    Case Specific Autopsy Process of Corpses Pulled Out of Water

    DergiPark: 378989tmsjIntroduction: Our aim is to state the significant aspects of autopsy which is performed in order to accurately identify corpses pulled out of water and to determine the cause of death. Depending on the time spent in the water, various traumatic alterations occur on the corpses pulled out of water. Independent of the cause of death, these alterations that can be named as artifacts, don’t contribute to the determination of the cause of death but rather with the loss of evidence they induce, they greatly detriment the process of reaching a a diagnosis. As a routine, standard procedures cannot be applied to corpses that are decomposed and pulled out of water, different methods need to be followed. One of the most significant aspects is that to find out the whether the cause of death is drowning or any other reason. Case Report: A corpse of a male, greatly decomposed, found in the coastal region of Demirköy (Kırklareli) in January 2001, has been sent to our faculty for autopsy. It has been determined that the corpse is 178cm tall, soft tissues have been greatly decomposed except the chest-wall fully covered with fine sea sand and a small amount of skin with hair, and the skeleton has surfaced to a great extent. All the necessary examinations have been carried out on the highly decomposed corpse, the identity and the cause of death have been endeavored to be revealed by applying the method to be followed in such cases. Conclusion: In the case we present, the examinations executed on the corpse partially skeletonized and the tissue sample identification procedures have proven that this corpse is not the lost member of at least a family. Identification of cases beyond recegnition in the events with several deaths as a result of exhumation of mass graves, plane crashes and natural disasters etc is possible with the systematic approach of forensic medicine and in the light of its accurac

    Evaluation of Thoracolumbar Injuries in Trakya University School of Medicine According to Aospine Classification System

    DergiPark: 700170tmsjAims: This study aims to classify the thoracolumbar spinal injuries that were treated in Trakya University School of Medicineaccording to the recent Thoracolumbar AOSpine injury score and crosscheck the classified data with categorical modifiers suchas gender, trauma type and treatment type. Methods: AOSpine Classification System was used to classify thoracolumbar spinalinjuries. Classes were compared with patients’ gender, age, trauma energy and treatment type. Pearson Chi-Squared test and Shapiro-Wilk test were used for statistical analysis. Results: The total number of patients was 248. One hundred fifty-two (61.3%)were male and 96 (38.7%) were female. One hundred and three (86.6%) patients had high-energy trauma and 16 (13.4%) patientshad low-energy trauma in a total of 119 operated patients. Relationship between treatment type and AOSpine Classification System statistically significant. There was also a significant difference between trauma energy and AOSpine Classification Systemtypes. Conclusion: As a conclusion, gender and trauma energy were found to have a relationship and higher energy traumas weremost likely to cause spinal fractures. In addition, AOSpine classification system may be one of the confounding factors regardingthe choice of treatment. Keywords: Classification, spine, injur

    Effects of Weight Loss Among Metabolically Healthy Obese Men and Women

    OBJECTIVE — Weight loss among metabolically healthy obese (MHO) individuals may be unnecessary or result in elevated cardio-metabolic risk. We studied the effects of exercise- or diet-induced weight loss on cardio-metabolic risk among MHO and metabolically abnormal obese (MAO) adults. RESEARCH DESIGN AND METHODS — Participants were 63 MHO and 43 MAO adults who took part in 3 to 6 months of exercise- or diet-induced weight loss intervention. Changes in anthropometry, adipose tissue distribution, and cardio-metabolic risk factors were assessed. RESULTS — Body weight, waist circumference, and total abdominal and visceral adipose tissue were reduced in all subjects (P 0.05). Improvements in insulin sensitivity were observed in MHO and MAO men and women (P 0.05), but were greater in the MAO individuals (P 0.05). Fasting insulin was the only other cardio-metabolic improvement among MHO individ-uals (P 0.05). CONCLUSIONS — Lifestyle-induced weight loss among MHO subjects is associated with a reduction in total and abdominal obesity and improvement in selected cardio-metabolic risk factors.     Sağlık ekonomisi, sağlık sektörüne ayrılan kaynaklardan daha etkin bir şekilde yararlanabilmek için incelemeler yapan bir bilim dalıdır. Bu çalışmanın amacı sağlık ekonomisinde ekonomik değerlendirme yöntemlerinden biri olarak kullanılan fayda maliyet analizini ve sağlık sektörüne ayrılan kaynakların verimli bir şekilde kullanılması için fayda maliyet analizlerinde kullanılan bir araç olan sosyal iskonto oranı kavramını değerlendirmektir. Bir derleme olan bu çalışmada sosyal iskonto oranına ilişkin yürütülen ulusal ve uluslararası, teorik ve ampirik çalışmalara yönelik bir literatür incelemesi gerçekleştirilerek sosyal iskonto oranının seçim yöntemleri ve sağlık projelerinde kullanılması gereken uygun sosyal iskonto oranın ne olması gerektiğini değerlendirmeye yönelik bir analiz gerçekleştirilmiştir. Analiz yürütülürken sosyal iskonto oranına ilişkin akademik literatüre, dünya genelinde sağlıkta ekonomik değerlendirmelerde uygulanan indirim oranlarına ve küresel olarak yol gösterici kılavuz ve yönergelere ilişkin bir inceleme gerçekleştirilmiştir. Bu bağlamda ekonomik bir bakış açısı çerçevesinde sağlık sektörü açısından sosyal iskonto oranı kavramsal olarak değerlendirilmeye çalışılmış, önerilen iskonto hadleri, iskonto haddinin seçimine etki eden faktörler ve oranının seçiminde yaşanan belirsizliğin nedenleri değerlendirilmiştir. Sonuç olarak politik tercihlerin yön verdiği sosyal iskonto oranı için kamusal kaynakların optimal kullanımının önemi ve kuşaklararasında bu kaynakların dağılımına olan etkileri nedeniyle toplumun geneline yayılan sonuçlar doğurduğu söylenebilir.Health economics is a branch of science that examines in order to benefit more effectively from the resources allocated to the health sector. The aim of this study is to evaluate the concept of social discount rate, which is used as one of the economic evaluation methods in health economics and used in cost-benefit analysis, which is a tool for the efficient use of resources allocated to the health sector. In this study, which is a compilation, a literature review is carried out on national and international, theoretical and empirical studies on the social discount rate, and another analysis is carried out to evaluate the selection methods of the social discount rate and what the appropriate social discount rate should be used in health projects. While conducting the analysis, a review of the academic literature on the social discount rate, the discount rates applied in economic evaluations in health around the world, and globally guiding and instructions were performed. In this context, within the framework of an economic perspective, the social discount rate has been tried to be evaluated conceptually in terms of the health sector, the proposed discount rates and the factors affecting the selection of the discount rate and the reasons for the uncertainty in the selection of the rate have been evaluated. As a result, it can be said that the optimal use of public resources for the social discount rate, which is directed by political preferences, and its effects on the distribution of these resources between generations have consequences that spread throughout the society
