Szenzoros neuropeptid felszabadulás farmakológiai gátlása, mint új gyógyszerhatás mechanizmus az asthma és a krónikus obstruktív légúti betegség terápiájában = Pharmacological inhibition of sensory neuropeptide release as a novel mechanism of action in the treatment of bronchial asthma and chronic obstructive airway disease
In vitro trachea perfúziós rendszerünkben 94 új szomatosztatin (SOM) agonistát (Biostatin Kft. Budapest) vizsgáltunk a kapszaicin-érzékeny afferensekből történő P-anyag (SP) felszabadulására, melyek közül 24 szignifikáns gátlást okozott. A legjobb 6 vegyületet in vivo is teszteltük, gyulladásmodellekben hatásosnak bizonyultak. A PACAP-38 és a szelektív sst4 receptor agonista peptidomimetikum J-2156 (Juvantia Pharma, Turku) e modellben koncentráció-függő módon gátolta az SP, CGRP és SOM felszabadulását. Specifikus RIA-t fejlesztettünk ki PACAP-38 mérésre, mellyel igazoltuk e peptid szenzoros rostokból történő felszabadulását. Endotoxinnal kiváltott szubakut légúti gyulladás egérmodellben funkcionális, morfológiai és biokémiai módszerekkel igazoltuk, hogy TRPV1 kapszaicin receptor génhiányos egerekben fokozott gyulladás és a következményes bronchialis hiper-reaktivitás alakult ki. SOM RIA és sst receptor antagonista kezeléssel biokémiai és funkcionális bizonyítékokat szolgáltattunk, hogy a gyulladás során TRPV1 receptor aktivációval SOM szabadul fel, amely gátolja a gyulladást és a bronchokonstrikciós hajlamot. A fenti eredményeink alapján szintetikus sst4 receptor agonisták, a heptapeptid TT-232 és peptidomimetikum J-2156, vizsgálata történt az előbbiekben ismertetett módszerekkel az LPS-indukálta pneumonitis és az ovalbuminnal kiváltott asztma modellekben egérben. E vegyületek mindkét modellben hatékonyan gátolták a légúti gyulladást és hiper-reaktivitást. | The effect of 94 novel somatostatin (SOM) agonists (Biostatin Ltd., Budapest) on substance P (SP) release from capsaicin-sensitive afferents were examined in our in vitro isolated trachea perfusion system. Among these 24 caused significant inhibition, the most effective 6 ligands diminished inflammation in vivo. PACAP-38 and the selective sst4 receptor agonist peptidomimetic J-2156 (Juvantia Pharma, Turku) concentration-dependently inhibited SP, CGRP and SOM release in this model. We have developed a specific RIA for measuring PACAP-38 and with its help the outflow of this peptide from capsaicin-sensitive afferents has been evidenced. Experiments using TRPV1 receptor gene-deficient mice provided functional, morphological and biochemical evidence for a novel type of counter-regulatory mechanism during endotoxin-induced airway inflammation. This is mediated by SOM released from sensory nerve terminals into the circulation in response to TRPV1 receptor activation. It inhibits inflammation and bronchial hyper-reactivity. Desensitization of capsaicin-sensitive afferents enhances inflammation which might be due to the lack of SOM released from these fibres. Based on the above described results, synthetic sst4 receptor agonists, the heptapeptide TT-232 and the peptidomimetic J-2156 have been studied in endotoxin-induced subacute pneumonitis and ovalbumin-evoked asthma models of the mouse and effectively inhibited airway inflammation and consequent hyper-reactivity
A HISS mechanizmus farmakológiai modulációjának vizsgálata = Investigation on pharmacological exploitation of HISS mechanism
1. Kimutattuk, hogy a plexus hepaticus anterior capsaicin-érzékeny rostjai szerepet játszanak az inzulin érzékenység szabályozásában. 2. A nitroglicerinnel létrehozott haemodinamikai nitrát tolerancia kifejlődésével párhuzamosan inzulin rezisztencia is kialakul. 3. Nitrát készítményt rövid ideig alkalmazva (6 óra) inzulin érzékenység növekedés jön létre. 4. A glibenclamide hosszan tartó adagolás mellett sem hoz létre inzulin rezisztenciát, sőt javítja az inzulin érzékenységet Goto-Kakizaki patkányokon. 5. A hiperinzulinémia önmagában is proarrythmiás hatással rendelkezik egészséges nyulakon. 6. A kilenc napon keresztül napi 3 mg/kg i.p. dózisban adott cisplatin szenzoros neuropátiát hozott létre és csökkentette az inzulin érzékenységet. 7. Normál állatokon a CGRP8-37 szignifikáns inzulin érzékenység növekedést eredményezett, míg a cisplatinnal kezelt állatokon a változás nem volt szignifikáns. 8. Cisplatinnal kezelt állatokon sem a plexus hepaticus anterior regionális capsaicin kezelése, sem a szisztémás capsaicin kezelés nem változtatta meg az állatok inzulin érzékenységét. 9. A szív izomzata 4-szer nagyobb mértékben veszi fel a glukózt, mint a harántcsíkolt gastrocnemius izomzat. Cisplatin kezelés mindkét területen szignifikáns glükóz felvétel csökkenést eredményezett. 10. L-NAME 10 mg/kg dózisban intraportálisan adva szignifikánsan csökkent a glükóz felvétel mértéke cisplatin kezelést követően mind a szívizom, mind a vázizom esetében. | 1. The capsaicin-sensitive fibers of anterior hepatic plexus are playing role in the regulation of insulin sensitivity. 2. Insulin resistance developed parallel with the hemodynamic nitrate tolerance induced by nitroglycerin administration. 3. Nitrates applied for short term (6 hours) produced an improvement in insulin sensitivity. 4. Even the prolonged administration of glibenclamide was not able to develop insulin resistance, but induced improvement in insulin sensitivity in Goto-Kakizaki rats. 5. Hyperinsulinaemia in its own was able to produce proarrhythmic effects in healthy rabbits. 6. Cisplatin administered over 9 days in the dose of 3 mg/kg i.p. evoked development of sensory neuropathy and a decrease in insulin sensitivity. 7. CGRP8-37 induced significant increase in insulin sensitivity in normal animals, while no significant change was observed in cisplatin-treated animals. 8. Neither the regional capsaicin treatment of anterior hepatic plexus, nor the systemic capsaicin treatment resulted in any change in the insulin sensitivity in cisplatin-treated animals. 9. Glucose uptake was more than four times higher in cardiac muscle than in gastrocnemius muscle. Cisplatin treatment produced a significant decrease in glucose uptake in both sites. 10. The effect of intraportally administered L-NAME in the dose of 10 mg/kg on the glucose uptake was significantly reduced after cisplatin treatment in cardiac and in skeletal muscle as well

Endogén kardioprotektív mechanizmusok farmakológiai kihasználása = Novel pharmacotherapeutic strategies of ishaemic heart disease associated with hypercholesterolaemia and insulin resistance. Pharmacological exploitation of endogenous cardioproctective mechanisms
A kutatás során fény derült arra, hogy az insulin rezisztenciához társult iszkémiás szívbetegség farmakoterápiája során egy újonnan felismert endogén insulin érzékenyítő mechanizmus, a HISS (hepatic insulin sensitizing substance) mechanizmus igen fontos lehet. Felderítettük, hogy ez a mechanizmus NO donorokkal és ciklikus GMP foszfodiészteráz gátlókkal a terápia számára kiaknázható. Bizonyítottuk azt is, hogy az említett gyógyszercsoportok különböző tagjai alacsonyabb dózisban képesek insulin érzékenyítő hatást elérni, mint ''klasszikus'' indikációikban (hypertonia, congestív szívelégtelenség, iszkémiás szívbetegség. Az eredmények alapján gyógyszerfejlesztési folyamatok indultak meg. | The resuts achieved revealed the potential of a novel endogenous insulin sensitizing mechanism, the HISS process: hepatic insulin sensitizing substance mechanism) in treatment and prevention of cardiovascular disease underpinned by the insulin resistance syndrome. Our findings provided evidence for the use of cyclic GMP phosphodiesterase inhibitors and nitric oxide donors as tools for pharmacological exploitation of this mechanism. Moreover, it has also benn shown that drugs in these groups acieve insulin sensitization at doses much lower than those necessary to produce effects in theis ''classical'' indications such as sustained arterial hypertension, congestive heart failure and ischaemic heart disease. In summary, our data provide evidence
that the development of reperfusioninduced
VF inhibits HO-1 mRNA
expression and enzyme activity in both
electrically fibrillated myocardium and
ischemic and reperfused fibrillated hearts.
The results clearly show that HO-1 mRNA
expression and enzyme activity were
increased in ischemic and reperfused
nonfibrillated myocardium suggesting that
interventions which are able to increase
HO-1 mRNA expression and enzyme activity
may prevent the development of VF.
Furthermore in the studies presented in
this part of my thesis, superoxide
dismutase (SOD), catalase and glutathione
peroxidase (GPx) were characterized in
isolated perfused hearts of 18-month-old
SHR and the age-matched normotensive
control Wistar-Kyoto rats (WKY), before
and after 30 min infusion of 25 μM H2O2.
The results obtained in ischemic and
reperfused hearts show the same changes
in enzyme activities measured as it was
observed in H2O2 perfused hearts,
indicating that oxidative stress is independent
of the way it was induced. The
higher catalase activity derived from
elevated mRNA synthesis. The antioxidative
system in dilative cardiomyopathic
hearts of SHR is induced, probably due to
episodes of oxidative stress, during the
process of decompensation. This
conditioning of the antioxidative poten-tial
may help overcome acute stress
situations caused by reactive oxygen
species in the failing myocardium.
In the last part of my studies the
effects of red grape seed proanthocyanidins
were studied on the recovery of
postischemic function in isolated rat
hearts. In rats treated with 50 and 100
mg/kg of grape seed proanthocyanidins,
the incidence of reperfusion-induced VF
was reduced from its control value of
92% to 42% (p<0.05) and 25% (p<0.05),
respectively. The incidence of ventricular
tachycardia (VT) showed the same
pattern. In rats treated with 100 mg/kg
proanthocyanidins, after 60 min reperfusion,
the recovery of coronary flow,
aortic flow, and developed pressure was
improved by 32% ± 8% (p<0.05), 98%
± 8% (p<0.05), 37% ± 3% (p<0.05),
respectively, compared to the drug-free
controls. ESR studies indicate that
proanthocyanidins significantly inhibited
the formation of oxygen free radicals. In
rats treated with 100 mg/kg of proanthocyanidins,
free radical intensity was
reduced by 75% ± 7% (p<0.05) compared
to the drug-free value. Grape seed
proanthocyanidins possess cardioprotective
effect against reperfusioninduced
injury through their abilities to
reduce or remove, directly or indirectly,
free radicals in the ischemic/reperfused
