    Risk and protective factors for meningococcal disease in adolescents: matched cohort study

    Objective: To examine biological and social risk factors for meningococcal disease in adolescents. Design: Prospective, population based, matched cohort study with controls matched for age and sex in 1:1 matching. Controls were sought from the general practitioner. Setting: Six contiguous regions of England, which represent some 65% of the country’s population. Participants: 15-19 year olds with meningococcal disease recruited at hospital admission in six regions (representing 65% of the population of England) from January 1999 to June 2000, and their matched controls. Methods: Blood samples and pernasal and throat swabs were taken from case patients at admission to hospital and from cases and matched controls at interview. Data on potential risk factors were gathered by confidential interview. Data were analysed by using univariate and multivariate conditional logistic regression. Results: 144 case control pairs were recruited (74 male (51%); median age 17.6). 114 cases (79%) were confirmed microbiologically. Significant independent risk factors for meningococcal disease were history of preceding illness (matched odds ratio 2.9, 95% confidence interval 1.4 to 5.9), intimate kissing with multiple partners (3.7, 1.7 to 8.1), being a university student (3.4, 1.2 to 10) and preterm birth (3.7, 1.0 to 13.5). Religious observance (0.09, 0.02 to 0.6) and meningococcal vaccination (0.12, 0.04 to 0.4) were associated with protection. Conclusions: Activities and events increasing risk for meningococcal disease in adolescence are different from in childhood. Students are at higher risk. Altering personal behaviours could moderate the risk. However, the development of further effective meningococcal vaccines remains a key public health priority

    Business Angels, Social Networks, and Radical Innovation

    Innovation is critical for firm and national competitiveness. However, financing innovation is increasingly difficult for early-stage, high-risk projects, as banks and venture capital firms are focusing on later-stage, less risky projects. To fill this gap, US and European entrepreneurs are turning for seed funding to Business Angels (BAs) and Business Angel Networks (BANs). We describe the role of BAs and BANs in the US and Europe from the perspectives of entrepreneurship theory and social network theory. We show how BANs can strengthen ties between entrepreneurs and individual investors under highly uncertain conditions. We also study the links between formal and informal private equity finance, raising wider questions about the funding and performance of clusters of innovation. Finally, we suggest that differences in network characteristics, rather than the availability of projects, explain the large differences in the size and performance of the BA sectors in the US and Europe

    Absence of Dipole Transitions in Vortices of Type II Superconductors

    The response of a single vortex to a time dependent field is examined microscopically and an equation of motion for vortex motion at non-zero frequencies is derived. Of interest are frequencies near Δ2/EF\Delta^{2}/E_{F}, where Δ\Delta is the bulk energy gap and EFE_{F} is the fermi energy. The low temperature, clean, extreme type II limit and maintaining of equilibrium with the lattice are assumed. A simplification occurs for large planar mass anisotropy. Thus the results may be pertinent to materials such as NbSe2NbSe_2 and high temperature superconductors. The expected dipole transition between core states is hidden because of the self consistent nature of the vortex potential. Instead the vortex itself moves and has a resonance at the frequency of the transition.Comment: 12 pages, no figure

    Nearly 60% of the Dutch population undergoing surgery is aged 65 years and over. Older patients are at increased risk of developing perioperative complications (e.g., myocardial infarction, pneumonia, or delirium), which may lead to a prolonged hospital stay or death. Preoperative risk stratification calculates a patient's risk by evaluating the presence and extent of frailty, pathophysiological risk factors, type of surgery, and the results of (additional) testing. Type of anesthesia, fluid management, and pain management affect outcome of surgery. Recent developments focus on multimodal perioperative care of the older patient, using minimally invasive surgery, postoperative anesthesiology rounds, and early geriatric consultation

    The Entrepreneurial State: An Ownership Competence Perspective

    Academics, pundits, and policymakers have recently called for a stronger governmental role in the economy to tackle social issues such as inequality and grand challenges like global warming. Despite a general recognition among economists and management scholars that government efforts to guide and control innovation or subsidize private entrepreneurs have failed to yield results, these calls also describe an entrepreneurial state in which bureaucrats, not entrepreneurs, direct not only basic research but also applied technological development. Building on the notions of economic competence and ownership competence we argue that even well-intentioned and strongly motivated public actors lack the ability to manage the process of innovation, especially under Knightian uncertainty. As stewards of resources owned by the public, government bureaucrats do not exercise the ultimate responsibility that comes with ownership. Moreover, government ownership of firms and labs and government intervention in the management of privately owned assets hampers the competitive process of putting ownership of innovative firms and projects in the hands of individuals and groups with higher levels of ownership ability. We suggest that ownership competence differs systematically between public and private actors, particularly around innovation, with important implications for innovation policy

    Potential energy landscape-based extended van der Waals equation

    The inherent structures ({\it IS}) are the local minima of the potential energy surface or landscape, U(r)U({\bf r}), of an {\it N} atom system. Stillinger has given an exact {\it IS} formulation of thermodynamics. Here the implications for the equation of state are investigated. It is shown that the van der Waals ({\it vdW}) equation, with density-dependent aa and bb coefficients, holds on the high-temperature plateau of the averaged {\it IS} energy. However, an additional ``landscape'' contribution to the pressure is found at lower TT. The resulting extended {\it vdW} equation, unlike the original, is capable of yielding a water-like density anomaly, flat isotherms in the coexistence region {\it vs} {\it vdW} loops, and several other desirable features. The plateau energy, the width of the distribution of {\it IS}, and the ``top of the landscape'' temperature are simulated over a broad reduced density range, 2.0ρ0.202.0 \ge \rho \ge 0.20, in the Lennard-Jones fluid. Fits to the data yield an explicit equation of state, which is argued to be useful at high density; it nevertheless reproduces the known values of aa and bb at the critical point

    Classical Stabilization of Homogeneous Extra Dimensions

    If spacetime possesses extra dimensions of size and curvature radii much larger than the Planck or string scales, the dynamics of these extra dimensions should be governed by classical general relativity. We argue that in general relativity, it is highly nontrivial to obtain solutions where the extra dimensions are static and are dynamically stable to small perturbations. We also illustrate that intuition on equilibrium and stability built up from non-gravitational physics can be highly misleading. For all static, homogeneous solutions satisfying the null energy condition, we show that the Ricci curvature of space must be nonnegative in all directions. Much of our analysis focuses on a class of spacetime models where space consists of a product of homogeneous and isotropic geometries. A dimensional reduction of these models is performed, and their stability to perturbations that preserve the spatial symmetries is analyzed. We conclude that the only physically realistic examples of classically stabilized large extra dimensions are those in which the extra-dimensional manifold is positively curved.Comment: 25 pages; minor changes, improved reference

    Generation of defects and disorder from deeply quenching a liquid to form a solid

    We show how deeply quenching a liquid to temperatures where it is linearly unstable and the crystal is the equilibrium phase often produces crystalline structures with defects and disorder. As the solid phase advances into the liquid phase, the modulations in the density distribution created behind the advancing solidification front do not necessarily have a wavelength that is the same as the equilibrium crystal lattice spacing. This is because in a deep enough quench the front propagation is governed by linear processes, but the crystal lattice spacing is determined by nonlinear terms. The wavelength mismatch can result in significant disorder behind the front that may or may not persist in the latter stage dynamics. We support these observations by presenting results from dynamical density functional theory calculations for simple one- and two-component two-dimensional systems of soft core particles.Comment: 25 pages, 11 figure

    Calculation of the interspecies s-wave scattering length in an ultracold Na-Rb vapor

    We report the calculation of the interspecies scattering length for the sodium-rubidium (Na-Rb) system. We present improved hybrid potentials for the singlet X1Σ+X^1\Sigma^+ and triplet a3Σ+a^3\Sigma^+ ground states of the NaRb molecule, and calculate the singlet and triplet scattering lengths asa_{s} and ata_{t} for the isotopomers 23^{23}Na87^{87}Rb and 23^{23}Na85^{85}Rb.
