    The use of new psychoactive substances (NPS) in young people and their role in mental health care: a systematic review

Accepted for publication in Expert Review of Neurotherapeutics, 09/09/2019.Introduction: Over the past 10 years, a large number of New Psychoactive Substances (NPS) have entered the recreational drug scenario. NPS intake has been associated with health-related risks, and especially so for vulnerable populations such as the youngsters. Currently, most knowledge on the NPS health effects is learnt from both a range of users’ reports, made available through the psychonauts’ web fora, and from the few published, related toxicity, clinical observations. Areas covered: This paper aims at providing an overview of NPS effects on youngsters’ mental health, whilst performing a systematic review of the current related knowledge. Expert opinion: NPS consumption poses serious health risks, due to both a range of unpredictable clinical pharmacological properties and the typical concomitant use of other psychoactive molecules; overall, this can lead to near misses and fatalities. In comparison with adults, the central nervous system of children/adolescents may be more vulnerable to the activity of these molecules, hence raising even further the levels of health-related concerns. More research is needed to provide evidence of both short- and long-term effects of NPS, related health risks, and their addiction potential.Peer reviewedFinal Accepted Versio

    A Scoping Review of Home Produced Heroin and Amphetamine Type Stimulant Substitutes: Implications for Prevention, Treatment and Policy

    Several home-produced substances such as krokodil and boltushka are prevalent in many Eastern European countries. Anecdotal reports of its use have been circulating in Germany and Norway; however, this has not been confirmed. Its use has also been reported by the media in the USA, although only one confirmed report of its use exists. Home-produced drugs are associated with high levels of morbidity and a number of complex health issues such as the spread of blood borne viruses, gangrene, and internal organ damage. The high incidence of HIV rates amongst people who inject home-produced substances is a public health concern. The resulting physical health consequences of injecting these crude substances are very severe in comparison to heroin or amphetamine acquired in black markets. Due to this fact and the increased mortality associated with these substances, professionals in the area of prevention, treatment, and policy development need to be cognisant of the presentation, harms, and the dangers associated with home-produced substances globally. This scoping review aimed to examine existing literature on the subject of home-produced heroin and amphetamine-type stimulant substitutes. The review discussed the many implications such research may have in the areas of policy and practice. Data were gathered through the use of qualitative secondary resources such as journal articles, reports, reviews, case studies, and media reports. The home production of these substances relies on the utilisation of precursor drugs such as less potent stimulants, tranquillizers, analgesics, and sedatives or natural plant ingredients. The Internet underpins the facilitation of this practice as recipes, and diverted pharmaceutical sales are available widely online, and currently, ease of access to the Internet is evident worldwide. This review highlights the necessity of prevention, education, and also harm reduction related to home-produced drugs and also recommends consistent monitoring of online drug fora, online drug marketplaces, and unregulated pharmacies

    The Unregulated Psychoactive Compound: ‘Benzo Fury’

    Introduction: The ‘legal high’ ‘Benzo Fury’ [1-(benzofuran-5-yl)propan-2-amine (5-APB) and 1-(benzofuran-6-yl)propan-2-amine (6-APB)] is an unregulated psychoactive compound which has similar effects as known controlled drugs like amphetamines and ecstasy (with hallucinogenic and stimulant properties). It is known to pose significant health risks, such as causing anxiety, sinus tachycardia, hypertension, and QT prolongation. Furthermore, it is easily obtained from the internet. Methods: A systematic review based on PubMed and Medline literature was conducted. Deaths involving ‘Benzo Fury’ (implicated in death and/or identified at post mortem toxicology) in the UK from 1997-2012 were identified from the national programme on Substance Abuse Deaths (np-SAD) database. The programme periodically receives information regarding drugrelated deaths in the UK submitted voluntarily by Coroners, the Northern Ireland Statistics and Research Agency, and the Scottish Crime and Drug Enforcement Agency. The general characteristics of these fatalities were analysed. Results: Ten cases were found with ‘Benzo Fury’ identified at post mortem and it was directly implicated in eight of these deaths. In nine cases, ‘Benzo Fury’ was found in combination with other psychoactive substances. All deaths occurred between 2011-12; the median age at death was 24 years; the majority were male (8/10); and where ethnicity was known, all were white. Conclusions: Our findings indicate a need for a Public Health education campaign on the risks of ‘Benzo Fury’ consumption, especially targeting young users and drugs education workers. Further research into the toxicity of ‘Benzo Fury’ and the risks associated with its consumption is also needed.Peer reviewe

    How does ayahuasca work from a psychiatric perspective? Pros and cons of the entheogenic therapy

    Objective: Ayahuasca is a hallucinogenic plant preparation, traditionally consumed in sacred ceremonies by indigenous North-Westerner Amazonian countries like Colombia, Peru, Brazil, and Ecuador. It is fundamental to carefully balance benefits/risks related to the ayahuasca intake, both during ceremonies and experimental settings. The aim is at evaluating and comparing the potential therapeutic benefits versus health risks related to ayahuasca intake (both acutely and chronically), focusing on its application in psychedelic psychiatry. Design: A comprehensive mini overview focusing on psychiatric outcomes following ayahuasca intake both in healthy volunteers and in clinical samples. Results: Preclinical, observational, and experimental studies in healthy volunteers as well as in clinical samples suggest that ayahuasca may be beneficial as an antidepressant, emotional regulator, anxiolytic, and antiaddictive drug, by exerting fast-acting and enduring clinical effects. Ayahuasca appears to be safe and well tolerated, nausea and emesis being the most reported and transient side effects. Some findings suggest not to use ayahuasca in bipolar or psychotic patients because of an increased risk of manic switch and/or psychotic onset. Conclusions: Further research should be carried out in randomized, double-blind, placebo-controlled trials, by implementing neuroimaging studies, in order to better evaluate therapeutic potential of ayahuasca in mental disorders

    The use of antipsychotic drug, regardless of dose and exposure time, is associated with potential structural teratogenic effects on the fetus together with perineonatal and postnatal complications and neurobehaviorual sequelae.There are few specifc datas on the antipsychotics' pharmacocinetics and pharmacodynamics in pregnancy therefore the giudelines for therapy are based on observational stadies and basic priciples. All antipsychoitics can pass through placenta and in milk during the pregnancy and lactation. Their administration in pregnancy can result in maternal metabolic complications (gestational diabetes, gestational hypertension and obesity, associated with neural tube defects) and the fetal macrosomia in the new born.Neonathal extrapyramidal adverse effects are less frequent comared to FGA

    Background: Quetiapine, an atypical antipsychotic endowed with weak dopamine antagonist, potent 5-HT2A-blocking, partial 5-HT1A-agonist, anti-H1 histamine, adrenolytic, and sigma1 receptor agonist activities, since an original 2004 report is increasingly misused. Although some of its pharmacodynamics might explain some motives for voluptuary use, most of its actions are directed at setting-off those motives. Hence, it is possible that its popularity in special populations is due to the fact that the unpleasant or unwanted effects of addiction substances are somehow soothed by quetiapine. Currently, quetiapine is tested in substance use disorders, showing some promise, but it is likely to be misused in certain contexts. Objectives: To review the evidence for the use of quetiapine as addiction substance and investigate the characteristics of populations involved in such addiction. Methods: A systematic review of literature on various databases retrieved on September 7, 2018 87 records to comment. Results. We reviewed the evidence for quetiapine’s addictive potential in the light of its pharmacodynamics properties and presented two cases of recreational quetiapine use, by a 35-year old male patient with past addictive behavior and by a 50-year-old woman with major depressive disorder and conversion disorder. We found quetiapine to be abused mainly by addict populations and people with law involvement. Conclusions/Importance: There is no reason to include quetiapine among regulated substances, but monitoring of its use in selected populations is warranted. Psychiatrists and physicians working in the penitentiary system should be aware of the addictive potential of quetiapine and adopt measures restricting its use

    Substance-related psychopathology and aggressiveness in a nightlife holiday resort: Results from a pilot study in a psychiatric inpatient unit in Ibiza

    Objectives: We aimed to describe a sample of subjects admitted to a psychiatric unit after the intake of psychoactive substances for recreational purposes. Methods: Between June and September 2015, 49 subjects were included. Sociodemographic characteristics and psychopathological aspects were investigated, and urine samples for further analysis were collected. Three subgroups (cannabinoids, stimulants, and depressors users) were identified, according to the structured interview regarding substance use and urinalysis. Results: Level of aggressiveness was found to be significantly higher (p <.05) in the cannabinoids subgroup. Self-reported symptom severity was comparable among groups, but trends could be identified: SCL-90 results showed a prevalence of anxiety symptoms among depressors users, hostility or aggression in the tetrahydrocannabinol subgroup, and psychoticism in the stimulants subgroup. Conclusions: The use of psychoactive substances was be characterised by poly-use of both traditional and novel substances. The presence of aggressiveness emerged as a main feature associated with the use of cannabis and other cannabinoids. Binge drinking and sleep deprivation also represented a relevant component in almost all the evaluated subjects
