Avaliação da qualidade de vida em idosos submetidos ao tratamento hemodialítico
A hemodiálise afeta não só aspectos físicos, como psicológicos e sociais, com repercussão na vida pessoal e familiar.Considerando que no Brasil está ocorrendo um aumento da população idosa, esta pesquisa objetivou avaliar aqualidade de vida dos idosos com insuficiência renal crônica, submetidos ao tratamento hemodialítico. Trata-se deuma pesquisa quantitativa, de caráter descritivo exploratório, com idosos de um serviço especializado em hemodiálisede Guarapuava, Paraná. Os dados foram coletados entre maio a junho de 2010, através de um instrumentoestruturado e, após, submetidos ao programa estatístico Statistica 7.1. Analisando os domínios do questionário, omaior escore diz respeito ao domínio social (70,42) e o menor, ao domínio físico (49,37). Assim, a qualidade de vidadesses idosos apresentou-se baixa, com variações de acordo com o domínio analisado. Pesquisas voltadas à avaliaçãode qualidade de vida são relevantes e instrumentalizam a prática diária do cuidado

Comparison of Treatment Goals between Users of Biological and Non-Biological Therapies for Treatment of Psoriasis in Japan
Background: Previously, our cross-sectional observational study in Japan revealed high (68%) discordance within treatment goals between psoriasis patients and their physicians. Objective: This secondary analysis aimed to determine whether patient and physician users of biologics have higher treatment goals than users of non-biologics. Methods: A survey for both patients and physicians on background characteristics, disease severity, treatment goals, treatment satisfaction, and health-related quality of life was conducted at 54 sites. Association between treatment goals and biologic/non-biologic users was assessed using ordinal logistic regression models. Results: In total, 449 patient-physician pairs agreed to participate; 425 completed the survey and were analyzed. More biologic users than non-biologic users reported complete clearance (Psoriasis Area and Severity Index 100) as a treatment goal (patient-reported: 23.6% vs. 16.1%; physician-reported: 26.9% vs. 2.2%). Biologic users were significantly associated with higher treatment goals than non-biologic users (patient-reported: 1.8 (1.15–2.87) (odds ratio (9 5% CI)), p = 0.01; physician-reported: 11.0 (5.72–21.01), p < 0.01). Among biologic users, higher treatment goals were associated with higher treatment satisfaction (patient- and physician-rated); lower treatment goals were associated with back lesions and increasing patient age (patient-rated) and higher disease severity (physician-rated). Conclusion: Use of biologics among patients with psoriasis was associated with higher treatment goals. Further use of biologics contributed to treatment satisfaction. Appropriate treatment goals that are shared among patients and their physicians may improve treatment outcomes
Immediate-type hypersensitivity to pyridoxal 5'- phosphate: Study of in vivo and in vitro cross-reactivity and identification of the antigenic determinant
Background: A case in which a 45-year-old female patient with peritonitis experienced immediate-type hypersensitivity on two occasions after administration of preparations containing six different medications was referred to us for closer inspection.
Methods/Results: Skin tests performed on the six medications revealed a positive reaction to the vitamin B6 preparation Biosechs (Wakamoto Pharmaceutical, Tokyo, Japan). Further investigation showed that the principal ingredient, pyridoxal 5'-phosphate (PLP), produced a positive reaction, whereas the injection solvent with the principal ingredient removed produced a negative reaction. When compounds similar to PLP were tested, pyridoxine (PN), pyridoxamine and pyridoxal produced a negative reaction, whereas pyridoxine 5'-phosphate and pyridoxamine 5'-phosphate (PMP) produced a positive reaction. Adenosine 2'-phosphate and adenosine 5'-diphosphate were also tested and these produced a negative reaction. When a histamine-release test was performed, PLP and PMP produced a positive reaction, whereas PN produced a negative reaction. When all tests were performed on three control subjects, the results were all negative.
Conclusions: In this very rare case, phosphate radical conjugates with a pyridine nucleus became haptenic-epitope and an immediate-type reaction occurred. In past cases involving hypersensitivity to vitamin B6, two cases involved a photoallergic reaction caused by PN and one case involved an immediate-type hypersensitivity caused by PLP In the past cases, closely related substances had not been tested and an epitope was not identified
