    The creativity of language Chomsky puts so much importance on must be questioned because the same creativity has produced lethal weapons such as atomic bombs. Modern science developed by the power of language has certainly produced many beneficial things, but we should not overlook its destructive side. Besides, language capable of inventing a new reality leads us to believe in it blindly. Let us remember philosophers such as Wittgenstein or Nagarjuna who warned us not to believe in the construct called “reality” made up by language power.Now, is it better and safer then to use a metaphorical language that composes poetry than a conceptual one that composes science and mathematics? Not necessarily, for so long as metaphors evoke a sense of reality in us, it can be as dangerous as the conceptual one. The problem today is that the conceptual and logical language that supports science and technology is predominant over the metaphorical and poetical one. Even if Rousseau, Adorno and Horkheimer alarmed us of the danger, our history has advanced ignoring their alarms. Science and technology are making progress day by day, and no one is capable of checking their progress because no one sees what is really happening in there.The wall between the “Two Cultures” is still there, but let us not forget the efforts of poetic scientists and scientific poets such as Kenji Miyazawa or Claude Lévi-Strauss, who try to diminish the gap. They surely help us to find a way out of the danger

    Effect of Electrical Stimulation on Quadriceps Muscle Strengthening Following Reconstructive Surgery of the Anterior Cruciate Ligament

    The purpose of this study was to compare the effectiveness of both electrical stimulation and isometric exercise with that of isometric exercise alone on the quadriceps muscle in 30 patients who had undergone anterior cruciate ligament (ACL) reconstruction. The 30 patients were divided into two groups. The electrical stimulation (ES) group (N=18) carried out isometric contraction with ES. The non-ES group (N=12) performed isometric contraction only. The patients were examined clinically in terms of thigh circumference and time from surgery to disappearance of extension lag and giving way. The ES group had better muscle function within these clinical parameters. This study indicated that ES is a useful technique for muscle strengthening after ACL reconstruction

    Profile of rhythmic gene expression in the livers of obese diabetic KK-Ay mice

    金沢大学大学院医学系研究科環境社会医学Although a number of genes expressed in most tissues, including the liver, exhibit circadian regulation, gene expression profiles are usually examined only at one scheduled time each day. In this study, we investigated the effects of obese diabetes on the hepatic mRNA levels of various genes at 6-h intervals over a single 24-h period. Microarray analysis revealed that many genes are expressed rhythmically, not only in control KK mice but also in obese diabetic KK-Ay mice. Real-time quantitative PCR verified that 19 of 23 putative circadianly expressed genes showed significant 24-h rhythmicity in both strains. However, obese diabetes attenuated these expression rhythms in 10 of 19 genes. More importantly, the effects of obese diabetes were observed throughout the day in only two genes. These results suggest that observation time influences the results of gene expression analyses of genes expressed circadianly. © 2006 Elsevier Inc. All rights reserved

    Primary placement technique of jejunostomy using the entristar™ skin-level gastrostomy tube in patients with esophageal cancer

    <p>Abstract</p> <p>Background</p> <p>We developed a skin-level jejunostomy tube (SLJT) procedure for patients undergoing esophagectomy using a skin-level gastrostomy tube (G-tube) (Entristar™; Tyco Healthcare, Mansfield, Mass), in order to improve their nutrition status and quality of life (QOL). We describe the procedure and the adverse effects of SLJT in patients with esophageal cancer (EC).</p> <p>Methods</p> <p>Over a 24-month period (March 2008 to March 2010), there were 16 patients (mean age: 61.8 years; age range: 49-75 years; 15 men, 1 woman) who had Stage II or III EC. Primary jejunostomy was performed under general anesthesia during esophagectomy. The technical success and the immediate and delayed complications of the procedure were recorded.</p> <p>Jejunostomy techniques</p> <p>SLJT placement using the G-tube (20Fr) was performed 20 cm from the Treitz ligament on the side opposing the jejunal mesenterium. The internal retention bolster was exteriorized through an incision in the abdominal wall. A single purse string suture using a 4-0 absorbable suture was performed. The internal retention bolster was then inserted into the jejunal lumen via the small incision. The intestine adjacent to the tube was anchored to the peritoneum using a single stitch.</p> <p>Results</p> <p>The SLJT was successfully inserted in all 16 patients. No early complications were documented. Follow-up for a median of 107 days (range, 26-320 days) revealed leakage to the skin in four patients, including superficial wound infections in two patients. There were no cases of obstruction of the tube or procedure-related death.</p> <p>Conclusions</p> <p>This SLJT placement technique using the G-tube is a safe procedure in patients with EC and allows the creation of a long-term feeding jejunostomy.</p

    Plaque REgression with Cholesterol absorption Inhibitor or Synthesis inhibitor Evaluated by IntraVascular UltraSound (PRECISE-IVUS Trial): Study protocol for a randomized controlled trial

    AbstractBackgroundAlthough the positive association between achieved low-density lipoprotein cholesterol (LDL-C) level and the risk of coronary artery disease (CAD) has been confirmed by randomized studies with statins, many patients remain at high residual risk of events suggesting the necessity of novel pharmacologic strategies. The combination of ezetimibe/statin produces greater reductions in LDL-C compared to statin monotherapy.PurposeThe Plaque REgression with Cholesterol absorption Inhibitor or Synthesis inhibitor Evaluated by IntraVascular UltraSound (PRECISE-IVUS) trial was aimed at evaluating the effects of ezetimibe addition to atorvastatin, compared with atorvastatin monotherapy, on coronary plaque regression and change in lipid profile in patients with CAD.MethodsThe study is a prospective, randomized, controlled, multicenter study. The eligible patients undergoing IVUS-guided percutaneous coronary intervention will be randomly assigned to receive either atorvastatin alone or atorvastatin plus ezetimibe (10mg) daily using a web-based randomization software. The dosage of atorvastatin will be increased by titration within the usual dose range with a treatment goal of lowering LDL-C below 70mg/dL based on consecutive measures of LDL-C at follow-up visits. IVUS will be performed at baseline and 9–12 months follow-up time point at participating cardiovascular centers. The primary endpoint will be the nominal change in percent coronary atheroma volume measured by volumetric IVUS analysis.ConclusionPRECISE-IVUS will assess whether the efficacy of combination of ezetimibe/atorvastatin is noninferior to atorvastatin monotherapy for coronary plaque reduction, and will translate into increased clinical benefit of dual lipid-lowering strategy in a Japanese population
