Автоматический метод анализа мультиспектральных кольпоскопических изображений для телевизионной системы диагностики рака шейки матки

Automated method of fluorescence images analysis obtained by excitation radiation with a wavelength of 360 and 390 nm is proposed. The method allows to detect the status of tissues of cervix: normal, chronic nonspecific inflammation (CNI) and cervical intraepithelial neoplasia (CIN), and build differential map pathology. For the border CIN/CNI achieved a sensitivity of 87 % and specificity 71 %. The method includes a specific preprocessing of the original images: combining images taken in different lighting conditions and highlight the area of interest. Features of the method are the use of a combination of features calculated for images of different types, and decision rule for classification based data mining techniques.Предложен автоматический метод анализа флуоресцентных изображений, полученных при возбуждающих излучениях с длиной волны 360 и 390 нм. Метод позволяет выявить состояния тканей шейки матки: норму, воспалительный процесс (chronic nonspecific inflammation - CNI), и онкологические изменения (cervical intraepithelial neoplasia - CIN), и построить дифференциальную карту патологии. Для границы CIN/CNI достигнуты чувствительность 87 % и специфичность 71 %. Метод включает специальную предобработку исходных изображений: совмещение изображений, полученных в разных условиях освещения и выделение области интереса. Особенностями метода являются использование совокупности признаков, рассчитанных по изображениям разного типа, и решающее правило при классификации на основе методов интеллектуального анализа данных

The proposed method of colposcopic images processing allows to synthesize a special image that simulates the application to visualize vessels and to provide better analysis of vascular structures by the physician.Рассмотрены новые методы обработки и анализа изображений для экспертно-консультирующей системы дифференциальной диагностики состояний тканей шейки матки. Предложенный метод обработки кольпоскопических изображений позволяет синтезировать специальное изображение, имитирующее применение зеленого оптического фильтра для визуализации сосудов и обеспечивающее более эффективный анализ сосудистых структур врачом. Отличительной особенностью разработанного метода анализа изображений с целью дифференциальной диагностики является использование совокупности признаков, рассчитанных по изображениям разных типов, и формирование решающих правил, основанных на технологиях "data mining". Многопризнаковый анализ обеспечивает для границы дифференциальной диагностики CIN/CNI чувствительность 87 % и специфичность 75 %. Важной составляющей многопризнакового анализа является анализ изображений, полученных в белом свете. Автоматический метод сегментации флуоресцентных изображений, полученных в ближнем инфракрасном диапазоне

Introduction. Near-infrared fluorescence imaging technology is widely used in laparoscopic surgery. Intraoperative fluorescence navigation is based on accurate segmentation of fluorescent regions in near-infrared images (NIR images), thus increasing the accuracy and safety of surgical intervention. Moreover, it is an important auxiliary technology for laparoscopic surgery. Therefore, the search for an automatic method that allows for accurate segmentation of fluorescent regions in NIR images can contribute to an improved efficiency of intraoperative navigation.Aim. Development of a method for automatic segmentation of fluorescent images obtained in the near infrared range. Materials and methods. The proposed method consists of two stages. At the first stage, a preliminary segmentation of the image is performed based on the adaptive threshold found by Otsu’s method. At the second stage, the segmented area is refined using Otsu’s weighted method. The main advantage of the proposed method consists in the automatic determination of parameter α, which determines the performance of Otsu’s weighted method. Experiments were carried out using 276 actual laparoscopic images. The metric misclassification error (ME) was used to assess the quality of segmentation.Results. The average ME of the proposed method was found to be 10.4 %, compared to that obtained by the conventional Otsu’s method of 27.1 %.Conclusion. In comparison with Otsu’s method, the developed method shows an increased efficiency and accuracy of fluorescent image segmentation. This allows for a higher diagnostic accuracy and a more efficient navigation during laparoscopic surgery.Введение. В настоящее время технология флуоресцентной визуализации в ближнем инфракрасном диапазоне широко применяется при проведении лапароскопических операций. Основой технологии является сегментация области флуоресценции на изображениях, полученных в ближнем инфракрасном диапазоне (БИК-изображениях). Для повышения качества и эффективности навигации необходимо разработать автоматический метод, позволяющий сегментировать флуоресцентные области на БИК-изображениях с максимальной точностью.Цель работы. Повышение точности автоматической сегментации флуоресцентных изображений, полученных в ближнем инфракрасном диапазоне.Материалы и методы. Предложенный метод состоит из двух этапов. На первом этапе выполняется предварительная сегментация изображения на основе адаптивного порога, найденного по методу Оцу. На втором этапе сегментированная область уточняется с помощью взвешенного метода Оцу. Главной особенностью метода является автоматическое определение параметра α, являющегося ключевым для эффективной работы взвешенного метода Оцу. Экспериментальное исследование метода было выполнено на реальных лапароскопических изображениях, общее число изображений в исследовании – 276. Значение ошибки сегментации (метрика ME – misclassification error) было использовано для оценки качества работы предложенного метода.Результаты. Среднее значение ошибки сегментации (метрика ME) предложенного метода составляет 10.4 %, а традиционного метода Оцу – 27.1%.Заключение. По сравнению с традиционным методом Оцу использование разработанного метода позволяет повысить точность сегментации флуоресцентных изображений. Это обеспечивает высокую чувствительность и специфичность при проведении диагностики и позволяет реализовать более эффективную навигацию в процессе лапароскопической операции

Cytokines and local chronic inflammation in the formation of infertility in fertile age women

The purpose of the study was to examine the characteristics of the content of cytokines in the peritoneal fluid in women of reproductive age with infertility against the background of chronic inflammatory diseases of the pelvic organs in the stage of clinical remission.Material and methods. A study was conducted of 50 patients aged 23-36 years with a verified diagnosis of infertility of tubal peritoneal genesis against a background of chronic inflammatory diseases of the reproductive sphere in the stage of clinical remission. As a control group, 15 practically healthy women aged 25-37 years were examined. Concentration of cytokines TNF-a, IL-4, IL-6, IL-10, IFN-y, MCP-1 in the peritoneal fluid was measured by enzyme-linked immunosorbent assay.Results and discussion. In the main group of patients with tubal-peritoneal infertility, the activity of local inflammatory reactions is tested in the period of clinical remission of a chronic inflammatory process. This is confirmed by a significantly higher level of TNF-a, IL-4, IL-6, IFN-y, MCP-1 in the peritoneal fluid compared to the control group. The studied biologically active substances play a significant role in the development of the immune response, as well as fibroplastic processes. The results can be regarded as the latent development of the immune response in reactions to constant antigenic stimulation from the focus of chronic inflammation, which is a significant factor in the development of adhesions in the formation of infertility, and requires the inclusion of immunotropic therapy methods in treatment

AB0-incompatibility of mother and fetus: the role of anti-glycan alloantibodies in the hemolytic disease of newborns

The mother and fetus incompatibility due to Rh-factor, blood group or other blood factors can lead to hemolytic disease of the fetus and newborn (HDN). HDN is a clinical disease condition of the fetus and newborn as a result of hemolysis, when maternal IgG alloantibodies cross the placenta and destroy the red blood cells of the fetus and newborn. The child disease begins in utero and can dramatically increase immediately after birth. As a result, hyperbilirubinemia and anemia develop, that can lead to abortions, serious complications, or death of the neonates in the absence of proper therapy. The range of HDN has changed significantly now compared to previous decades. Half a century ago, HDN was considered an almost complete synonym of RhD-alloimmunization, and this was a frequent problem for newborns. By now due to the high effective of Rh-conflict prevention, immunological AB0-conflicts have become the most common cause of HDN. The review aimes to one of the main causes of jaundice and anemia in neonates at present, i.e. HDN due to immunological AB0-conflict of mother and newborn (AB0-HDN). The main participants of the AВ0- incompatibility mother and child are considered, namely A- and B-glycans, as well as the corresponding anti-glycan alloantibodies. Close attention is paid to the structure features of glycan alloantigens on the red blood cells of the fetus and adult. The possible correlation of the frequency and severity of HDN with the blood group of mother and child, as well as with the titer of maternal alloantibodies, has been considered. The influence of immunoglobulin G subclasses on the AB0-HDN development has been evaluated. In most cases, AB0-HDN appear when the mother has the blood group 0, and the fetus has the group A (subgroup A1) or the group B. Other rare incidences of AB0-incompatibility with severe course are occurred. As a whole the etiology of AB0-HDN is complex and the HDN severity is influenced by many factors. The authors have analyzed statistical data, as well as the prevalence of AB0-incompatibility and AB0-HDN in various regions of the world. Current approaches to the diagnosis of AB0-HDN are discussed in addition. By now the problems of AB0- HDN occurrence and developing of ways to overcome this disease remain relevant

The article discussed the possibility of using renewable energy for water distillation. Presented desalination plant works on energy of solar collectors and principles of action desalination plant. The purpose of the article is aimed at identifying the personal characteristics of the subjects of the two different groups of Bank employees in their professional activities. A leading method is a method of testing aimed at identifying personal characteristics such as: empathic abilities, personality predisposition to conflict behavior, ability to listen, machiavellism. The article revealed that the socio-psychological competence of the subjects of the two different groups of Bank employees is determined by a set of specific vocational skills, such as empathic abilities, personality predisposition to conflict behavior, listening skills, the manifestation of machiavellism. The article can be useful for selection and placement of personnel in the internal environment
