closed loop control. MCS will be located at the Cassegrain focus of Subaru
telescope in order to to cover the whole focal plane with one 50M pixel Canon
CMOS camera. It is a 380mm Schmidt type telescope which generates a uniform
spot size with a 10 micron FWHM across the field for reasonable sampling of
PSF. Carbon fiber tubes are used to provide a stable structure over the
operating conditions without focus adjustments. The CMOS sensor can be read in
0.8s to reduce the overhead for the fiber configuration. The positions of all
fibers can be obtained within 0.5s after the readout of the frame. This enables
the overall fiber configuration to be less than 2 minutes. MCS will be
installed inside a standard Subaru Cassgrain Box. All components that generate
heat are located inside a glycol cooled cabinet to reduce the possible image
motion due to heat. The optics and camera for MCS have been delivered and
tested. The mechanical parts and supporting structure are ready as of spring

A Model for Selecting a Biomass Furnace Supplier Based on Qualitative and Quantitative Factors
In developing countries, solar energy is the largest source of energy, accounting for 35%–45% of the total energy supply. This energy resource plays a vital role in meeting the energy needs of the world, especially in Vietnam. Vietnam has favorable natural conditions for this energy production. Because it is hot and humid, and it has much rainfall and fertile soil, biomass develops very quickly. Therefore, byproducts from agriculture and forestry are abundant and continuously increasing. However, byproducts that are considered natural waste have become the cause of environmental pollution; these include burning forests, straw, and sawdust in the North; and rice husks dumped into rivers and canals in the Mekong Delta region. Biomass energy is provided in a short cycle, is environmentally safe to use and is encouraged by organizations that support sustainable development. Taking advantage of this energy source provides energy for economic development and ensures environmental protection. Due to the abovementioned favorable conditions, many biomass energy plants are being built in Vietnam. Like other renewable energy investment projects, the selection of the construction contractor, the selection of equipment for the installation of the power plant, and the choice of construction site are complex multi-criteria decisions. In this case, decision-makers must evaluate many qualitative and quantitative factors. These factors interact with each other and it is difficult to use personal experience to choose the optimal solution for such complex decision-making problems, especially in a fuzzy decision-making environment. Therefore, in this study, the authors use a Multi-Criteria Decision-Making (MCDM) model that uses a Fuzzy Analytic Hierarchy Process (FAHP) model and the Combined Compromise Solution (CoCoSo) algorithm to select biomass furnace suppliers utilizing both qualitative and quantitative factors. Furthermore, the results of this work will provide the first look at a hybrid CoCoSo/FAHP method that decision-makers in other fields can use to find the best supplier
Genetic diversity and C2-like subgenogroup strains of enterovirus 71, Taiwan, 2008
<p>Abstract</p> <p>Background</p> <p>Human enterovirus 71 (EV-71) is known of having caused numerous outbreaks of hand-foot-mouth disease, and other clinical manifestations globally. In 2008, 989 EV-71 strains were isolated in Taiwan.</p> <p>Results</p> <p>In this study, the genetic and antigenic properties of these strains were analyzed and the genetic diversity of EV-71 subgenogroups surfacing in Taiwan was depicted, which includes 3 previously reported subgenogroups of C5, B5, and C4, and one C2-like subgenogroup. Based on the phylogenetic analyses using their complete genome nucleotide sequences and neutralization tests, the C2-like subgenogroup forms a genetically distinct cluster from other subgenogroups, and the antisera show a maximum of 128-fold decrease of neutralization titer against this subgenogroup. In addition, the subgenogroup C4 isolates of 2008 were found quite similar genetically to the Chinese strains that caused outbreaks in recent years and thus they should be carefully watched.</p> <p>Conclusions</p> <p>Other than to be the first report describing the existence of C2-like subgenogroup of EV-71 in Taiwan, this article also foresees a potential of subgenogroup C4 outbreaks in Taiwan in the near future.</p

Pediatric Thalassemic Patients Have Higher Incidence of Asthma: A Nationwide Population-Based Retrospective Cohort Study
INTRODUCTION: Patients with hemoglobinopathies have been reported to have higher rates of pulmonary complications. Few studies have investigated the association between thalassemia and asthma in children.
METHODS: We used the data of one million individuals randomly selected from the Registry for Beneficiaries of the National Health Insurance Research Database. One thalassemic child was matched with four control children without thalassemia according to sex, birth year, birth season, prematurity, and previous enteroviral infection.
RESULTS: A total of 800 hundred thalassemic children and 3200 controls were included. Children with thalassemia had higher rates of developing asthma (41.81 vs 25.70 per 1000 person-years, P \u3c 0.001) than the non-thalassemia controls with an adjusted hazard ratio of 1.37 (95% confidence interval [CI] = 1.19-1.58). Boys in the thalassemia cohort had a significantly higher adjusted incidence hazard ratio (IRR) of asthma than those in the non-thalassemia cohort (adjusted IRR = 1.45, 95% CI = 1.02-1.73). The risk of atopic and nonatopic asthma was higher in the thalassemia cohort than in the non-thalassemia cohort (IRR = 1.3, 1.61, respectively).
CONCLUSIONS: Children with thalassemia were more likely to develop asthma. More attention should be paid to the early diagnosis of asthma and prevention of asthma attacks
Adiponectin gene (ADIPOQ) polymorphisms correlate with the progression of nephropathy in Taiwanese male patients with type 2 diabetes
Aims: Polymorphisms of the ADIPOQ gene were associated with diabetic nephropathy (DN) in case-control studies predominantly among European populations. Gender may modify the ADIPOQ associated risk for DN. We investigated the association of 18 ADIPOQ polymorphisms with DN in a prospective Taiwanese cohort of type 2 diabetes (T2D) and explored whether gender plays a role in this genetic association. Methods: Selected single nucleotide polymorphisms (SNPs) of ADIPOQ were genotyped in 566 T2D patients with normoalbuminuria at baseline. DN was defined based on urinary albumin-to-creatinine ratio (ACR). The Cox proportional hazard model was used to explore the association of individual SNP to DN events under different genetic models over a 6-year follow-up period. Analyses were further stratified by gender. Results: In male patients, the adjusted hazard ratios under the recessive models were 1.81 for rs2241766 TT (vs. GT+GG, 95% CI=1.10-2.96, p=0.019) and 1.89 for rs1063537 CC (vs. CT+TT, 95% CI=1.15-3.11, p=0.013). In the Kaplan-Meier survival curve, males carrying rs2241766 TT (vs. GT+GG, p=0.050) and rs1063537 CC (vs. CT+TT, p=0.037) recessive homozygotes also had a reduced nephropathy-free survival rate. SNPs rs2241767 and rs2082940, both in strong correlation with tag SNP rs1063537 (r≥0.96), were also associated with DN progression in males. In females, ADIPOQ polymorphisms were not associated with the progression of DN. Conclusions: ADIPOQ genetic polymorphisms rs2241766 (+45T>G), rs1063537, rs2241767 and rs2082940 were correlated with the progression of DN in Taiwanese male patients with T2D. The role of gender in this ADIPOQ genetic association needs to be further investigated in other populations
Single-incision thoracoscopic surgery for primary spontaneous pneumothorax
<p>Abstract</p> <p>Objective</p> <p>Single-incision laparoscopic surgery had been proven effective for appendectomy, cholecystectomy, and inguinal hernia repair. However, single-incision thoracoscopic surgery (SITS) in primary spontaneous pneumothorax (PSP) has not been reported.</p> <p>Methods</p> <p>We prospectively enrolled 30 PSP patients who received thoracoscopic surgery in the division of Thoracic Surgery of China Medical University Hospital. Ten patients received SITS and 20 patients received traditional three-port thoracoscopic surgery. The operative time, blood loss, wound size, visual analog scale (VAS) pain score, and patient satisfaction score were compared.</p> <p>Results</p> <p>There was no significant difference in the operative time and blood loss between the two groups. However, the VAS pain scores were significantly better in the SITS group in first 24 hours after surgery. Patient satisfaction scores in the SITS group were also significantly better in the first 24 and 48 hours after operation.</p> <p>Conclusion</p> <p>Although three-port thoracoscopic surgery for PSP is well established, SITS results in better patient satisfaction and decreased postoperative pain in the treatment of PSP.</p
