    Unmet Needs of Unaccompanied Minors from Central America: Perceptions of Professionals from Multiple Sectors

    Background: In recent years, there has been a significant influx of Central American youth who cross the U.S.-Mexico border without a parent or legal guardian. While federal procedures are established to oversee the treatment and placement of unaccompanied minors, less is known about the needs of unaccompanied minors and available services afterthey are placed in appropriate custody. Methods: Purposive and strategic sampling of professionals from medical, social work, education and legal fields was conducted. Fourteen informants were recruited across the U.S. for confidential semi-structured interviews, which were audio recorded and transcribed in 2016 to 2017. Standard anthropological methods were employed, including immersion and crystallization techniques that incorporated within-case and across-case analytic strategies. Results: Recruited informants had previous or current direct experience working with immigrant minors for three or more years in addition to extensive public health experience. Unaccompanied minors were described as predominantly adolescent boys, ranging from 2 to 18 years old. Children faced unmet mental, medical and psychosocial needs that are interconnected and largely unmet due to childrenā€™s legal status and ineligibility to access services in most jurisdictions. The most pressing challenge affecting the health of youth was their immigration status. Across sectors,informants revealed an imbalance between the growing demand for services, including legal counsel, and the limited supply of professionals and well-funded services to meet childrenā€™s complex needs. Informants emphasized the value of trauma-informed practice, Spanish language proficiency, child-informed practice and intercultural awareness and humility towards their clients as key features of equipped professionals working with this vulnerable population. Regardless of sector, professionals emphasized the importance of culturally-informed care to immigrant youth. Building these skills is associated with greater confidence to provide services to unaccompanied minors, many of whom have experienced as significant burden of childhood trauma. Conclusions: The health needs of unaccompanied minors are complex and span across medical, social work, education, and legal fields. Interdisciplinary collaboration is needed to address the challenges faced by unaccompanied minors in their efforts to integrate themselves into their new communities and promote their resilience. Promising initiatives include co-location of inter-sector services for increased access and efficiency of services and development of professional trainings and resources for professionals in sectors that serve this population

    Prognostic impact of coronary microcirculation abnormalities in systemic sclerosis: a prospective study to evaluate the role of non-invasive tests

    INTRODUCTION: Microcirculation dysfunction is a typical feature of systemic sclerosis (SSc) and represents the earliest abnormality of primary myocardial involvement. We assessed coronary microcirculation status by combining two functional tests in SSc patients and estimating its impact on disease outcome. METHODS: Forty-one SSc patients, asymptomatic for coronary artery disease, were tested for coronary flow velocity reserve (CFR) by transthoracic-echo-Doppler with adenosine infusion (A-TTE) and for left ventricular wall motion abnormalities (WMA) by dobutamine stress echocardiography (DSE). Myocardial multi-detector computed tomography (MDCT) enabled the presence of epicardial stenosis, which could interfere with the accuracy of the tests, to be excluded. Patient survival rate was assessed over a 6.7- Ā± 3.5-year follow-up. RESULTS: Nineteen out of 41 (46%) SSc patients had a reduced CFR (ā‰¤2.5) and in 16/41 (39%) a WMA was observed during DSE. Furthermore, 13/41 (32%) patients showed pathological CFR and WMA. An inverse correlation between wall motion score index (WMSI) during DSE and CFR value (r = -0.57, P <0.0001) was observed; in addition, CFR was significantly reduced (2.21 Ā± 0.38) in patients with WMA as compared to those without (2.94 Ā± 0.60) (P <0.0001). In 12 patients with abnormal DSE, MDCT was used to exclude macrovasculopathy. During a 6.7- Ā± 3.5-year follow-up seven patients with abnormal coronary functional tests died of disease-related causes, compared to only one patient with normal tests. CONCLUSIONS: A-TTE and DSE tests are useful tools to detect non-invasively pre-clinical microcirculation abnormalities in SSc patients; moreover, abnormal CFR and WMA might be related to a worse disease outcome suggesting a prognostic value of these tests, similar to other myocardial diseases

We compare their decay under the effect of dissipation and show, through a phase diagram, that entanglement is more likely to survive asymptotically than twin oscillators correlations

    Isolation of Unknown Genes from Human Bone Marrow by Differental Screening and Single-Pass cDNA Sequences Determination

    A cDNA sequencing project was initiated to characterize gene expression in human bone marrow and develop strategies to isolate novel genes. Forty-eight random cDNAs from total human bone marrow were subjected to single-pass DNA sequence analysis to determine a limited complexity of mRNAs expressed in the bone marrow. Overall, 8 cDNAs (17%) showed no similarity to known sequences. Information from DNA sequence analysis was used to develop a differential prescreen to subtract unwanted cDNAs and to enrich for unknown cDNAs. Forty-eight cDNAs that were negative with a complex probe were subject to single-pass DNA sequence determination. Of these prescreened cDNAs, the number of unknown sequences increased to 23 (48%). Unknown cDNAs were also characterized by RNA expression analysis using 25 different human leukemic cell lines. Of 13 unknown cDNAs tested, 10 were expressed in all cell types tested and 3 revealed a hematopoietic lineage-restricted expression pattern. Interestingly, while a total of only 96 bone marrow cDNAs were sequenced, 31 of these cDNAs represent sequences from unknown genes and 12 showed significant similarities to sequences in the data bases. One cDNA revealed a significant similarity to a serine/threonine-protein kinase at the amino acid level (56% identity for 123 amino acids) and may represent a previously unknown kinase. Differential screening techniques coupled with single-pass cDNA sequence analysis may prove to be a powerful and simple technique to examine developmental gene expression

    A modelling study of discontinuous biological irrigation

    Irrigation of infaunal dwellings can lead to significant alteration of solute distributions in sediments. As a result, sediment-seawater fluxes of nutrients and dissolved carbon are greatly enhanced, and the biology of benthic communities is affected. The most realistic mathematical representation of irrigation and its effect on sediment geochemistry is Aller\u27s (1980) cylinder model. One critical assumption of this model is that burrows are irrigated continually, and that burrow water solute concentrations are identical to overlying water concentrations at all times. However, the vast majority of infaunal tube- and burrow-dwelling organisms irrigate periodically, i.e. in an on/off cycle. During periodic irrigation, the solute concentration at the tube wall may vary between the limits imposed by the flux from the porewater and the concentration in the overlying water. We introduce modifications to the cylinder model which allow for periodic irrigation. We assess how periodic irrigation affects solute profiles and fluxes of two chemical constituents, silica and ammonium, for different population densities (distance between burrows) and organism sizes (burrow radii). Silica and ammonium follow first and zeroth order reaction kinetics, respectively, and illustrate the behavior of two general reaction classes. Model results show that the effects of periodic irrigation vary with the class of reaction considered. For silica, radially-averaged profiles during discontinuous irrigation varied less than 15% from those with continuous irrigation for nearly all burrow sizes, burrow distances and reaction rate constants considered. However, we observed large temporal changes (as much as a factor of 6) in the areally-averaged silica flux over the irrigation cycle. Despite this time-dependence, the time-averaged silica flux was similar to that calculated for the continuous case. For ammonia, radially-averaged solute profiles were extremely sensitive to the duration of irrigation. In this case, the differences between discontinuous and continuous irrigation were greatest when the duration of irrigation was short (e.g. 5 min), and when the inter-burrow distance was small. As with silica, there was a strong time-dependence in areally-averaged ammonia flux when irrigation was periodic. However, the time-averaged ammonia flux is identical to the flux calculated for continuous irrigation. Our results suggest that irrigation behavior can affect the local burrow environment and this imposes a time dependence on solute fluxes
