    Prevalence and contributing factors of low birth weight babies in institutional delivery

    Introductions: Low Birth Weight (LBW) is a sensitive indicator of socio-economic conditions and indirectly measures the health of mother and child. Though, the health situation of Nepal has improved substantially over the years, LBW rate is still high. The present study explores the prevalence and contributing factors of LBW babies delivered in Patan hospital. Methods: The data were collected retrospectively from patients’ record for babies born at Patan Hospital, Patan Academy of Health Sciences, Kathmandu, Nepal. Records were retrieved through systematic randomization and IBM SPSS 15.0 was used for analysis. Non-parametric tests were applied with correlation done between dependent and independent variables. Results: A total of 4395 birth occurred during the study period of which 527 met the study criteria and remaining were of normal birth weight. The prevalence of LBW was 11.99% and the average LBW was calculated as 2074.97 ± 344.425 grams. The factors associated with LBW included preterm babies (Pearson correlation coefficient= 0.554, p= 0.001 < 0.05) and oligohydamnious (Spearman’s correlation coefficient= 0.307, p= 0.019 < 0.05). Conclusions: This study suggests preterm babies and oligohydramnious to be contributing factors of LBW babies. Keywords: amniotic fluid index, intra uterine growth retardation, maternal risk factors, Patan Hospital, retrospective hospital based studyÂ

    Formulation Optimization of Polyox Based Modified Release Drug Delivery System

    Polyethylene Oxide (PEOs) offer specific advantages to be used in pharmaceutical products as release modifiers by forming a hydrogel around the dosage form in aqueous environment causing the drug to follow a diffusional path before releasing into the media. The strength of the hydrogel governs the release retardation capacity of the PEO system. The objective of this dissertation work was to use the Design of Experiments methodology to develop and optimize a PEO based modified release formulation of a highly water-soluble drug like Metoprolol succinate. The effect of the different viscosity grade PEOs, their concentration with respect to the drug, combination of two different viscosity grades, % drug content in the formulation and the use of water soluble / insoluble fillers on the dissolution of metoprolol succinate was studied. The critical formulation parameters namely PEO concentration and % drug content were chosen as input factors and dissolution at 1, 4, 8 and 20 hours was recorded as responses to carry out optimization using the DOE approach. The results obtained after statistical treatment of data provided a design space that can be used for achieving the desired formulation profile. The model has been validated to predict the effect of the input factors (PEO type and concentration and % drug content) on the responses (in vitro dissolution).     Cobalt Complexes of Bulky PNP Ligand: H2 Activation and Catalytic Two-Electron Reactivity in Hydrogenation of Alkenes and Alkynes

    The reactivity of cobalt pincer complexes supported by the bulky tetramethylated PNP ligands Me4PNPR(R = iPr, tBu) has been investigated. In these ligands, the undesired H atom loss reactivity observed earlier in some classical CH2-arm PNP cobalt complexes is blocked, allowing them to be utilized for promoting two-electron catalytic transformations at the cobalt center. Accordingly, reaction of the formally CoIMe complex 3 with H2 under ambient pressure and temperature afforded the CoIII trihydride 4-H, in a reaction cascade reasoned to proceed by two-electron oxidative addition and reductive eliminations. This mechanistic proposal, alongside the observance of alkene insertion and ethane production upon sequential exposure of 3 to ethylene and H2, prompted an exploration into 3 as a catalyst for hydrogenation. Complex 4-H, formed in situ from 3 under H2, was found to be active in the catalytic hydrogenation of alkenes and alkynes. The proposed two-electron mechanism is reminiscent of the platinum group metals and demonstrates the utility of the bulky redox-innocent Me4PNPR ligand in the avoidance of one-electron reactivity, a concept that may show broad applicability in expanding the scope of earth-abundant first-row transition-metal catalysis.journal articl

    Large artery intracranial occlusive disease: a large worldwide burden but a relatively neglected frontier

    BACKGROUND AND PURPOSE: Large artery intracranial occlusive disease (LAICOD) is a common and important stroke subtype. In this commentary, we review key epidemiological aspects of LAICOD. SUMMARY OF REVIEW: LAICOD has emerged as the most common stroke subtype worldwide and is associated with a high risk of recurrent stroke. Hypotheses have been proposed to explain causation, which include such factors as traditional cardiovascular risk factors, high blood volume states, and genetic abnormalities. Approaches to treatment such as antithrombotic therapies, revascularization procedures, and counterpulsation devices hold promise. CONCLUSIONS: LAICOD poses a major stroke problem worldwide and is likely the most common stroke subtype. The etiology and treatment of this disorder remain poorly defined. International collaborations are needed to pool collective knowledge and develop definitive studies to better understand causation and treatment of LAICOD

    Self‐management for adults with epilepsy: Aggregate Managing Epilepsy Well Network findings on depressive symptoms

    ObjectiveTo assess depressive symptom outcomes in a pooled sample of epilepsy self‐management randomized controlled trials (RCTs) from the Managing Epilepsy Well (MEW) Network integrated research database (MEW DB).MethodsFive prospective RCTs involving 453 adults with epilepsy compared self‐management intervention (n = 232) versus treatment as usual or wait‐list control outcomes (n = 221). Depression was assessed with the nine‐item Patient Health Questionnaire. Other variables included age, gender, race, ethnicity, education, income, marital status, seizure frequency, and quality of life. Follow‐up assessments were collapsed into a visit 2 and a visit 3; these were conducted postbaseline.ResultsMean age was 43.5 years (SD = 12.6), nearly two‐thirds were women, and nearly one‐third were African American. Baseline sample characteristics were mostly similar in the self‐management intervention group versus controls. At follow‐up, the self‐management group had a significantly greater reduction in depression compared to controls at visit 2 (P < .0001) and visit 3 (P = .0002). Quality of life also significantly improved in the self‐management group at visit 2 (P = .001) and visit 3 (P = .005).SignificanceAggregate MEW DB analysis of five RCTs found depressive symptom severity and quality of life significantly improved in individuals randomized to self‐management intervention versus controls.     Assessment of phytoplankton diversity, distribution, and environmental variables along the southeast coast of India

    Coastal waters are dynamic because of anthropogenic activities that contribute nutrients and contaminants. These changes have the potential to alter patterns of primary production and thus pelagic food webs. Here, we investigated the spatial variation of the phytoplankton community and its response to changing environmental variables at 84 stations along the five coastal districts of Tamil Nadu (TN). During the present study, 85 phytoplankton species were recorded, such as diatoms (64), dinoflagellates (18), silicoflagellates (1), and Cyanophyceae (2). The maximum phytoplankton abundance was recorded on the Thanjavur coast and gradually decreased towards the south coast of Tamil Nadu. Among the phytoplankton community, 50% was dominated by pennate diatoms, attributed to higher NO3− concentrations in the coastal waters due to agricultural discharge. Cluster analysis revealed that Ramanathapuram and Tirunelveli formed a closed cluster, whereas Thanjavur and Pudukottai formed a separate closed cluster associated with higher nutrient and metal concentrations, highlighting the difference in physicochemical parameters between the northern and southern districts of the TN coast. Relatively high nutrient concentrations in the coastal waters of northern districts are of greater concern, which could impact the coastal ecosystem. Coastal eutrophication is becoming a widespread phenomenon, causing disruption in the food chain and ecosystem balances and hence requiring regular monitoring and management
