Gathering opinion leader data for a tailored implementation intervention in secondary healthcare: a randomised trial
Background: Health professionalsâ behaviour is a key component in compliance with evidence-based recommendations. Opinion leaders are an oft-used method of influencing such behaviours in implementation studies, but reliably and cost effectively identifying them is not straightforward. Survey and questionnaire based data collection methods have potential and carefully chosen items can â in theory â both aid identification of opinion leaders and help in the design of an implementation strategy itself. This study compares two methods of identifying opinion leaders for behaviour-change interventions. Methods: Healthcare professionals working in a single UK mental health NHS Foundation Trust were randomly allocated to one of two questionnaires. The first, slightly longer questionnaire, asked for multiple nominations of opinion leaders, with specific information about the nature of the relationship with each nominee. The second, shorter version, asked simply for a list of named âchampionsâ but no more additional information. We compared, using Chi Square statistics, both the questionnaire response rates and the number of health professionals likely to be influenced by the opinion leaders (i.e. the âcoverageâ rates) for both questionnaire conditions. Results: Both questionnaire versions had low response rates: only 15% of health professionals named colleagues in the longer questionnaire and 13% in the shorter version. The opinion leaders identified by both methods had a low number of contacts (range of coverage, 2â6 each). There were no significant differences in response rates or coverage between the two identification methods. Conclusions: The low response and population coverage rates for both questionnaire versions suggest that alternative methods of identifying opinion leaders for implementation studies may be more effective. Future research should seek to identify and evaluate alternative, non-questionnaire based, methods of identifying opinion leaders in order to maximise their potential in organisational behaviour change interventions
A pipeline for high throughput detection and mapping of SNPs from EST databases
Single nucleotide polymorphisms (SNPs) represent the most abundant type of genetic variation that can be used as molecular markers. The SNPs that are hidden in sequence databases can be unlocked using bioinformatic tools. For efficient application of these SNPs, the sequence set should be error-free as much as possible, targeting single loci and suitable for the SNP scoring platform of choice. We have developed a pipeline to effectively mine SNPs from public EST databases with or without quality information using QualitySNP software, select reliable SNP and prepare the loci for analysis on the Illumina GoldenGate genotyping platform. The applicability of the pipeline was demonstrated using publicly available potato EST data, genotyping individuals from two diploid mapping populations and subsequently mapping the SNP markers (putative genes) in both populations. Over 7000 reliable SNPs were identified that met the criteria for genotyping on the GoldenGate platform. Of the 384 SNPs on the SNP array approximately 12% dropped out. For the two potato mapping populations 165 and 185 SNPs segregating SNP loci could be mapped on the respective genetic maps, illustrating the effectiveness of our pipeline for SNP selection and validation
Abrasive, Silica Phytoliths and the Evolution of Thick Molar Enamel in Primates, with Implications for the Diet of Paranthropus boisei
Background: Primatesâincluding fossil species of apes and homininsâshow variation in their degree of molar enamel thickness, a trait long thought to reflect a diet of hard or tough foods. The early hominins demonstrated molar enamel thickness of moderate to extreme degrees, which suggested to most researchers that they ate hard foods obtained on or near the ground, such as nuts, seeds, tubers, and roots. We propose an alternative hypothesisâthat the amount of phytoliths in foods correlates with the evolution of thick molar enamel in primates, although this effect is constrained by a species â degree of folivory. Methodology/Principal Findings: From a combination of dietary data and evidence for the levels of phytoliths in plant families in the literature, we calculated the percentage of plant foods rich in phytoliths in the diets of twelve extant primates with wide variation in their molar enamel thickness. Additional dietary data from the literature provided the percentage of each primateâs diet made up of plants and of leaves. A statistical analysis of these variables showed that the amount of abrasive silica phytoliths in the diets of our sample primates correlated positively with the thickness of their molar enamel, constrained by the amount of leaves in their diet (R 2 = 0.875; p,.0006). Conclusions/Significance: The need to resist abrasion from phytoliths appears to be a key selective force behind the evolution of thick molar enamel in primates. Two approaches to the study of the origin of life.
This paper compares two approaches that attempt to explain the origin of life, or biogenesis. The more established approach is one based on chemical principles, whereas a new, yet not widely known approach begins from a physical perspective. According to the first approach, life would have begun with - often organic - compounds. After having developed to a certain level of complexity and mutual dependence within a non-compartmentalised organic soup, they would have assembled into a functioning cell. In contrast, the second, physical type of approach has life developing within tiny compartments from the beginning. It emphasises the importance of redox reactions between inorganic elements and compounds found on two sides of a compartmental boundary. Without this boundary, ÂżlifeÂż would not have begun, nor have been maintained; this boundary - and the complex cell membrane that evolved from it - forms the essence of life
The Gaia-ESO Public Spectroscopic Survey: Motivation, implementation, GIRAFFE data processing, analysis, and final data products
The Gaia-ESO Public Spectroscopic Survey is an ambitious project designed to
obtain astrophysical parameters and elemental abundances for 100,000 stars,
including large representative samples of the stellar populations in the
Galaxy, and a well-defined sample of 60 (plus 20 archive) open clusters. We
provide internally consistent results calibrated on benchmark stars and star
clusters, extending across a very wide range of abundances and ages. This
provides a legacy data set of intrinsic value, and equally a large wide-ranging
dataset that is of value for homogenisation of other and future stellar surveys
and Gaia's astrophysical parameters. This article provides an overview of the
survey methodology, the scientific aims, and the implementation, including a
description of the data processing for the GIRAFFE spectra. A companion paper
(arXiv:2206.02901) introduces the survey results. Gaia-ESO aspires to quantify
both random and systematic contributions to measurement uncertainties. Thus all
available spectroscopic analysis techniques are utilised, each spectrum being
analysed by up to several different analysis pipelines, with considerable
effort being made to homogenise and calibrate the resulting parameters. We
describe here the sequence of activities up to delivery of processed data
products to the ESO Science Archive Facility for open use. The Gaia-ESO Survey
obtained 202,000 spectra of 115,000 stars using 340 allocated VLT nights
between December 2011 and January 2018 from GIRAFFE and UVES. The full
consistently reduced final data set of spectra was released through the ESO
Science Archive Facility in late 2020, with the full astrophysical parameters
sets following in 2022

Management of complex systems: towards agent based gaming for policy
In this chapter, we discuss the implications of complexities in societal systems for management. After discussing some essential features of complex systems, we discuss the current focus of managers and management theory on prediction and the problems arising from this perspective. A short overview is given of the leadership and management literature, identifying what information is lacking concerning the management of complex systems. Next agent-based gaming, which allows for modeling a virtual and autonomous population in a computer-game setting, is introduced as a tool to explore the possibilities to manage complex systems. The chapter concludes with a research agenda for management and leadership in complex systems
Controversy and consensus on a clinical pharmacist in primary care in the Netherlands
Background Controversy about the introduction of a non-dispensing pharmacist in primary care practice hampers implementation. Objective The aim of this study is to systematically map the debate on this new role for pharmacists amongst all stakeholders to uncover and understand the controversy and consensus. Setting: Primary health care in the Netherlands. Method Q methodology. 163 participants rank-ordered statements on issues concerning the integration of a non-dispensing pharmacist in primary care practice. Main outcome measure: Stakeholder perspectives on the role of the non-dispensing pharmacist and pharmaceutical care in primary care. Results This study identified the consensus on various features of the non-dispensing pharmacist role as well as the financial, organisational and collaborative aspects of integrating a non-dispensing pharmacist in primary care practice. Q factor analysis revealed four perspectives: âthe independent community pharmacistâ, âthe independent clinical pharmacistâ, âthe dependent clinical pharmacistâ and âthe medication therapy management specialistâ. These four perspectives show controversies to do with the level of professional independency of the non-dispensing pharmacist and the level of innovation of task performance. Conclusion Despite the fact that introducing new professional roles in healthcare can lead to controversy, the results of this Q study show the potential of a non-dispensing pharmacist as a pharmaceutical care provider and the willingness for interprofessional collaboration. The results from the POINT intervention study in the Netherlands will be an important next step in resolving current controversies
