    EMERGING TRENDS IN PATIENT COUNSELLING: CURRENT SCENARIO

    Objective: To evaluate the impact of patient counselling in terms of knowledge, attitudes and practices (KAP) outcomes among retail and hospital pharmacist in Satara and Sanagli district.Methods: A cross-sectional survey was conducted on knowledge, attitudes and practices of patient counselling adopted by retail and hospital pharmacists from different areas of Satara and Sangali district. A specially designed questionnaire was used for data collection. Total 190 respondents were analyzed for the study.Results: The highlighting result of the survey based project revealed that almost all retail as well as hospital pharmacists were practicing patient counselling without any charges and there is necessity of separate patient counselling cabinet. Pharmacist was lacking in adopting modern techniques, aids for counselling and 20% only attended patient counselling course (PCC) by Maharashtra State Pharmacy Council (MSPC).Conclusion: KAP study revealed the positive attitude, knowledge and existence of patient counselling practices but not done in an appropriate manner. Step should be taken by authorities, pharmacist and the patient to make counselling more effective.Â

    Native-like aggregates of Factor VIII (FVIII) are immunogenic von Willebrand Factor deficient and hemophilia A mice

    The administration of recombinant Factor VIII (FVIII) is the first line therapy for Hemophilia A (HA), but 25–35% of patients develop an inhibitory antibody response. In general, the presence of aggregates contributes to unwanted immunogenic responses against therapeutic proteins. FVIII has been shown to form both native-like and non-native aggregates. Previously, we showed that non-native aggregates of FVIII are less immunogenic compared to the native protein. Here we investigated the effect of native-like aggregates of FVIII on immunogenicity in HA and von Willebrand Factor knockout (vWF−/−) mice. Mice immunized with native-like aggregates showed significantly higher inhibitory antibody titers compared to animals that received native FVIII. Following re-stimulation in vitro with native FVIII, the activation of CD4+ T cells isolated from mice immunized with native-like aggregates is ~4 fold higher than mice immunized with the native protein. Furthermore, this is associated with increases in the secretion of pro-inflammatory cytokines IL-6 and IL-17 in the native-like aggregate treatment group. The results indicate that the native-like aggregates of FVIII are more immunogenic than native FVIII for both the B cell and T cell responses

The main advantages of this protocol includes shorter reaction time, eco-friendly, mild reaction condition, easy work-up procedures, high yields of products, are the attractive features of the present methodology.     Vacuum foam dried sugar-phosphate amorphous mixtures for stabilization of doxorubicin hydrochloride

    The objective of the present study was to stabilize doxorubicin hydrochloride in sugar-phosphate amorphous mixtures at ambient temperature by drying it with vacuum foam drying. Finished products were evaluated for foam characteristic, residual moisture content, reconstitution time, percent drug recovery and drug-excipient interactions. FTIR studies revealed existence of physical interaction of drug with sugar. Light microscopy showed formation of amorphous glass which was supported by the observations of XRPD analyses. The optimized composition in vacuum foam drying was processed by lyophilization and their stability was compared. Storage at ambient temperature for 6 months showed that stability of vacuum foam dried product was better than lyophilized products. The amount of residual moisture affected the stability of drug. The detailed study revealed lactose and sodium dihydrogen phosphate is best suited for stabilization of doxorubicin hydrochloride at room temperature.Colegio de Farmacéuticos de la Provincia de Buenos Aire

    Use of a folding model and in situ spectroscopic techniques for rational formulation development and stability testing of Monoclonal antibody therapeutics

    Aggregation is a critical issue that hampers the development of monoclonal antibody therapeutics (Mabs). Traditionally, aggregation is considered a process in which native forms of proteins are transformed into an unstable highly associated form through an intermediate formation step. Here we describe the unfolding of an anti CD40 antibody using a folding model based on Lumry-Eyring nucleated polymerization (LENP) model. This model captures several experimental features of the thermal unfolding of this protein as studied by common in situ biophysical techniques such as circular dichroism, fluorescence spectroscopy and turbidity measurements. According to this model, the unfolding and aggregation of the anti CD40 antibody is determined by several distinct steps that include conformational change(s) to generate aggregation prone states, reversible oligomer formation, nucleation and growth as well as their kinetics and the formation of higher order assemblies/aggregates. Furthermore, the loss of monomer is controlled by both thermodynamic (equilibrium unfolding) and kinetic determinants of the unfolding process. This approach captures both of these rate-limiting steps. It can be concluded that this approach is sensitive to formulation conditions such as protein concentration, changes in buffer conditions, and temperature stress. The potential use of this approach in formulation development and stability testing of Mabs is discussed

    X-ray and ion emission studies from subnanosecond laser-irradiated SiO2 aerogel foam targets

    In this experiment, a comparative study of ion and X-ray emission from both a SiO2 aerogel foam and a quartz target is performed. The experiment is performed using Nd:glass laser system operated at laser energy up to 15 J with a pulse duration of 500 ps with focusable intensity of 1013–1014 W/cm2 on target. X-ray fluxes in different spectral ranges (soft and hard) are measured by using X-ray diodes covered with Al filters of thickness 5 µm (0.9–1.56 keV) and 20 µm (3.4–16 keV). A 2.5 times enhancement in soft X-ray flux (0.9–1.56 keV) and a decrease of 1.8 times in hard X rays (3.4–16 keV) for 50 mg/cc SiO2 aerogel foam is observed compared with the solid quartz. A decrease in the flux of the K-shell line emission spectrum of soft X rays is noticed in the case of the foam targets. The high-resolution K-shell spectra (He-like) of Si ions in both the cases are analyzed for the determination of plasma parameters by comparing with FLYCHK simulations. The estimated plasma temperature and density are T c = 180 eV, n e = 7 × 1020 cm−3 and T c = 190 eV, n e = 4 × 1020 cm−3 for quartz and SiO2 aerogel foam, respectively. To measure the evolution of the plasma moving away from the targets, four identical ion collectors are placed at different angles (22.5, 30, 45, and 67.5°) from target normal. The angular distribution of the thermal ions are scaled as cosnθ with respect to target normal, where n = 3.8 and 4.8 for the foam and quartz, respectively.     Matrix Metalloproteinase-1 and -9 in Human Placenta during Spontaneous Vaginal Delivery and Caesarean Sectioning in Preterm Pregnancy

    Preterm birth is a major public health problem in terms of loss of life, long-term and short term disabilities worldwide. The process of parturition (both term and preterm) involves intensive remodelling of the extracellular matrix (ECM) in the placenta and fetal membranes by matrix metalloproteinases (MMPs). Our previous studies show reduced docosahexaenoic acid (DHA) in women delivering preterm. Further omega 3 fatty acids are reported to regulate MMP levels. This study was undertaken to examine the placental levels of MMPs and their association with placental DHA levels in women delivering preterm. The levels of MMP-1 and MMP-9 in 74 women delivering preterm (52 by spontaneous vaginal delivery and 22 by caesarean sectioning) and 75 women delivering at term (59 by spontaneous vaginal delivery and 16 by caesarean sectioning) were determined by enzyme-linked immunosorbent assay (ELISA) and their association with placental DHA was studied. Placental MMP-1 levels were higher (p<0.05) in women delivering preterm (both by spontaneous vaginal delivery and caesarean sectioning) as compared to those delivering at term. In contrast, placental MMP-9 levels in preterm pregnancies was higher (p<0.05) in women with spontaneous vaginal delivery while lower (p<0.05) in women delivering by caesarean sectioning. Low placental DHA was associated with higher placental MMP-9 levels. Our study suggests a differential effect of mode of delivery on the levels of MMPs from placenta. Further this study suggests a negative association of DHA and the levels of MMP-9 in human placenta although the mechanisms need further study
