    How Syria\u27s Failure to Uphold the Kyoto Protocol and Paris Agreement Exacerbated the Effects of Climate Change in the Levant

    The drought in the Levant Region of the Arab Republic of Syria has caused massive destruction by disrupting agriculture and forcing migration to cities. The drought, induced by climate change, has destroyed livelihoods, structures, and health of scores of people. Environmental tensions fed a political discontent that had long been simmering in rural areas, and was a trigger for the Syrian Revolution. Syrians even turned to USAID for help in 2008 when the Syrian minister of agriculture stated publicly, the economic and social fallout from the drought was \u27beyond [Syria\u27s] capacity as a country to deal with. However, the impacts of climate change and the drought in the Levant are still felt today in both Syria and its surrounding countries.     Air-Gapped Structures as Magnetic Elements for Use in Power Processing Systems

    Methodical approaches to the design of inductors for use in LC filters and dc-to-dc converters using air gapped magnetic structures are presented. Methods for the analysis and design of full wave rectifier LC filter circuits operating with the inductor current in both the continuous conduction and the discontinuous conduction modes are also described. In the continuous conduction mode, linear circuit analysis techniques are employed, while in the case of the discontinuous mode, the method of analysis requires computer solutions of the piecewise linear differential equations which describe the filter in the time domain. Procedures for designing filter inductors using air gapped cores are presented. The first procedure requires digital computation to yield a design which is optimized in the sense of minimum core volume and minimum number of turns. The second procedure does not yield an optimized design as defined above, but the design can be obtained by hand calculations or with a small calculator. The third procedure is based on the use of specially prepared magnetic core data and provides an easy way to quickly reach a workable design

    The simulation of converter-controller combinations by means of a flexible digital computer program which produces output to a graphic display is discussed. The procedure is an alternative to mathematical analysis of converter systems. The types of computer programming involved in the simulation are described. Schematic diagrams, state equations, and output equations are displayed for four basic forms of inductor-energy-storage dc to dc converters.     Introduction Cardiopulmonary bypass (CPB) is widely regarded as an important contributor to renal failure, a well recognised complication, following coronary artery surgery (CABG). Off-pump coronary surgery (OPCAB) is intuitively considered renoprotective. We examine the extent of renal glomerular and tubular injury in low-risk patients undergoing either OPCAB or on-pump coronary artery bypass (ONCAB).Methods Forty patients awaiting elective CABG were prospectively randomized into those undergoing OPCAB (n = 20) and ONCAB (n = 20). Table 1 illustrates the exclusion criteria. Glomerular and tubular injury were assessed, respectively, by urinary excretion of microalbumin and retinol binding protein (RBP) indexed to urinary creatinine [1]. Daily measurements were made from admission to postoperative day 5. Fluid balance, serum creati-nine and blood urea were also monitored. Results No mortality or renal complication was observed. Both groups had similar demographic make-up. The OPCAB group received fewer coronary grafts than their counterparts (1.8 versus 2.8; P = 0.002). Serum creatinine and blood urea remained normal in both groups throughout the study. A dramatic and similar rise in mean ± 2SD urinary RBP:creatinine ratio occurred in both groups peaking on day 1 (3183 ± 2534 versus 4035 ± 4078; P = 0.43) before returning to baseline levels. These trends were also observed with the urinary microalbumin:creatinine ratio (5.05 ± 2.66 versus 6.77 ± 5.76; P = 0.22). ONCAB patients had a significantly more negative fluid balance on postoperative day 2 (-183 ± 1118 versus 637 ± 847 ml; P < 0.05). Conclusions Although renal dysfunction did not clinically occur in any patient, sensitive indicators revealed significant and similar injury to both renal tubules and glomeruli following either OPCAB or ONCAB. These suggest that avoidance of CPB per se does not offer additional renoprotection to patients at low risk of perioperative renal insult during CABG

    Optimizing parametrial aperture design utilizing HDR brachytherapy isodose distribution.

    Treatment of cervical cancer includes combination of external beam radiation therapy (EBRT) and brachytherapy (BRT). Traditionally, coronal images displaying dose distribution from a ring and tandem (R&T) implant aid in construction of parametrial boost fields. This research aimed to evaluate a method of shaping parametrial fields utilizing contours created from the high-dose-rate (HDR) BRT dose distribution. Eleven patients receiving HDR-BRT via R&T were identified. The BRT and EBRT CT scans were sent to FocalSim (v4.62)(®) and fused based on bony anatomy. The contour of the HDR isodose line was transferred to the EBRT scan. The EBRT scan was sent to CMS-XIO (v4.62)(®) for planning. This process provides an automated, potentially more accurate method of matching the medial parametrial border to the HDR dose distribution. This allows for a 3D-view of dose from HDR-BRT for clinical decision-making, utilizes a paperless process and saves time over the traditional technique

    The Inflammatory Response to Miniaturised Extracorporeal Circulation: A Review of the Literature

    Conventional cardiopulmonary bypass can trigger a systemic inflammatory response syndrome similar to sepsis. Aetiological factors include surgical trauma, reperfusion injury, and, most importantly, contact of the blood with the synthetic surfaces of the heart-lung machine. Recently, a new cardiopulmonary bypass system, mini-extracorporeal circulation (MECC), has been developed and has shown promising early results in terms of reducing this inflammatory response. It has no venous reservoir, a reduced priming volume, and less blood-synthetic interface. This review focuses on the inflammatory and clinical outcomes of using MECC and compares these to conventional cardio-pulmonary bypass (CCPB). MECC has been shown to reduce postoperative cytokines levels and other markers of inflammation. In addition, MECC reduces organ damage, postoperative complications and the need for blood transfusion. MECC is a safe and viable perfusion option and in certain circumstances it is superior to CCPB
