Stacking disorder in silicon carbide supported cobalt crystallites : an X-ray diffraction, electron diffraction and high resolution electron microscopy study
Supported cobalt Fischer–Tropsch catalysts are characteristically nanoparticulate and the
reduced SiC supported catalyst was found to contain both HCP and FCC polymorphs. This is
reflected in the powder XRD patterns and generally there is a poor fit between the
experimental and calculated diffractograms. This was ascribed to small crystallite sizes and
the occurrence of disorder, manifested as peak broadening and peak shifts. Selected area
electron diffraction data of suitably oriented cobalt catalyst grains on silicon carbide supports
show non-periodic disorder in the zone axis orientations that contain the common (001)
(HCP) and (111) (FCC) reciprocal lattice planes. Both FCC and HCP polymorphs are present
in the same grains and these show disorder mainly in the HCP component. The disorder is
further examined using high angle annular dark field (HAADF) scanning transmission
electron microscopy at atomic resolution and the stacking sequences elucidated. Random
sequences of mainly FCC are interrupted by HCP sequences and twin surfaces with reverse
stacking sequences are also present. This study highlights the presence of significant disorder

Human papillomavirus and carcinoma of the mucosal surfaces of the head and neck
Human papillomavirus induced cervical cancer is the fourth most prevalent malignancy affecting females globally. Over the past two decades scientific information unveiled an increasing role for the virus in the pathogenesis of malignancies developing from the mucosal surfaces of the oropharynx. It is feasible to postulate that we may be in the beginning of a global pandemic of oropharyngeal cancer if the mode of transmission of the virus is taken into account. Comparison of NITAG policies and working processes in selected developed countries
BACKGROUND: Vaccines are specific medicines characterized by two country-specific market access processes: (1) a recommendation by National Immunization Technical Advisory Group (NITAG), and (2) a funding policy decision. OBJECTIVES: The objective of this study was to compare and analyze NITAGs of 13 developed countries by describing vaccination committees' bodies and working processes. METHODS: Information about NITAGs bodies and working processes was searched from official sources from June 2011 to November 2012. Retrieved information was completed from relevant articles identified through a systematic literature review and by information provided by direct contact with NITAGs or parent organizations. An expert panel was also conducted to discuss, validate, and provide additional input on obtained results. RESULTS: While complete information, defined as 100%, was retrieved only for the UK, at least 80% of data was retrieved for 9 countries out of the 13 selected countries. Terms of references were identified in 7 countries, and the main mission for all NITAGs was to provide advice for National immunization programs. However, these terms of references did not fully encompass all the actual missions of the NITAGs. Decision analysis frameworks were identified for 10 out of the 13, and all NITAGs considered at least four criteria for decision-making: disease burden, efficacy/effectiveness, safety and cost-effectiveness. Advices were published by most NITAGs, but few NITAGs published meeting agendas and minutes. Only the United States had open meetings. CONCLUSIONS: This study supports previous findings about the disparities in NITAGs processes which could potentially explain the disparity in access to vaccinations and immunization programs across Europe. With NITAGs recommendations being used by policy decision makers for implementation and funding of vaccine programs, guidances should be well-informed and transparent to ensure National Immunization Programs' (NIP) credibility among the public and health care professionals

Parasite co-infections show synergistic and antagonistic interactions on growth performance of East African zebu cattle under one year
The co-occurrence of different pathogen species and their simultaneous infection of hosts are common, and may affect host
health outcomes. Co-infecting pathogens may interact synergistically (harming the host more) or antagonistically (harming
the host less) compared with single infections. Here we have tested associations of infections and their co-infections with
variation in growth rate using a subset of 455 animals of the Infectious Diseases of East Africa Livestock (IDEAL) cohort
study surviving to one year. Data on live body weight, infections with helminth parasites and haemoparasites were collected
every 5 weeks during the first year of life. Growth of zebu cattle during the first year of life was best described by a linear
growth function. A large variation in daily weight gain with a range of 0·03–0·34 kg, and a mean of 0·135 kg (0·124, 0·146;
95% CI) was observed. After controlling for other significant covariates in mixed effects statistical models, the results
revealed synergistic interactions (lower growth rates) with Theileria parva and Anaplasma marginale co-infections, and
antagonistic interactions (relatively higher growth rates) with T. parva and Theileria mutans co-infections, compared with
infections with T. parva only. Additionally, helminth infections can have a strong negative effect on the growth rates but this
is burden-dependent, accounting for up to 30% decrease in growth rate in heavily infected animals. These findings present
evidence of pathogen–pathogen interactions affecting host growth, and we discuss possible mechanisms that may explain
observed directions of interactions as well as possible modifications to disease control strategies when co-infections are

International Survey on Mechanical Ventilation During Extracorporeal Membrane Oxygenation
The optimal ventilation strategy for patients on extracorporeal membrane oxygenation (ECMO) remains uncertain. This survey reports current mechanical ventilation strategies adopted by ECMO centers worldwide. An international, multicenter, cross-sectional survey was conducted anonymously through an internet-based tool. Participants from North America, Europe, Asia, and Oceania were recruited from the extracorporeal life support organization (ELSO) directory. Responses were received from 48 adult ECMO centers (response rate 10.6%). Half of these had dedicated ventilation protocols for ECMO support. Pressure-controlled ventilation was the preferred initial ventilation mode for both venovenous ECMO (VV-ECMO) (60%) and venoarterial ECMO (VA-ECMO) (34%). In VV-ECMO, the primary goal was lung rest (93%), with rescue therapies commonly employed, especially neuromuscular blockade (93%) and prone positioning (74%). Spontaneous ventilation was typically introduced after signs of pulmonary recovery, with few centers using it as the initial mode (7%). A quarter of centers stopped sedation within 3 days after ECMO initiation. Ventilation strategies during VA-ECMO focused less on lung-protective goals and transitioned to spontaneous ventilation earlier. Ventilation strategies during ECMO support differ considerably. Controlled ventilation is predominantly used initially to provide lung rest, often facilitated by sedation and neuromuscular blockade. Few centers apply "awake ECMO" early during ECMO support, some utilizing partial neuromuscular blockade.</p

Effect of neoadjuvant chemoradiotherapy on health-Related quality of life in esophageal or junctional cancer: Results from the randomized CROSS trial
Purpose To compare pre-agreed health-related quality of life (HRQOL) domains in patients with esophageal or junctional cancer who received neoadjuvant chemoradiotherapy (nCRT) followed by surgery or surgery alone. Secondary aims were to examine the effect of nCRT on HRQOL before surgery and the effect of surgery on HRQOL. Patients and Methods Patients were randomly assigned to nCRT (carboplatin plus paclitaxel with concurrent 41.4-Gy radiotherapy) followed by surgery or surgery alone. HRQOL was measured using the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire–Core 30 (QLQ-C30) and –Oesophageal Cancer Module (QLQ-OES24) questionnaires pretreatment and at 3, 6, 9, and 12 months postoperatively. The nCRT group also received preoperative questionnaires. Physical functioning (PF; QLQ-C30) and eating problems (EA; QLQ-OES24) were chosen as predefined primary end points. Predefined secondary end points were global QOL (GQOL; QLQ-C30), fatigue (FA; QLQ-C30), and emotional problems (EM; QLQ-OES24). Results A total of 363 patients were analyzed. No statistically significant differences in postoperative HRQOL were found between treatment groups. In the nCRT group, PF, EA, GQOL, FA, and EM scores deteriorated 1 week after nCRT (Cohen’s d: 20.93, P, .001; 0.47, P, .001; 20.84, P, .001; 1.45, P, .001; and 0.32, P = .001, respectively). In both treatment groups, all end points declined 3 months postoperatively compared with baseline (Cohen’s d: 21.00, 0.33, 20.47, 20.34, and 0.33, respectively; all P, .001), followed by a continuous gradual improvement. EA, GQOL, and EM were restored to baseline levels during follow-up, whereas PF and FA remained impaired 1 year postoperatively (Cohen’s d: 0.52 and 20.53, respectively; both P, .001). Conclusion Although HRQOL declined during nCRT, no effect of nCRT was apparent on postoperative HRQOL compared with surgery alone. In addition to the improvement in survival, these findings support the view that nCRT according to the Chemoradiotherapy for Esophageal Cancer Followed by Surgery Study–regimen can be regarded as a standard of care
Parasite co-infections and their impact on survival of indigenous cattle
In natural populations, individuals may be infected with multiple distinct pathogens at a time. These pathogens may act
independently or interact with each other and the host through various mechanisms, with resultant varying outcomes on
host health and survival. To study effects of pathogens and their interactions on host survival, we followed 548 zebu cattle
during their first year of life, determining their infection and clinical status every 5 weeks. Using a combination of clinical
signs observed before death, laboratory diagnostic test results, gross-lesions on post-mortem examination, histo-pathology
results and survival analysis statistical techniques, cause-specific aetiology for each death case were determined, and effect
of co-infections in observed mortality patterns. East Coast fever (ECF) caused by protozoan parasite Theileria parva and
haemonchosis were the most important diseases associated with calf mortality, together accounting for over half (52%) of
all deaths due to infectious diseases. Co-infection with Trypanosoma species increased the hazard for ECF death by 6 times
(1.4–25; 95% CI). In addition, the hazard for ECF death was increased in the presence of Strongyle eggs, and this was burden
dependent. An increase by 1000 Strongyle eggs per gram of faeces count was associated with a 1.5 times (1.4–1.6; 95% CI)
increase in the hazard for ECF mortality. Deaths due to haemonchosis were burden dependent, with a 70% increase in
hazard for death for every increase in strongyle eggs per gram count of 1000. These findings have important implications
for disease control strategies, suggesting a need to consider co-infections in epidemiological studies as opposed to singlepathogen

The Future of Psychopharmacological Enhancements: Expectations and Policies
The hopes and fears expressed in the debate on human enhancement are not always based on a realistic assessment of the expected possibilities. Discussions about extreme scenarios may at times obscure the ethical and policy issues that are relevant today. This paper aims to contribute to an adequate and ethically sound societal response to actual current developments. After a brief outline of the ethical debate concerning neuro-enhancement, it describes the current state of the art in psychopharmacological science and current uses of psychopharmacological enhancement, as well as the prospects for the near future. It then identifies ethical issues regarding psychopharmacological enhancements that require attention from policymakers, both on the professional and on the governmental level. These concern enhancement research, the gradual expansion of medical categories, off-label prescription and responsibility of doctors, and accessibility of enhancers on the Internet. It is concluded that further discussion on the advantages and drawbacks of enhancers on a collective social level is still needed
