Large vessel injuries - own experience
Wstęp. Przedstawiono charakterystykę obrażeń dużych naczyń u chorych leczonych w latach 1966–2004
w Klinice Chirurgii Ogólnej, Onkologicznej i Naczyniowej WIM oraz Klinice Chirurgii Ogólnej, Onkologicznej
Klatki Piersiowej WIM w Warszawie.
Materiał i metody. Przeprowadzono retrospektywną analizę 249 przypadków chorych operowanych
z powodu obrażeń dużych naczyń, czyli naczyń szyi, klatki piersiowej, brzucha oraz kończyn. Postępowanie uzależniono od rodzaju uszkodzenia naczyń oraz ich lokalizacji. Przeprowadzając zabiegi rekonstrukcyjne,
wykorzystano wiele różnych metod.
Wyniki. U 59% (115/193) pacjentów stwierdzono obrażenia tętnicy udowej, u 24% (47/193) - tętnicy
podkolanowej, u 10% (20/193) - tętnicy ramiennej, u 1% (2/193) - tętnicy szyjnej wspólnej, u 1%
(2/193) - aorty, u 1% (2/193) - tętnicy płucnej i w 0,5% (1/193) tętnicy krezkowej górnej. Następnie
w 32% (25/76) przypadków rozpoznano uszkodzenie żyły głównej dolnej, w 21% (16/76) - żyły podkolanowej,
w 11% (9/76) - żyły udowej, w 5% (4/76) - żyły ramiennej, w 5% (4/76) - żyły podobojczykowej,
w 5% (4/76) - żyły pachowej, w 5% (4/76) - żyły nerkowej, w 5% (4/76) - żyły biodrowej, w 2,5%
(2/76) - żyły szyjnej, w 2,5% (2/76) - żyły płucnej i w tym samym procencie żyły poprzecznej szyi. Czas
od urazu do przyjęcia do szpitala wynosił od 40 minut do 87 godzin. U 4 chorych ze względu na zaawansowane
zmiany martwicze a priori wykonano amputacje.
Wnioski. Obrażenia dużych naczyń są stanami bezpośrednio zagrażającymi życiu i wymagają właściwego
ukierunkowanego postępowania.Background. The aim of the authors was to present the characteristics of large vessel injuries in patients
wounded during 1966-2004 and their management in the Department of General, Oncological and Vascular
Surgery and Department of General, Oncological and Thoracic Surgery of the Military Institute of Health
Service in Warsaw.
Material and methods. Retrospective analysis was performed on the medical records of 249 patients treated
for large vessel injuries e.g. vessels of the neck, chest, abdomen and extremities. According to the type of
injury and vessel localization, different types of reconstruction were performed. All possible methods of vessel
reconstruction were done.
Results. A total of 249 patients presenting large vessel injuries were recorded. 59% (115/193) presented
femoral artery injuries, 24% (47/193) - popliteal artery injuries, 10% (20/193) - brachial artery injuries,
1% (2/193) - common carotid artery injuries, 1% (2/193) - aorta injuries, 1% (2/193) pulmonary artery
injuries and 0.5% (1/193) mesenteric artery injuries. Then 32% (25/76) of inferior vein cava injuries, 21%
(16/76) - popliteal vein, 11% (9/76) - femoral vein injuries, 5% (4/76) - brachial vein injuries, 5% (4/76)
- subclavian vein injuries, 5% (4/76) - axillary vein injuries, 5% (4/76) - renal vein injuries, 5% (4/76)
- iliac vein injuries, 2.5% (2/76) - jugular vein injuries, 2.5% (2/76) - pulmonary vein injuries and 2.5%
(2/76) - transverse vein of neck were treated. The time from injury and hospital admittance ranged from 40
minutes up to 87 hours. In 4 cases, facing evident peripheral necrotic lesions of the leg, amputation was
performed.
Conclusions. These injuries require prompt treatment to save life. The wounds have to be treated individually.
Early and adequate primary treatment, combined with early reparation and reconstruction of vessel wall,
establishment of circulation and closure of soft tissue defects gives the best functional and aesthetic results,
decreased mortality, morbidity and cost of treatment
Comparison of circular- and linear-stapled gastrojejunostomy in laparoscopic Roux-en-Y gastric bypass : a multicenter study
Introduction : Laparoscopic Roux-en-Y gastric bypass (LRYGB) is a common, well-established procedure, but no consensus regarding selection of the gastrojejunostomy (GJ) technique has been reached, and standardization of this precise technique is far from being achieved.
Aim : To compare circular-stapled and linear-stapled GJ in LRYGB in terms of operative time and postoperative complications.
Material and methods: This retrospective case-control study compared the perioperative and postoperative outcomes of LRYGB with a circular-stapled (LRYGB-CS) versus linear-stapled (LRYGB-LS) gastrojejunostomy. All patients, operated on in two academic referral care centers for bariatric surgery, were enrolled from April 2013 to June 2016. 457 patients were included (255 and 202 respectively in the LRYGB-CS and LRYGB-LS groups). After matching the groups for age, sex, body mass index, arterial hypertension, and presence of type 2 diabetes in a 1 : 1 ratio, 99 patients were enrolled in each.
Results : The total operative time was longer in the LRYGB-LS group (140 vs. 85 min, p < 0.001). The postoperative hemorrhage and wound infection rates were lower in the LRYGB-LS group (2.1% vs. 10.3%, p = 0.021, and 1.0% vs. 9.3%, p = 0.011). The readmission rates were comparable (8.2% vs. 6.1%, p = 0.593). There was no significant difference in the incidence of gastrojejunostomy leakage, stricture, port-site hernia, or marginal ulcer.
Conclusions : Both anastomosis types for LRYGB are safe and have low and comparable risks of postoperative complications. After LRYGB-CS, postoperative bleeding and wound infections are slightly more frequent; however, the operative time is shorter
Clinical manifestations of EVALI in adolescents and the impact of e-cigarettes on the respiratory disease
In recent years there has been a significant increase in the popularity of electronic cigarettes among adolescents and young adults worldwide. E-cigarettes have reached the market without the extensive preclinical toxicology testing or long-term safety studies required for conventional therapeutics and medical devices. Their effectiveness as a smoking cessation intervention, their impact at the population level and whether they are less harmful than combustible tobacco products are highly controversial. Studies show measurable adverse biological effects on organ and cell health in humans, animals and in vitro. E-cigarette use also increases the risk of respiratory diseases such as chronic obstructive pulmonary disease (COPD) and asthma. There is a serious risk of complications associated with e-cigarette use, such as EVALI (E-cigarette or Vaping Product Use-Associated Lung Injury) - serious lung damage associated with, especially for adolescents. Porównanie stężeń IL-6 i IL-10 w osoczu dzieci poddanych zabiegowi kardiochirurgicznemu z krążeniem pozaustrojowym i bez niego
Wstęp: Zabieg kardiochirurgiczny z krążeniem pozaustrojowym (CPB) wiąże
się z występowaniem odpowiedzi zapalnej. Czynnikiem sprawczym jest ekspozycja
ustroju na działanie silnych bodźców prozapalnych. Przedmiotem badań była ocena
zmian stężeń IL-6 i IL-10 podczas zabiegu na otwartym sercu i po nim.
Materiał i metody: Badaniami objęto 14 dzieci poddanych zabiegowi operacyjnemu
z użyciem CPB i 7 dzieci bez CPB. Stężenia IL-6 i IL-10 badano kolejno: przed
znieczuleniem, na początku zabiegu (z CPB i bez CPB), 30 min po rozpoczęciu CPB,
po zakończeniu zabiegu (z CPB i bez CPB), po 24 i po 72 godzinach od zakończenia
zabiegu (z CPB i bez CPB).
Wyniki: Wykazano istotny wzrost stężenia IL-6 w obu grupach 24 godziny
po zabiegu. W grupie pacjentów z CPB stężenie IL-6 po zabiegu było istotnie wyższe
niż w grupie dzieci bez CPB. W grupie dzieci, u których zabieg przeprowadzono
z zastosowaniem CPB, stwierdzono wzrost stężenia IL-10 obserwowany w momencie
zakończenia zabiegu, który powracał do wartości rejestrowanej przed zabiegiem
po 72 godzinach. Natomiast, w grupie dzieci, u których nie zastosowano CPB, stężenie
IL-10 wzrastało, osiągając najwyższą wartość 24 godziny po zakończeniu zabiegu.
Wnioski: Wyniki badania potwierdziły, że zabieg kardiochirurgiczny u dzieci
wiąże się z istotnymi zmianami stężeń cytokin w osoczu krwi obwodowej. Zastosowanie
CPB w trakcie zabiegu pogłębia zaburzenia równowagi cytokin i istotnie wpływa

Late caecal fistula after laparoscopic appendectomy managed mini-invasively – case report
Laparoscopic appendectomy is being performed increasingly, worldwide. The laparoscopic approach is associated witha lower complication rate and a shorter period of disability but some major complications still occur. We present a caseof a 22-year-old woman who underwent laparoscopic appendectomy for acute appendicitis. In 3 weeks time after surgery,after physical activity the patient presented acute abdomen. Exploratory laparoscopy revealed peritonitis causedby caecal fistula. Laparoscopic lavage and drainage of the peritoneal cavity with formal caecostomy was performed.The postoperative course was uneventful. The stoma was closed in 6 months’ time without other complications. Inour opinion laparoscopy is proven to be a safe and good option for diagnostics and treatment of some complicationsof appendectomy
The impact assessment of weight loss on an aggressive behavior and satisfaction with the connubial or cohabitation relationship in patients after Roux-en-Y gastric-by-pass surgery performed laparoscopically
The aim of the studywas an assesment the impact of weight loss in patients undergoing gastricby‑pass surgery on an aggressive behavior affecting the satisfaction with the connubial or cohabitation relationship
Material and methods. The study included a total number of 100 people (50 people with morbid obesity underwent gastric-bypass surgery and their male or female partners). The study was conducted by using two questionnaires: the Psychological Inventory of Aggression Syndrome-1 authorship by Z.B. Gaś as well as Extinguishes and the Chosen Marriage Questionnaire-2 developed by M. Plop and J. Rostowski
Results. The analysis of the results showed the influence of the weight loss on the aggressive behaviour at the examined group. Important differences were shown in the first phase of the examination among the examined group and the control group on scales: emotional self-aggression, the hostility towards surroundings and directed outside aggression. Regression analysis showed a statistical relationship between outward aggression and disappointment, 0.346 p<0.01, intimacy 0.943 p<0.01, and the result of general satisfaction with the relationship 0.832 p<0.05.While self-realization is negatively correlated with a displaced aggression -0.342 p<0.01 and the intermediate one -0.225 p<0.01. Hostility towards the environment correlates positively with intimacy 0.326 p<0.01. Indirect aggression correlates negatively with a disappointment -0.324 for p <0.05. Important differences were shown in the second stage of the examination among groups examined on the scale inspection of the aggressive behaviour. Substantial results weren’t demonstrated on scales: emotional self-aggression, hostility towards the environment and directed outside aggression In the regression analysis we received a statistically significant result: controlling an aggressive behavior correlates negatively with disappointment -0, 355 p <0,01. However, no statistically significant results were received from the partners of obese people.
Conclusions. Weight loss after gastric-by-pass surgery has the significant impact on the rarer occurrence of an aggressive behavior, which improving the quality of the connubial or cohabitation relationship
