    Nosocomial Wound Infection amongst Post Operative Patients and their Antibiograms at Tertiary Care Hospital in India

    Nosocomial infection constitutes a major public health problem worldwide. Increasing antibiotic resistance of pathogens associated with nosocomial infections also becomes a major therapeutic challenge for physicians. Thus, the aim of this study was to identify post operative bacterial infections in the patients developing surgical site infections at a tertiary University hospital in North India during July 2013 to Dec 2013.Methods: One hundred and ninety six swabs/pus specimens from various types of surgical sites suspected to be infected onclinical grounds were processed, by standard methods and antibiotic susceptibility testing of all the isolates was done by usingKirby Baur disc diffusion technique.Results: Of the one hundred and fifty-eight organisms isolated, the most common was Staphylococcus aureus (27.8 %), followedby Escherchia coli (24.05 %), Klebsiella pneumoniae (13.29 %), Pseudomonas aeruginosa (6.32%), Klebsiella oxytoca (5%),Enterococcus (5.6%) and other miscellaneous gram negative rods (9.4%) and Streptococcus pyogenes (1.30%). About 50% of theStaphylococcus aureus isolates were found to be methicillin resistant. In case of Escherichia coli, more than one-third of the isolates were found to be ESBL producers. The resistance to third generation cephalosporins and the quinolone ciprofloxacin was also quite high. Other isolates also showed a very high level of antibiotic resistance.Conclusion: In addition to the economic burden for antibiotic treatment, such infections for multi-resistant organisms are a serious threat to our surgical patients. To prevent these happenings, there is ar urgent need to adopt basic principles of asepsis and sterilization and to make judicious use of prophylactic and therapeutic antibiotics and determine current antimicrobial resistance to commonly prescribed drugs.Keywords: Wound infection; microorganisms; anti-microbial sensitivit

    Interannual and long term variability of low level jetstream of the Asian summer monsoon

    Interannual and long-term variability of the low level jetstream (LLJ) of the Asian summer monsoon during June to September was studied using reanalysis wind data. In order to study the variability, the domain of the LLJ covering the Indian and west Pacific Oceans was divided into six regions (boxes) based on their physical characteristics. The long term variability of LLJ and its relation with Indian monsoon rainfall and El Nino index were analysed. The low level circulation over Indian Ocean becomes weak during the positive phase of El Nino Southern Oscillation leads to warmer sea surface temperature over Indian Ocean. The strength of the low level jet over India depends on the convective heating of the atmosphere and not on the strength of the south Indian Ocean trade wind (ITz) which remains a passive component of the LLJ. The LLJ flow through peninsular India is weaker and the flow south of peninsular India is stronger during the recent decades in association with increasing number of break monsoon days. The cross equatorial flow over west Pacific Ocean has statistically significant increasing trend related to the increasing convection in the west Pacific Ocean

    Salivary levels of Streptococcus mutans and Streptococcus sanguinis in early childhood caries: An in vivo study

    Background: The oral microflora primarily involved in early childhood caries (ECC) is Streptococcus mutans while Streptococcus sanguinis is related to healthy caries-free mouth. The antagonism between the two at biological level is well recognized. Aim: This study aims to compare the pre- and posttreatment salivary levels of S. mutans and S. sanguinis in ECC. Methods: This cross-sectional study was performed on 40 eligible participants. Clinical examination was done, and decayed teeth were recorded. Pre- and Posttreatment saliva sample was taken, processed and bacterial counts were compared. Results: A statistically significant posttreatment reduction was found in S. mutans counts; however, a significant posttreatment elevation was seen in S. sanguinis counts (P < 0.001) using Wilcoxon signed-rank test. A significant weak positive correlation was found between S. mutans and decayed teeth in the mouth (r = 0.366 and P = 0.028) using Spearman's correlation. No significant gender association was found between pre- and posttreatment S. mutans and S. sanguinis counts (P = 0.908) and also between decayed teeth (P = 0.321) using Mann–Whitney U-test. Conclusion: Complete dental treatment procedure showed significant reduction and elevation in S. mutans and S. sanguinis counts, respectively. A positive association was observed between S. mutans and ECC while S. sanguinis were positively associated with posttreatment oral environment

    Crystallography, Molecular Modeling, and COX-2 Inhibition Studies on Indolizine Derivatives

    The cyclooxygenase-2 (COX-2) enzyme is an important target for drug discovery and development of novel anti-inflammatory agents. Selective COX-2 inhibitors have the advantage of reduced side-effects, which result from COX-1 inhibition that is usually observed with nonselective COX inhibitors. In this study, the design and synthesis of a new series of 7-methoxy indolizines as bioisostere indomethacin analogues (5a–e) were carried out and evaluated for COX-2 enzyme inhibition. All the compounds showed activity in micromolar ranges, and the compound diethyl 3-(4-cyanobenzoyl)-7-methoxyindolizine-1,2-dicarboxylate (5a) emerged as a promising COX-2 inhibitor with an IC50 of 5.84 µM, as compared to indomethacin (IC50 = 6.84 µM). The molecular modeling study of indolizines indicated that hydrophobic interactions were the major contribution to COX-2 inhibition. The title compound diethyl 3-(4-bromobenzoyl)-7-methoxyindolizine-1,2-dicarboxylate (5c) was subjected for single-crystal X-ray studies, Hirshfeld surface analysis, and energy framework calculations. The X-ray diffraction analysis showed that the molecule (5c) crystallizes in the monoclinic crystal system with space group P 21/n with a = 12.0497(6)Å, b = 17.8324(10)Å, c = 19.6052(11)Å, α = 90.000°, β = 100.372(1)°, γ = 90.000°, and V = 4143.8(4)Å3. In addition, with the help of Crystal Explorer software program using the B3LYP/6-31G(d, p) basis set, the theoretical calculation of the interaction and graphical representation of energy value was measured in the form of the energy framework in terms of coulombic, dispersion, and total energy
