    Identifying experiences of supportive care of children and young people affected by kidney failure: a qualitative systematic review.

    Children and young people affected by kidney failure experience complexities in their care. Little is known about the unique needs of this young patient population group living with a long-term condition. A meta-aggregation of all qualitative studies was conducted to identify experiences of supportive care among children and young people living with kidney failure. A systematic review of qualitative studies was conducted following the Joanna Briggs Institute meta-aggregation method. This review has been reported according to the PRISMA statement guidelines. Six electronic databases (CINAHL, Cochrane Library, MEDLINE, Proquest, PsycINFO, and Scopus) were comprehensively searched by an expert systematic review librarian using keywords and subject headings, from inception to September 2022. All studies were accessed using a predetermined inclusion and exclusion criteria. Methodological quality assessment and data extraction performed. Qualitative findings accompanied by illustrative quotes from included studies were extracted and grouped into categories which created the overall synthesised findings. A total of 34 studies were included in this review representing a total of 613 children and young people affected by kidney failure. There was a total of 190 findings which created 13 categories representing experiences of supportive care. The meta-aggregation developed five synthesised findings namely: 'physical needs', 'information and technology', 'treatment and healthcare', 'social needs' and 'psychological impacts'. This systematic review identified that children and young people affected by kidney failure can experience a range of unmet supportive care needs in routine clinical services. Kidney failure impacted children and young people's self-identify, social and peer networks, introduced daily practical needs because of inherent physical and psychological burden due to the failure and associated treatments. Despite improvements in the medical management of kidney failure in children and young people, further attention is needed to optimise supported self-management in this young patient group

    Diffusion Limited Aggregation with Power-Law Pinning

    Using stochastic conformal mapping techniques we study the patterns emerging from Laplacian growth with a power-law decaying threshold for growth RNγR_N^{-\gamma} (where RNR_N is the radius of the NN- particle cluster). For γ>1\gamma > 1 the growth pattern is in the same universality class as diffusion limited aggregation (DLA) growth, while for γ<1\gamma < 1 the resulting patterns have a lower fractal dimension D(γ)D(\gamma) than a DLA cluster due to the enhancement of growth at the hot tips of the developing pattern. Our results indicate that a pinning transition occurs at γ=1/2\gamma = 1/2, significantly smaller than might be expected from the lower bound αmin0.67\alpha_{min} \simeq 0.67 of multifractal spectrum of DLA. This limiting case shows that the most singular tips in the pruned cluster now correspond to those expected for a purely one-dimensional line. Using multifractal analysis, analytic expressions are established for D(γ)D(\gamma) both close to the breakdown of DLA universality class, i.e., γ1\gamma \lesssim 1, and close to the pinning transition, i.e., γ1/2\gamma \gtrsim 1/2.Comment: 5 pages, e figures, submitted to Phys.     Interspecies transfer of blaimp-4 in a patient with prolonged colonization by IMP-4-producing enterobacteriaceae

    A patient was colonized by IMP-4-producing Enterobacter cloacae and Escherichia coli strains for 7 months. IMP-4-producing E. cloacae strains were first and last isolated at day 33 and at 8 months after admission, respectively. IMP-4-producing E. coli strains were first and last isolated at days 88 and 181 after admission, respectively. The E. cloacae and E. coli isolates shared identical genetic features in terms of bla(IMP-4), bla(TEM-1), qnrB2, aacA4, HI2 plasmids, and ISCR1. This study shows the first prolonged colonization with in vivo interspecies transfer of bla(IMP-4)

    Mode-coupling approach to non-Newtonian Hele-Shaw flow

    The Saffman-Taylor viscous fingering problem is investigated for the displacement of a non-Newtonian fluid by a Newtonian one in a radial Hele-Shaw cell. We execute a mode-coupling approach to the problem and examine the morphology of the fluid-fluid interface in the weak shear limit. A differential equation describing the early nonlinear evolution of the interface modes is derived in detail. Owing to vorticity arising from our modified Darcy's law, we introduce a vector potential for the velocity in contrast to the conventional scalar potential. Our analytical results address how mode-coupling dynamics relates to tip-splitting and side branching in both shear thinning and shear thickening cases.     Few studies have quantitatively compared benthic macrofaunal assemblages between different tropical intertidal habitats over differing spatial scales. The present study uses spatially nested material from vegetated (seagrass) and non-vegetated (mudflat) habitats in SW Thailand to address this issue. Polychaetes were the numerically dominant component of benthic assemblages throughout the region, comprising over 74% of the total macrofauna. Despite great within-site and sample variation, based on species diversity, polychaete assemblages were mostly site-specific. Not so visibly obvious were the evident differences in polychaete assemblages between habitats.     Molecular epidemiology of NDM-1-producing Enterobacteriaceae and Acinetobacter baumannii isolates from Pakistan

    The molecular epidemiology of 66 NDM-producing isolates from 2 Pakistani hospitals was investigated, with their genetic relatedness determined using repetitive sequence-based PCR (Rep-PCR). PCR-based replicon typing and screening for antibiotic resistance genes encoding carbapenemases, other β-lactamases, and 16S methylases were also performed. Rep-PCR suggested a clonal spread of Enterobacter cloacae and Escherichia coli. A number of plasmid replicon types were identified, with the incompatibility A/C group (IncA/C) being the most common (78%). 16S methylase-encoding genes were coharbored in 81% of NDM-producing Enterobacteriaceae.
