The outcome of kidney transplants with multiple renal arteries
BACKGROUND: The use of grafts with multiple renal arteries has been considered a relative contraindication because of the increased incidence of vascular and urologic complications The aim of this study is to determine whether the kidney grafts with multiple arteries have any adverse effect upon post-transplant graft and patient survival. METHODS: We reviewed the records of 225 adult kidney transplants done consecutively at our institution. Twenty-nine patients (12.8%) had grafts with multiple renal arteries. We analyzed the incidence of post-transplant hypertension and vascular complications, mean creatinine levels, patient and graft survival. In 17 cases reconstruction was done as conjoined anastomosis between two arteries of equal size, and in 6 cases as end-to-side anastomosis of smaller arteries to larger arteries. Multiple anastomoses were performed in 6 cases. RESULTS: In one patient postoperative bleeding occurred. Mean systolic blood pressures, creatinine levels at first year and last follow-up and complication rates were all in acceptable ranges. There was no significant difference in graft and patient survival between multiple and single renal artery allografts. CONCLUSION: Although the kidney grafts with multiple renal arteries have been considered a relative contraindication because of the increased risk of complications, in our study allografts with multiple arteries were used successfully in kidney transplantation
Effects of PUVA therapy on kidney allografts: results of a randomized prospective double-blind study

In einer retrospektiven Analyse wurden 1.102 Patienten mit Kinderwunsch
nachuntersucht, die sich zwischen 1994 und 2010 einer VVS und/oder TVS
unterzogen hatten. Folgende Parameter wurden analysiert: TVS-Inzidenz in
Abhängigkeit von der Verschlusszeit sowie Permeabilitäts- und
Schwangerschaftsraten unter Berücksichtigung der Operationstechnik, des
Partnerinnenalters und -status (neue oder gleiche Partnerin wie vor der
Sterilisation) sowie der Zweitoperation. Mit zunehmender Dauer der
Verschlusszeit wurde die Notwendigkeit einer Tubulovasostomie größer und
erreichte nach mehr als 10 Jahren eine Häufigkeit von 42,4 %. Die technisch
aufwändigere ein- bzw. beidseitige Tubulovasostomie führte mit 60,5 % bzw.
48,8 % zu signifikant geringeren Schwangerschaftsraten im Vergleich zur
beidseitigen Vasovasostomie mit 75,1 %. Das Alter der Partnerin war der
entscheidende prognostische Parameter für den Erfolg der Operation. Frauen
unter 30 Jahren erreichen eine Schwangerschaftsrate von 84,3 %, während sie ab
einem Alter von 40 Jahren signifikant auf 22,0 % sank. Bei gleicher Partnerin
wurde mit 86 % die beste Schwangerschaftsrate erreicht, nach einer
Zweitoperation noch 51 %.Vasovasostomy (VVS) and tubulovasostomy (TVS) are the treatment options of
first choice to restore fertility after vasectomy. Aim of this study was to
determine the outcome of vasectomy reversal and its predictors of success. :
Intraoperative azoospermia was found in 148 of 678 cases (21, 8 %) depending
on obstructive interval: 10
years in 42, 8 %. Postoperatively overall patency rate was 90, 9 % (602/662)
depending significantly on the anastomosis technique: VVS+VVS in 94, 2 %,
VVS+TVS in 84, 9 % and bilateral TVS in 70, 6 % of the patients. The resulting
overall pregnancy rate was 71, 1 % (404/568) depending significantly on the
maternal age: < 30 years in 84, 3 %, 30-34 years in 77, 1 %, 35-39 years in
60, 1 % and ≥ 40 years in only 22, 0 %. The best results were achieved with
the same female partner. After failed vasectomy reversal a second reversal
procedure was a good option with pregnancy rate of 51 %
