    APEnet+: high bandwidth 3D torus direct network for petaflops scale commodity clusters

    We describe herein the APElink+ board, a PCIe interconnect adapter featuring the latest advances in wire speed and interface technology plus hardware support for a RDMA programming model and experimental acceleration of GPU networking; this design allows us to build a low latency, high bandwidth PC cluster, the APEnet+ network, the new generation of our cost-effective, tens-of-thousands-scalable cluster network architecture.     Path dependence and universal health coverage: The case of Egypt

    Universal health coverage (UHC) is the big objective in health policy which several countries are seeking to achieve. Egypt is no different and its endeavors to attain UHC have been going on since the 1960s. This article discusses the status of UHC in Egypt using theories of political science and economics by analyzing the historical transformations in the Egyptian health system and its institutional settings. This article then specifically examines the path dependence theory against the sociopolitical background of Egypt and assesses any pattern between the theory and the current UHC status in Egypt. The important finding of this analysis is that the health policies and reforms in Egypt have been significantly influenced and limited by its historical institutional structure and development. Both the health policies and the institutional settings adopted a dependent path that limited Egypt’s endeavors to achieve the universal coverage. This dependent path also yielded many of the present-day challenges as in the weaknesses of the healthcare financing system and the inability to extend health coverage to the poor and the informal sector. These challenges subsequently had a negative impact on the accessibility of the healthcare services

    Causality in collective filtering

    In this paper, we describe a proposal for improving the practice of web-based collective filtering, in particular for what regards discussions and selection of issues about policy, based on the intuitive concept of causality. Causality, especially when presented in visual form, is especially suited to the task since it is intuitive to understand and to use, and at the same time, it's rich enough to create a semantic network between the representations of real world facts. We give some examples of the suggested system workflow and we present guidelines for its implementation

    A Scalable Telemetry Framework for Zero Touch Optical Network Management

    The interest about Zero Touch Network and Service Management (ZSM) is rapidly emerging. As defined by ETSI, the ZSM architecture is based on a closed-loop/feedback control of the network and the services. Such closed-loop control can be based on the Boyd's Observe Orient Decide and Act (OODA) loop that matches some specific management functions such as Data Collection, Data Analytics, Intelligence, Orchestration and Control. An efficient implementation of such control loop allows the network to timely adapt to changes and maintain the required quality of service.Many solutions for collecting network parameters (i.e., implementing ZSM data collection) are proposed that fall under the broad umbrella of network telemetry. An example is the Google gRPC, that represented one of the first solutions to provide a framework for data collection. Since then, the number of available frameworks is proliferating. In this paper we propose the utilisation of Apache Kafka as a framework for collecting optical network parameters. Then, the paper goes beyond that by proposing and showing how Apache Kafka can be effective for supporting data exchange and management of whole ZSM closed-loop.Experimental evaluation results show that, even when a large number of data are collected, the solution is scalable and the time to disseminate the parameter values is short. Indeed, the difference between the reception time and the generation time of data is, on average, 40-50ms when about four thousand messages are generated

    Immunotherapy with low-dose recombinant interleukin 2 after high-dose chemotherapy and autologous stem cell transplantation in neuroblastoma.

    The purpose of this study was to evaluate in a phase I-II trial whether low doses of recombinant human interleukin 2 (rHuIL-2) over a prolonged period of time are safe and effective in eradicating or controlling minimal residual disease in children with neuroblastoma given high-dose chemotherapy (HDCT) and autologous stem cell transplantation (ASCT). From January 1992 to July 1996, 17 consecutive patients, with either stage IV or relapsed neuroblastoma, were enrolled. Patients received rHuIL-2 after a median time interval (min-max) of 105 days (56-153) after HDCT and ASCT. The protocol consisted of 2 'priming' courses of rHuIL-2 at escalating doses administered intravenously at 72-h intervals, followed by 'maintenance' with 11 monthly and six bimonthly boosting 5-day courses administered subcutaneously on an outpatient basis. At April 1997, 7 out of the 17 patients had completed the treatment schedule, four had discontinued treatment because of toxicity and four because of relapse; the remaining two patients are still on treatment, having completed 15 courses. Expansion of T lymphocytes, together with an increase in both natural killer cells and in activated T lymphocytes was evidenced. After a median (min-max) follow-up time of 30 (16-64) months, 12 out of 17 patients are alive and well. Two patients relapsed and died 14 and 35 months after transplant. Three patients are alive after having relapsed at 41, 21 and 13 months. The actuarial 2-year event-free survival and overall survival are 67% and 92% respectively. Intermittent administration of low doses of rHuIL-2 given for a long period of time is well tolerated and seems capable of controlling minimal residual disease after HDCT and ASCT in children with high-risk neuroblastoma
