    We derive the interior Kottler solution of the incompressible fluid and show that the bending of light in this solution does depend on the cosmological constant.Comment: The inner Kottler solution derived and used in this paper is not new.     Regularization, Renormalization and Range: The Nucleon-Nucleon Interaction from Effective Field Theory

    Regularization and renormalization is discussed in the context of low-energy effective field theory treatments of two or more heavy particles (such as nucleons). It is desirable to regulate the contact interactions from the outset by treating them as having a finite range. The low energy physical observables should be insensitive to this range provided that the range is of a similar or greater scale than that of the interaction. Alternative schemes, such as dimensional regularization, lead to paradoxical conclusions such as the impossibility of repulsive interactions for truly low energy effective theories where all of the exchange particles are integrated out. This difficulty arises because a nonrelativistic field theory with repulsive contact interactions is trivial in the sense that the SS matrix is unity and the renormalized coupling constant zero. Possible consequences of low energy attraction are also discussed. It is argued that in the case of large or small scattering lengths, the region of validity of effective field theory expansion is much larger if the contact interactions are given a finite range from the beginning.Comment: 7 page

    Definitive spectroscopic determination of the transverse interactions responsible for the magnetic quantum tunneling in Mn12-acetate

    We present detailed angle-dependent single crystal electron paramagnetic resonance (EPR) data for field rotations in the hard plane of the S=10 single molecule magnet Mn12-acetate. A clear four-fold variation in the resonance positions may be attributed to an intrinsic fourth order transverse anisotropy (O44). Meanwhile, a four-fold variation of the EPR lineshapes confirms a recently proposed model wherein disorder associated with the acetic acid of crystallization induces a locally varying quadratic (rhombic) transverse anisotropy (O22). These findings explain most aspects of the magnetic quantum tunneling observed in Mn12-acetate.Comment: 7 pages, including figures, accepted for publication in Phys. Rev. Let

    Isoflurane Protects Against Human Endothelial Cell Apoptosis by Inducing Sphingosine Kinase-1 via ERK MAPK

    Endothelial dysfunction is a major clinical problem affecting virtually every patient requiring critical care. Volatile anesthetics are frequently used during the perioperative period and protect the heart and kidney against ischemia and reperfusion injury. We aimed to determine whether isoflurane, the most commonly used volatile anesthetic in the USA, protects against endothelial apoptosis and necrosis and the mechanisms involved in this protection. Human endothelial EA.hy926 cells were pretreated with isoflurane or carrier gas (95% room air + 5% CO2) then subjected to apoptosis with tumor necrosis factor-α or to necrosis with hydrogen peroxide. DNA laddering and in situ Terminal Deoxynucleotidyl Transferase Biotin-dUTP Nick-End Labeling (TUNEL) staining determined EA.hy926 cell apoptosis and percent LDH released determined necrosis. We also determined whether isoflurane modulates the expression and activity of sphingosine kinase-1 (SK1) and induces the phosphorylation of extracellular signal regulated kinase (ERK MAPK) as both enzymes are known to protect against cell death. Isoflurane pretreatment significantly decreased apoptosis in EA.hy926 cells as evidenced by reduced TUNEL staining and DNA laddering without affecting necrosis. Mechanistically, isoflurane induces the phosphorylation of ERK MAPK and increased SK1 expression and activity in EA.hy926 cells. Finally, selective blockade of SK1 (with SKI-II) or S1P1 receptor (with W146) abolished the anti-apoptotic effects of isoflurane. Taken together, we demonstrate that isoflurane, in addition to its potent analgesic and anesthetic properties, protects against endothelial apoptosis most likely via SK1 and ERK MAPK activation.     Looking Beyond a Rare Presentation of Betel Nut Use in a North American Patient- an Opportunity for Lessons and Intervention in Global Health and Gender Equity

    Betel nut is estimated to be the fourth most common psychoactive substance used globally behind caffeine, alcohol, and nicotine with 600 million global users. Betel nut use has a strong association with oral cancer and many other systemic effects including oral submucosal fibrosis. Although North American use is rare, its enduring effects may surface long after use posing a diagnostic challenge to practitioners caring for an increasingly global diaspora. Herein, we present a patient with severe trismus due to advanced oral submucosal fibrosis who was unexpectedly found to be importing and using Betel nut for over thirty years after immigrating. The prevalence of use in North America is unknown due to a paucity of current epidemiologic data. Betel nut side effects include a strong association with oral squamous cell carcinoma, hepatocellular carcinoma, cirrhosis, and cardiovascular disease, among others. However, there remains widespread lack of attributable risk awareness and entrenched social norms that permit betel nut use in early childhood in endemic areas. Importantly, women often bear a higher proportion of betel nut-related disease not only due to limited means of healthcare access but also due to cultural norms that often discourage women from smoking tobacco and instead encourage betel nut use. Here, standard tobacco and cancer screening had the potential to overlook a silent but significant driver of poor health. This case demonstrates a need for awareness of betel nut availability and its effects in the North American context. Moreover, it underscores the judicious care warranted for migrant patients who may have a history of exposure to betel nut and other imported cultural habits of clinical importance; these considerations are relevant to the individualized provision of culturally informed, gender equitable care
