    Social and dental status along the life course and oral health impacts in adolescents: a population-based birth cohort

    <p>Abstract</p> <p>Background</p> <p>Harmful social conditions in early life might predispose individuals to dental status which in turn may impact on adolescents' quality of life.</p> <p>Aims</p> <p>To estimate the prevalence of oral health impacts among 12 yr-old Brazilian adolescents (<it>n </it>= 359) and its association with life course socioeconomic variables, dental status and dental services utilization in a population-based birth cohort in Southern Brazil.</p> <p>Methods</p> <p>Exploratory variables were collected at birth, at 6 and 12 yr of age. The Oral Impacts on Daily Performances index (OIDP) was collected in adolescence and it was analyzed as a ranked outcome (OIDP from 0 to 9). Unadjusted and adjusted multivariable Poisson regression with robust variance was performed guided by a theoretical determination model.</p> <p>Results</p> <p>The response rate was of 94.4% (<it>n </it>= 339). The prevalence of OIDP = 1 was 30.1% (CI95%25.2;35.0) and OIDP ≥ 2 was 28.0% (CI95%23.2;32.8). The most common daily activity affected was eating (44.8%), follow by cleaning the mouth and smiling (15.6%, and 15.0%, respectively). In the final model mother schooling and mother employment status in early cohort participant's life were associated with OIDP in adolescence. As higher untreated dental caries at age 6 and 12 years, and the presence of dental pain, gingival bleeding and incisal crowing in adolescence as higher the OIDP score. On the other hand, dental fluorosis was associated with low OIDP score.</p> <p>Conclusion</p> <p>Our findings highlight the importance of adolescent's early life social environmental as mother schooling and mother employment status and the early and later dental status on the adolescent's quality of life regardless family income and use of dental services.</p

    Social gradient in the cost of oral pain and related dental service utilisation among South African adults

    Background: Oral pain affects people's daily activities and quality of life. The burden of oral pain may vary across socio-economic positions. Currently, little is known about the social gradient in the cost of oral pain among South Africans. This study therefore assessed the social gradient in the cost of oral pain and the related dental service utilisation pattern among South African adults. Methods: Data were obtained from a nationally representative cross-sectional survey of South African adults ?16 year-old (n = 2651) as part of the South African Social Attitudes Survey conducted by the South African Human Sciences Research Council. The survey included demographic data, individual-level socio-economic position (SEP), self-reported oral health status, past six months' oral pain experience and cost. The area-level SEP was obtained from the 2010 General Household Survey (n = 25,653 households) and the 2010/2011Quarterly Labour Force Survey conducted in South Africa. The composite indices used for individual-level SEP (? = 0.76) and area-level SEP (? = 0. 88) were divided into tertiles. Data analysis was done using t-tests and ANOVA. Significance was set at p < 0.05. Results: The prevalence of oral pain among the adult South Africans was 19.4 % (95 % CI = 17.2-21.9). The most commonly reported form of oral pain was 'toothache' (78.9 %). The majority of the wealthiest participants sought care from private dental clinics (64.7 %), or from public dental clinics (19.7 %), while the poorest tended to visit a public dental clinic (45 %) or nurse/general medical practitioner (17.4 %). In the poorest areas, 21 % responded to pain by 'doing nothing'. The individual expenditure for oral pain showed a social gradient from an average of ZAR61.44 spent by those of lowest SEP to ZAR433.83 by the wealthiest (national average ZAR170.92). Average time lost from school/work was two days over the six-month period, but days lost was highest for those living in middle class neighbourhoods (3.41), while those from the richest neighbourhood had lost significantly fewer days from oral pain (0.64). Conclusions: There is a significant social gradient in the burden of oral pain. Improved access to dental care, possibly through carefully planned universal National Health Insurance (NHI), may reduce oral health disparities in South Africa.Scopus 201

    Perfis epidemiologicos de saude bucal no Brasil e os modelos de vigilancia

    Abstract published in English and Portuguese English title: Oral health epidemiology and surveillance models in BrazilNational surveys are important tools for public health surveillance and thus key elements in monitoring health conditions and system performance. In the field of oral health, such surveys began with the oral health survey in 1986 and later in 1996 and with the SBBrasil Project in 2003. The 2010 edition of SBBrasil is the principal oral health surveillance strategy for the production of primary data. In order to contribute to this discussion, this article proposes: (a) to present and discuss the Brazilian experience with nationwide oral health surveys and (b) to discuss the use of data in health surveillance models. One can conclude that oral health surveys in Brazil have great possibilities as a tool for health services and academia. Such surveys have shown evident potential for verifying trends in the oral health profile, as well as for producing valid indicators for use in health services. = Inquéritos nacionais são importantes ferramentas no campo da vigilância em saúde e elementos fundamentais no monitoramento das condições de saúde e do desempenho do sistema. Na saúde bucal, tais inquéritos surgem a partir do primeiro levantamento em saúde bucal de 1986 e, posteriormente, o de 1996 e o Projeto SBBrasil 2003. A edição 2010 do SBBrasil se coloca como a principal estratégia de vigilância em saúde bucal no que diz respeito à produção de dados primários. No sentido de contribuir com essa discussão, este artigo se propôs a: (a) apresentar e discutir a experiência brasileira em inquéritos nacionais de saúde bucal; (b) discutir a utilização dos dados em modelos de vigilância em saúde. Pode-se concluir que os inquéritos de saúde bucal no Brasil têm grandes possibilidades de se estabelecer como ferramenta para os serviços e para a academia.     Dor dentaria e fatores associados em adolescentes brasileiros: a Pesquia Nacional de Saude do Escolar (PeNSE), Brasil, 2009

    Abstract published in English and Portuguese English title: Dental pain and associated factors in Brazilian adolescents: the National school-based health survey (PeNSE), Brazil, 2009The aim of this study was to assess the prevalence of dental pain and associated socio-demographic and behavioral factors in Brazilian adolescents, using data from the National School-Based Health Survey (PeNSE), Brazil, 2009. The survey was conducted by the Brazilian Institute of Geography and Statistics (IBGE) and Ministry of Health in students 11 to 17 years of age or older in the 27 State capitals, using a self-administered questionnaire. Analyses included Poisson regression following a hierarchical approach. Prevalence of dental pain in the sample (n = 54,985) in the previous six months was 17.8% (95%CI: 17.5-18.1). Higher prevalence was associated with female gender, age 14 years and over, racial self-identification as black, brown, or indigenous, enrollment in public schools, lower maternal schooling, not living with the mother, history of smoking or drinking, less frequent toothbrushing, and heavy consumption of sweets and soft drinks. Dental pain was thus associated with socio-demographic factors and health-related behaviors. = O objetivo deste estudo foi estimar a prevalência da dor de dente em adolescentes brasileiros e analisar fatores sociodemográficos e comportamentais associados, utilizando os dados da Pesquisa Nacional de Saúde do Escolar (PeNSE) de 2009. A pesquisa foi realizada pelo Instituto Brasileiro de Geografia e Estatística e pelo Ministério da Saúde em escolares com idades entre 11 e 17 anos ou mais, das 27 capitais brasileiras, por meio de questionário autoaplicável. Utilizou-se a análise de regressão de Poisson, segundo um modelo hierárquico de determinação. A prevalência de dor na amostra (n = 54.985) nos últimos seis meses foi de 17,8% (IC95%: 17,5-18,1). Prevalências mais elevadas foram encontradas em mulheres, naqueles com 14 anos ou mais, das raças preta, parda e indígena, de escolas públicas, cujas mães tinham baixa escolaridade, que não moravam com a mãe, que haviam experimentado cigarro e álcool alguma vez na vida, que relataram menor frequência de escovação e maior consumo de guloseimas e refrigerantes.
