    Strong Networks Grow Distance Learning

    This article presents a snapshot of one state’s experience with connectivity from the early 1980s to the present and illustrates how distance learning has utilized that infrastructure to grow to serve more than 100,000 Ohioans. In early 1980s, most of Ohio’s telecommunications traffic traveled on dial-up connections. Ohio’s history of formidable statewide networking began in 1987, when Compuserve and OARnet (Ohio Academic Resources Network) were among few regional networks in existence. Through various mergers and acquisitions, Compuserve became Worldcom, AOL, MCI-Worldcom, and, finally, Verizon. OARnet became the Third Frontier Network (TFN) in 2004 and now is referred to as OSCnet and Broadband Ohio Network (BON). OARnet was created in 1987 by the Ohio Board of Regents to provide statewide connectivity to resources at the Ohio Supercomputer Center (OSC). In later years, the network extended support to the 89 member institutions of the Ohio Library and Information Network (OhioLINK), and the 83 colleges and universities of the Ohio Learning Network (OLN), a consortium offering blended, online, and distance education. OLN provides faculty development, infrastructure support via Collaborative Learning Environments (CLE), and various student support services and grants

    Prevalence of specific learning disabilities among Gujarati medium primary school children

    Introduction: Learning disability in children is an assorted group of disorders where the individual unpredictably fails to proficiently attain, regain, and use information. Objective: The objective of this study was to measure the prevalence of specific learning disabilities (SpLDs) such as dyslexia and dysgraphia among the Gujarati medium primary schoolchildren. Methods: A cross-sectional study was conducted in public schools of Gujarati medium among children aged 7–12 years from the second, third, and fourth standard. After obtaining a sociodemographic profile, a multilevel screening approach that begins with the identification of educational backwardness followed by exclusion of vision, hearing impairment, chronic health conditions, and subnormal intelligence was carried out among these children. In the last stage, remaining children were subjected to the National Institute of Mental Health and Neurosciences test for SpLDs. Results: The prevalence of SpLDs was 9.6% in sampled children, whereas 7.4%, 8.6%, and 7.1% had dyslexia, dysgraphia, and dyscalculia, respectively. Among children diagnosed with SpLD, 65.7% (n=25/38) of children had a combination of all three types of SpLDs. Conclusions: This study suggests that the prevalence of SpLDs in public schools is the same as private schools and metro center. We have used a multilevel screening approach that can be utilized for early identification of children with SpLD by Rashtriya Bal Swasthya Karyakram team without imposing load on specialist services. There is a need for sensitization of school teacher for timely referral, remediation strategies, and policy interventions to improve school performance and to reduce dropouts of schoolchildren

    Thermal performance of diamond field-effect transistors

    In this report, the thermal performance of a hydrogen (H)-terminated diamond field-effect transistor (FET) is investigated using Raman spectroscopy and electrothermal device modeling. First, the thermal conductivity (κdiamond) of the active diamond channel was determined by measuring the temperature rise of transmission line measurement structures under various heat flux conditions using nanoparticle-assisted Raman thermometry. Using this approach, κdiamond was estimated to be 1860 W/m K with a 95% confidence interval ranging from 1610 to 2120 W/m K. In conjunction with measured electrical output characteristics, this κ was used as an input parameter for an electrothermal device model of an H-terminated diamond FET. The simulated thermal response showed good agreement with surface temperature measurements acquired using nanoparticle-assisted Raman thermometry. These diamond-based structures were highly efficient at dissipating heat from the active device channel with measured device thermal resistances as low as ∼1 mm K/W. Using the calibrated electrothermal device model, the diamond FET was able to operate at a very high power density of 40 W/mm with a simulated temperature rise of ∼33 K. Finally, the thermal resistance of these lateral diamond FETs was compared to lateral transistor structures based on other ultrawide bandgap materials (Al0.70Ga0.30N, β-Ga2O3) and wide bandgap GaN for benchmarking. These results indicate that the thermal resistance of diamond-based lateral transistors can be up to ∼10× lower than GaN-based devices and ∼50× lower than other UWBG devices

    Massive benign pericardial cyst presenting with simultaneous superior vena cava and middle lobe syndromes

    A 66 year old woman presented in extremis with symptoms and clinical and radiological signs of simultaneous obstruction of superior vena cava and middle lobe of right lung secondary to compression by a massive benign anterior mediastinal cyst. Excision of the cyst at median sternotomy resulted in complete resolution of all symptoms. This report is unusual on account of a) the concomitant presence of superior vena cava and middle lobe syndromes caused by a benign cyst because of its sheer size producing obstruction of these structures and b) the complete resolution of all symptoms and signs after removal of the cyst. Benign anterior mediastinal cysts are unknown to cause either of the two syndromes. To our knowledge, it is the first report of a benign anterior mediastinal cyst causing either superior vena cava syndrome or middle lobe syndrome or both simultaneously.     Cardiac biomarkers of acute coronary syndrome: from history to high-sensitivity cardiac troponin

    The role of cardiac troponins as diagnostic biomarkers of myocardial injury in the context of acute coronary syndrome (ACS) is well established. Since the initial 1st-generation assays, 5th-generation high-sensitivity cardiac troponin (hs-cTn) assays have been developed, and are now widely used. However, its clinical adoption preceded guidelines and even best practice evidence. This review summarizes the history of cardiac biomarkers with particular emphasis on hs-cTn. We aim to provide insights into using hs-cTn as a quantitative marker of cardiomyocyte injury to help in the differential diagnosis of coronary versus non-coronary cardiac diseases. We also review the recent evidence and guidelines of using hs-cTn in suspected ACS

    Changes in the gastric enteric nervous system and muscle: A case report on two patients with diabetic gastroparesis

    <p>Abstract</p> <p>Background</p> <p>The pathophysiological basis of diabetic gastroparesis is poorly understood, in large part due to the almost complete lack of data on neuropathological and molecular changes in the stomachs of patients. Experimental models indicate various lesions affecting the vagus, muscle, enteric neurons, interstitial cells of Cajal (ICC) or other cellular components. The aim of this study was to use modern analytical methods to determine morphological and molecular changes in the gastric wall in patients with diabetic gastroparesis.</p> <p>Methods</p> <p>Full thickness gastric biopsies were obtained laparoscopically from two gastroparetic patients undergoing surgical intervention and from disease-free areas of control subjects undergoing other forms of gastric surgery. Samples were processed for histological and immunohistochemical examination.</p> <p>Results</p> <p>Although both patients had severe refractory symptoms with malnutrition, requiring the placement of a gastric stimulator, one of them had no significant abnormalities as compared with controls. This patient had an abrupt onset of symptoms with a relatively short duration of diabetes that was well controlled. By contrast, the other patient had long standing brittle and poorly controlled diabetes with numerous episodes of diabetic ketoacidosis and frequent hypoglycemic episodes. Histological examination in this patient revealed increased fibrosis in the muscle layers as well as significantly fewer nerve fibers and myenteric neurons as assessed by PGP9.5 staining. Further, significant reduction was seen in staining for neuronal nitric oxide synthase, heme oxygenase-2, tyrosine hydroxylase as well as for c-KIT.</p> <p>Conclusion</p> <p>We conclude that poor metabolic control is associated with significant pathological changes in the gastric wall that affect all major components including muscle, neurons and ICC. Severe symptoms can occur in the absence of these changes, however and may reflect vagal, central or hormonal influences. Gastroparesis is therefore likely to be a heterogeneous disorder. Careful molecular and pathological analysis may allow more precise phenotypic differentiation and shed insight into the underlying mechanisms as well as identify novel therapeutic targets.</p
