    Unraveling the mysteries of moyamoya: a rare case report of stroke in young

    Moyamoya disease is a unique cerebrovascular disease that is characterized by chronic progressive stenosis of distal part of internal carotid artery with consequent development of a network of collateral vessels in response to brain ischemia. It is mainly seen in individuals of Asian descent and is the most common cause of stroke in Asian children. However, it is rare in Indian subcontinent. Here we report a case of young adult who manifested with moyamoya disease, evident from acute onset ischemic stroke. The patient underwent a diagnostic cerebral angiogram that showed bilateral posterior cerebral artery stenosis with pathognomonic collateral moyamoya vessels. The patient subsequently underwent elective surgical intervention procedures to prevent further complications

    An intervention to improve the knowledge of anganwadi workers pertaining to growth monitoring in rural areas of Varanasi district, India

    Background: Growth monitoring is one of the important functions of anganwadi workers (AWWs), for which they should have sufficient knowledge and training. With this background it was hypothesized that educational intervention can make a difference in the level of knowledge of AWWs in regard to growth monitoring. The main objective of the study was to evaluate the changes in knowledge of Anganwadi workers regarding growth monitoring through knowledge up-gradation training.Methodology: It was a field based interventional study, which was conducted on 66 AWWs each from Chiraigaon (intervention) and Cholapur (control) blocks of Varanasi district. The primary tool in this study was a pre-designed and pretested interview schedule for collecting information’s pertaining to study objective.Results: The mean score of knowledge was around 19.7 (max. Score was 30) in both the blocks during baseline survey and almost all the AWWs were having either average or good knowledge of growth monitoring. Post intervention a significant (p<0.01) difference has been observed between the mean knowledge scores of AWWs in intervention (25.32±2.44) and control (20.35±2.70) blocks and as much as 48.5% of AWWs were having excellent knowledge about growth monitoring in Chiraigaon block (p<0.01), while the situation was unchanged in Cholapur block.Conclusion: The knowledge up-gradation was found effective in improving the knowledge status of AWWs with regard to growth monitoring.

    Thermo-acoustics and its detection in a premixed flame

    A new optical technique based on light-matter interaction is devised in-house to detect thermo-acoustic disturbances generated after ignition and during propagation of a premixed flame front in a half open channel. The technique involves passing a polarized laser light through a medium whose density or refractive index varies due to the passage of acoustic waves and/or flame front and then capturing the leaked depolarised light through an analyser by a photo-detector. The technique is applied to combustor involving premixed flame propagation and tulip inversion. The thermo-acoustic signals and the flame front are distinguished by comparing the oscilloscope signal with high speed photography of the flow-field. Acoustic waves are found to intercept the flame propagation at various axial locations and time instants

    Evans syndrome (ES) is characterized by the simultaneous or consecutive occurrence of warm autoimmune hemolytic anemia (AIHA) along with immune thrombocytopenia (ITP), and less commonly, autoimmune neutropenia. It may manifest spontaneously or as a result of autoimmune, malignancy or lymphoproliferative disease. Clinical manifestations may be associated with hemolysis and thrombocytopenia, potentially leading to life-threatening outcomes. ES is a rare diagnosis of exclusion. Due to its infrequency, the treatment is typically empirical, relying largely on intravenous corticosteroids or immunoglobulins. We are presenting case of a 46-year-old- female with bleeding from the mouth and gums and rashes all over the body with no prior diagnosis of rheumatological disorder. This case is pivotal as it highlights a key factor contributing to ES and presents a pragmatic method for addressing the condition

    Dense plasma heating by crossing relativistic electron beams

    Here we investigate, using relativistic fluid theory and Vlasov-Maxwell simulations, the local heating of a dense plasma by two crossing electron beams. Heating occurs as an instability of the electron beams drives Langmuir waves, which couple nonlinearly into damped ion-acoustic waves. Simulations show a factor 2.8 increase in electron kinetic energy with a coupling efficiency of 18%. Our results support applications to the production of warm dense matter and as a driver for inertial fusion plasmas

    Variations in angiotensin-converting enzyme gene insertion/deletion polymorphism in Indian populations of different ethnic origins

    The pattern of angiotensin-converting enzyme (ACE) gene insertion/deletion (I/D) polymorphism in the Indian population is poorly known. In order to determine the status of the polymorphism, young unrelated male army recruits were screened. The population had cultural and linguistic differences and lived in an environment that varied significantly from one region to another. Analysis of the genotype, showed higher frequency of the insertion allele in four of the five groups i.e. I allele frequency was significantly higher (P&lt;005) in Dogras, Assamese and Kumaonese. The deletion allele frequency was comparatively higher in the fifth group that belonged to Punjab. A correlation was observed between the genotype and enzyme activity. Involvement of a single D allele in the genotype enhanced the activity up to 37.56 Âą 313%. The results suggested ethnic heterogeneity with a significant gene cline with higher insertion allele frequency. Such population-based data on various polymorphisms can ultimately be exploited in pharmacogenomics

    Motion limitations of non-contact photoplethysmography due to the optical and topological properties of skin

    Non-contact photoplethysmography (PPG) provides multiple benefits over in-contact methods, but is not as tolerant to motion due to the lack of mechanical coupling between the subject and sensor. One limitation of non-contact photoplethysmography is discussed here, specifically looking at the topology and optical variations of the skin and how this impacts upon the ability to extract a photoplethysmogram when a subject moves horizontally across the field of view of the detector (a panning motion). When this occurs it is shown that whilst the general relationships between the speed of traversal, detection area and resultant signal quality can be found, the quality of signal in each individual case is determined by the properties of the area of skin chosen

    SummarySmall molecules inhibiting hypoxia inducible factor (HIF) prolyl hydroxylases (PHDs) are the focus of drug development efforts directed toward the treatment of ischemia and metabolic imbalance. A cell-based reporter produced by fusing HIF-1α oxygen degradable domain (ODD) to luciferase was shown to work as a capture assay monitoring stability of the overexpressed luciferase-labeled HIF PHD substrate under conditions more physiological than in vitro test tubes. High throughput screening identified novel catechol and oxyquinoline pharmacophores with a “branching motif” immediately adjacent to a Fe-binding motif that fits selectively into the HIF PHD active site in in silico models. In accord with their structure-activity relationship in the primary screen, the best “hits” stabilize HIF1α, upregulate known HIF target genes in a human neuronal line, and exert neuroprotective effects in established model of oxidative stress in cortical neurons
