    Bleeding Meckel’s diverticulum in a 4-month-old infant: Treatment with laparoscopic diverticulectomy. A case report and review of the literature

    A bleeding Meckel’s diverticulum is presented in a 4-month-old African American infant. This event is rare at this age, and our patient is only the second 4-month-old infant reported in the English literature. The infant presented with painless frank rectal bleeding, the blood being maroon-colored, and clots were found in the diaper. There was also anemia, with an hemoglobin of less than 8 gm/dl. The color of the blood suggested a bleeding site in the ileo-cecal region, a Meckel’s diverticulum was suspected, which was then confirmed by an isotope scan. A typical Meckel’s diverticulum was found on laparoscopic surgery, was excised, and the infant made an uneventful recovery

    A Base-Excitation Approach to Polynomial Chaos-Based Estimation

    Abstract-This paper presents a novel method for identifying in real-time the sprung mass of a 2-DOF quarter-car suspension model. It does so by uniquely combining the base-excitation concept with polynomial chaos estimation. This unique combination of the two methods provides two important benefits. First, the base-excitation concept makes it possible to estimate the sprung mass without explicitly measuring or knowing the terrain profile prior to estimation. Second, the polynomial chaos estimation strategy makes it possible to perform such mass estimation using sprung and unsprung acceleration measurements without pseudo-integration filters that can be difficult to tune. This paper derives the proposed method in detail and presents computer simulations to evaluate its convergence speed and accuracy. The simulation results consistently converge to within 10% of the true mass value typically within 120 seconds

    Nudibranch Range Shifts associated with the 2014 Warm Anomaly in the Northeast Pacific

    Abstract.—The Northeast Pacific Ocean was anomalously warm in 2014, despite ENSO-neutral conditions in the tropical Pacific. We documented northern range shifts associated with this anomaly for 30 species of nudibranchs and other shallow-water, benthic heterobranch gastropods from southern California to southern Oregon. Nine of these (Placida cremoniana, Trapania velox, Doriopsilla fulva, Janolus anulatus, J. barbarensis, Flabellina cooperi, Anteaeolidiella chromosoma, A. oliviae, and Noumeaella rubrofasciata) were recorded from new northernmost localities, while the remainder were found at or near northern range limits established mainly during past El Niño events. All 30 species have planktotrophic larval development, and six were observed spawning at northern localities, increasing the likelihood that their ranges will continue to shift poleward as the strong 2015-16 El Niño develops. Notable among these was Okenia rosacea, usually found south of San Francisco and last observed in Oregon as a single specimen found during the 1997-98 El Niño. In 2015 this bright pink nudibranch reached high densities and was observed spawning throughout northern California and into southern Oregon. Okenia rosacea is therefore poised to exploit abundant prey resources previously out of its reach in northern Oregon and Washington. Our results not only demonstrate a striking biological response to the 2014 warm anomaly in the North Pacific Ocean, but also support early physical indications of a larger regional climate shift, one reinforced by long-term global warming. Combined with historical data, these results highlight how shallow-water nudibranchs, with their planktotrophic larvae, short life cycles, conspicuous coloration, and accessibility are excellent biological indicators of ocean climate in the region

We report the isolation of Kerstersia gyiorum from two patients: the first, a patient with chronic ear infections, and the second, a patient with a chronic leg wound. Both isolates were resistant to ciprofloxacin, which has not been previously reported

    Mapping the Asymmetric Thick Disk I. A Search for Triaxiality

    A significant asymmetry in the distribution of faint blue stars in the inner Galaxy, Quadrant 1 (l = 20 to 45 degrees) compared to Quadrant 4 was first reported by Larsen & Humphreys (1996). Parker et al (2003, 2004) greatly expanded the survey to determine its spatial extent and shape and the kinematics of the affected stars. This excess in the star counts was subsequently confirmed by Juric et al. (2008) using SDSS data. Possible explanations for the asymmetry include a merger remnant, a triaxial Thick Disk, and a possible interaction with the bar in the Disk. In this paper we describe our program of wide field photometry to map the asymmetry to fainter magnitudes and therefore larger distances. To search for the signature of triaxiality, we extended our survey to higher Galactic longitudes. We find no evidence for an excess of faint blue stars at l > 55 degrees including the faintest magnitude interval. The asymmetry and star count excess in Quadrant 1 is thus not due to a triaxial Thick Disk.Comment: 36 pages, 8 figures. Accepted by Astronomical Journa

    Comparative effectiveness of dual-action versus single-action antidepressants for the treatment of depression in people living with HIV/AIDS

    Background Depression is the most common psychiatric comorbidity among people living with HIV/AIDS (PLWHA). Little is known about the comparative effectiveness between different types of antidepressants used to treat depression in this population. We compared the effectiveness of dual-action and single-action antidepressants in PLWHA for achieving remission from depression. Methods We used data from the Centers for AIDS Research Network of Integrated Clinic Systems to identify 1175 new user dual-action or single-action antidepressant treatment episodes occurring from 2005 to 2014 for PLWHA diagnosed with depression. The primary outcome was remission from depression defined as a Patient Health Questionnaire-9 (PHQ-9) score <5. Mean difference in PHQ-9 depressive symptom severity was a secondary outcome. The main approach was an intent-to-treat (ITT) evaluation complemented with a per protocol (PP) sensitivity analysis. Generalized linear models were fitted to estimate treatment effects. Results In ITT analysis, 32% of the episodes ended in remission for both dual-action and single-action antidepressants. The odds ratio (OR) of remission was 1.02 (95%CI=0.63,1.67). In PP analysis, 40% of dual-action episodes ended in remission compared to 32% in single-action episodes. Dual-action episodes had 1.33 times the odds of remission (95%CI=0.55,3.21), however the result was not statistically significant. Non-significant differences were also observed for depressive symptom severity. Limitations Missing data was common but was addressed with inverse probability weights. Conclusions Results suggest that single-action and dual-action antidepressants are equally effective in PLWHA. Remission was uncommon highlighting the need to identify health service delivery strategies that aid HIV providers in achieving full remission of their patients’ depression

    Despite the clear fitness consequences of animal decisions, the science of animal decision making in evolutionary biology is underdeveloped compared with decision science in human psychology. Specifically, the field lacks a conceptual framework that defines and describes the relevant components of a decision, leading to imprecise language and concepts. The ‘judgment and decision-making’ (JDM) framework in human psychology is a powerful tool for framing and understanding human decisions, and we apply it here to components of animal decisions, which we refer to as ‘cognitive phenotypes’. We distinguish multiple cognitive phenotypes in the context of a JDM framework and highlight empirical approaches to characterize them as evolvable traits
