    Photodynamic therapy of early stage oral cavity and oropharynx neoplasms: an outcome analysis of 170 patients

    The indications of photodynamic therapy (PDT) of oral cavity and oropharynx neoplasms are not well defined. The main reason is that the success rates are not well established. The current paper analyzes our institutional experience of early stage oral cavity and oropharynx neoplasms (Tis-T2) to identify the success rates for each subgroup according to T stage, primary or non-primary treatment and subsites. In total, 170 patients with 226 lesions are treated with PDT. From these lesions, 95 are primary neoplasms, 131 were non-primaries (recurrences and multiple primaries). The overall response rate is 90.7% with a complete response rate of 70.8%. Subgroup analysis identified oral tongue, floor of mouth sites with more favorable outcome. PDT has more favorable results with certain subsites and with previously untreated lesions. However, PDT can find its place for treating lesions in previously treated areas with acceptable results

    APEX: Current Status of the Airborne Dispersive Pushbroom Imaging Spectrometer

    ABSTRACT Over the past few years, a joint Swiss/Belgium ESA initiative resulted in a project to build a precursor mission of future spaceborne imaging spectrometers, namely APEX (Airborne Prism Experiment). APEX is designed to be an airborne dispersive pushbroom imaging spectrometer operating in the solar reflected wavelength range between 400 and 2500 nm. The system is optimized for land applications including limnology, snow, and soil, amongst others. The instrument is optimized with various steps taken to allow for absolute calibrated radiance measurements. This includes the use of a pre-and post-data acquisition internal calibration facility as well as a laboratory calibration and a performance model serving as a stable reference. The instrument is currently in its breadboarding phase, including some new results with respect to detector development and design optimization for imaging spectrometers. In the same APEX framework, a complete processing and archiving facility (PAF) is developed. The PAF not only includes imaging spectrometer data processing up to physical units, but also geometric and atmospheric correction for each scene, as well as calibration data input. The PAF software includes an Internet based web-server and provides interfaces to data users as well as instrument operators and programmers. The software design, the tools and its life cycle are discussed as well

    This short communication outlines an experiment where tokens, associated with application oriented IP streams, authorize access to an optical lightpath during iGrid 2005. The experiment showed the practical viability of this mechanism to perform access control and resource management within optical networks. The experiment was conducted in collaboration with the Virtual Machine Turntable (NL-103) experiment, which used the mechanism to obtain access to continental and transatlantic optical network segments that connected Virtual Machine sites

    Seamless live migration of virtual machines over the MAN/WAN

    The “VM Turntable” demonstrator at iGRID 2005 pioneered the integration of Virtual Machines (VMs) with deterministic “lightpath” network services across a MAN/WAN. The results provide for a new stage of virtualization—one for which computation is no longer localized within a data center but rather can be migrated across geographical distances, with negligible downtime, transparently to running applications and external clients. A noteworthy data point indicates that a live VM was migrated between Amsterdam, NL and San Diego, USA with just 1–2 s of application downtime. When compared to intra-LAN local migrations, downtime is only about 5–10 times greater despite 1000 times higher round-trip times

    Item does not contain fulltextBACKGROUND: Numerous patients will develop recurrent disease after esophagectomy for esophageal carcinoma (EC). In literature, survival after recurrent EC is poor with 6-8 months. In these studies, diagnostic imaging during follow-up (FU) is routinely performed. In the Netherlands, routine imaging is not part of FU and only performed on indication. The aim of this study was to determine survival after diagnosis of recurrent disease in patients after esophagectomy without routine imaging during FU. METHODS: All EC patients who underwent esophagectomy between 1993 and 2010 were included and followed for clinical evidence of recurrent EC. Location, symptoms, diagnosis, and treatment of recurrent disease were registered. Pattern of recurrence was compared between patients who underwent neoadjuvant therapy and patients who underwent surgery alone. Survival after detection of recurrence was determined in all patients and related to the year of surgery. RESULTS: A total of 493 of 1,088 patients (45 %) who underwent esophagectomy between 1993 and 2010 developed recurrent disease. Median interval between esophagectomy and recurrence was 10.5 months. Within the first 2 years after surgery, 33 % of patients developed recurrent EC. The majority of patients (51 %) were diagnosed with distant metastases. Locoregional recurrence occurred significantly less often among patients who underwent neoadjuvant therapy (6 vs 16 %, p = .017). Median survival after diagnosis of recurrent disease was 3 months. No relation was observed between the year of surgery and survival after recurrent disease (p = .931). CONCLUSIONS: Survival after recurrent EC in patients who undergo FU without routine imaging after esophagectomy is approximately 3 months and has not improved over the past 18 years.1 augustus 201
