Submergence sensitivity of durum wheat, bread wheat and barley at the germination stage
Soil waterlogging at initial growth stages can cause heavy yield losses of winter cereals. Therefore, the screening for submergence tolerance traits in seeds of commercial varieties is of high concern worldwide. Ten Italian varieties of durum wheat (Triticum durum Desf.), bread wheat (T. aestivum L.) and barley (Hordeum vulgare L.) were investigated for their ability to germinate in submerged conditions and to recover after submergence periods of three to 15 days. Submergence prevented germination and decreased germinability, at rates that increased with duration of submergence. Sensitivity ranked in the order: barley >durum wheat >bread wheat. We related the higher sensitivity of barley to its slower germination and slightly higher leakage of electrolytes, whereas the percentage of abnormal seedlings was lower than in other species. It was less than 4%, compared to less than 15 and 8% in durum wheat and bread wheat, respectively. Wide varietal differences were found in all species. According to variety, after 6-day submergence, germinability ranged from 2 to 42% in barley, from 5 to 80% in durum wheat, and from 30 to 77% in bread wheat. Varieties with more than 40% seed survival were three, six and seven per species, in the same order. The differential submergence sensitivity of varieties indicates a potential to select for waterlogging tolerance within Italian genotypes of winter cereal crops
Central precocious puberty in a girl with LEGIUS syndrome: an accidental association?
BACKGROUND: Central precocious puberty is a condition characterized by precocious activation of the hypothalamic-pituitary-gonadal axis. It may be idiopathic or secondary to organic causes, including syndromes such as Neurofibromatosis type 1 (NF1).CASE PRESENTATION: We presented a girl of 6years and 10months with almost 11 cafe-au-lait skin macules, without other clinical or radiological signs typical of NF1, and with a central precocious puberty. Genetic analysis evidenced the new variant NM-152594.2:c.304delAp. (Thr102Argfs*19) in SPRED1 gene, which allowed to diagnose Legius syndrome.CONCLUSIONS: We report for the first time a case of central precocious puberty in a girl with Legius syndrome. The presence of central precocious puberty in a child with characteristic cafe-au-lait macules should suggest pediatricians to perform genetic analysis in order to reach a definitive diagnosis. Further studies on timing of puberty in patients with RASopathies are needed to better elucidate if this clinical association is casual or secondary to their clinical condition
Estratégia como prática em uma organização hospitalar

Assim, teve como objetivo analisar o processo de formação/formulação da estratégia na prática do hospital filantrópico pesquisado, para tanto, identificou-se os atores envolvidos, o modo como as práticas organizacionais interferem na formação de estratégias e as práxis relacionadas. Também foi estudado se a postura da instituição, frente a seu meio, tende a ser voluntarista ou determinista. Configura-se como uma pesquisa cientÃfica aplicada, caracterizada como um estudo de caso; a análise de dados foi qualitativa e pautada na análise crÃtica do discurso. Os resultados obtidos evidenciam que as estratégias da instituição não são dissociadas das práticas organizacionais, mas, ao contrário, exercem influências recÃprocas. Isso propicia uma visão sistêmica, foco para ações e resulta no alcance dos objetivos estipulados. O setor de saúde possui inúmeras exigências, deve englobar inovações constantemente e suprir uma grande demanda de serviços, o que inflama a necessidade de possuir uma postura estratégica.The monograph is about strategic management in hospitals, its development was founded in the approach of the Strategy as Practice. Therefore, its objective was to analyze the process of formation/formulation of strategy in the practice of the philanthropic hospital researched, for this purpose, the players involved, how organizational practices interfere in the formation of strategies and the related praxis were identified. It studied, also, if the position of the institution, in relation to its environment, tends to be voluntarist or deterministic. It‟s an applied scientific research, characterized as a case study; the data analysis was qualitative and guided by the critical discourse analysis. The results show that the strategies of the institution are not separated from its organizational practices but exert mutual influences. This provides a systemic view, focus on actions and allows reach the stipulated goals. The health care industry has many requirements, must encompass innovations constantly and attend a great demand for services, which inflames the necessity of have a strategic posture

The Differential Diagnosis of Discrepant Thyroid Function Tests: Insistent Pitfalls and Updated Flow-Chart Based on a Long-Standing Experience.
Background: Discrepant thyroid function tests (TFTs) are typical of inappropriate secretion of TSH (IST), a rare entity encompassing TSH-secreting adenomas (TSHoma) and Resistance to Thyroid Hormone (RTHβ) due to THRB mutations. The differential diagnosis remains a clinical challenge in most of the cases. The objective of this study was to share our experience with patients presenting with discrepant TFTs outlining the main pitfalls in the differential diagnosis. Methods: medical records of 100 subjects with discrepant TFTs referred to Thyroid Endocrine Centers at the University of Milan were analyzed, retrospectively. Patients were studied by dynamic testing (TRH test, T3-suppression test, or a short course of long-acting somatostatin analog, when appropriate), THRB sequencing, and pituitary imaging. Results: 88 patients were correctly diagnosed as RTHβ with (n = 59; 16 men, 43 women) or without THRB variants (n = 6; 2 men, 4 female) or TSHoma (n = 23; 9 men, 14 women). We identified 14 representative subjects with an atypical presentation or who were misdiagnosed. Seven patients, with spurious hyperthyroxinemia due to assays interference were erroneously classified as RTHβ (n = 4) or TSHoma (n = 3). Three patients with genuine TSHomas were classified as laboratory artifact (n = 2) or RTHβ (n = 1). Two TSHomas presented atypically due to coexistent primary thyroid diseases. In one RTHβ a drug-induced thyroid dysfunction was primarily assumed. These patients experienced a mean diagnostic delay of 26 ± 14 months. Analysis of the investigations which can differentiate between TSHoma and RTHβ showed highest accuracy for the T3-suppression test (100% specificity with a cut-off of TSH <0.11 μUI/ml). Pituitary MRI was negative in 6/26 TSHomas, while 11/45 RTHβ patients had small pituitary lesions, leading to unnecessary surgery in one case. Conclusions: Diagnostic delay and inappropriate treatments still occur in too many cases with discrepant TFTs suggestive of central hyperthyroidism. The insistent pitfalls lead to a significant waste of resources. We propose a revised flow-chart for the differential diagnosis
The Role of Bronchoalveolar Lavage in Systemic Sclerosis Interstitial Lung Disease: A Systematic Literature Review
The role of Bronchoalveolar Lavage (BAL) in the evaluation of systemic sclerosis (SSc) interstitial lung disease (ILD) is still controversial. The aim of this systematic literature review was to investigate the use of BAL in SSc-ILD, and to focus on the pros and cons of its real-life application. Methods: PubMed, Cochrane, and Embase were questioned from inception until 31 December 2021. Results: Eighteen papers were finally analyzed. A positive correlation was observed between lung function and BAL cytology; in particular, BAL neutrophilia/granulocytosis was related to lower diffusing capacity for carbon monoxide (DLCO) values and lower forced vital capacity (FVC). Moreover, a positive correlation between BAL cellularity and high-resolution computed tomography (HRCT) findings has been reported by several authors. Cytokines, chemokines, growth factors, coagulation factors, and eicosanoids have all been shown to be present, more often and in higher quantities in SSc-ILD patients than in the health control and, in some cases, they were related to more severe pulmonary disease. Presenza dei genitori e vittimizzazione dei giovani in Italia: risultati dell’International Self-report Delinquency Study-3
Il presente studio si inserisce all’interno di un più ampio progetto di ricerca internazionale sulla devianza minorile, International Self-Report Delinquency Study-3 (ISRD-3), e si propone di fornire una maggiore conoscenza del legame esistente tra la presenza/assenza genitoriale, il supporto emotivo parentale percepito dai giovani e la vittimizzazione giovanile. I dati elaborati rientrano tra quelli raccolti attraverso la somministrazione del questionario ISRD-3 ad un campione nazionale di 3.508 studenti italiani delle classi seconde e terze della scuola media inferiore e delle classi prime della scuola media superiore. I risultati mostrano come gli adolescenti con maggiore rischio di vittimizzazione siano coloro che vivono in famiglie mono-genitoriali o con caregivers diversi dai genitori (nonni, zii, altri parenti ecc.). Inoltre, ponendo l’attenzione alla percezione che l’adolescente ha del supporto emotivo parentale, dai risultati appare come più scarsa è tale percezione maggiore è la vitti-mizzazione subìta. La ricerca suggerisce che le strategie di intervento sul fenomeno della vittimizzazione giovanile dovrebbero coinvolgere tutti i membri della famiglia in cui il giovane è inserito e avere come obiettivo principale il miglioramento della qualità delle relazioni
The CUORE Cryostat: A 1-Ton Scale Setup for Bolometric Detectors
The cryogenic underground observatory for rare events (CUORE) is a 1-ton
scale bolometric experiment whose detector consists of an array of 988 TeO2
crystals arranged in a cylindrical compact structure of 19 towers. This will be
the largest bolometric mass ever operated. The experiment will work at a
temperature around or below 10 mK. CUORE cryostat consists of a cryogen-free
system based on pulse tubes and a custom high power dilution refrigerator,
designed to match these specifications. The cryostat has been commissioned in
2014 at the Gran Sasso National Laboratories and reached a record temperature
of 6 mK on a cubic meter scale. In this paper, we present results of CUORE
commissioning runs. Details on the thermal characteristics and cryogenic
