Similar mortality and reduced loss to follow-up in integrated compared with vertical programs providing antiretroviral treatment in sub-saharan Africa.

Vertical HIV programs have achieved good results but may not be feasible or appropriate in many resource-limited settings. Médecins sans Frontières has treated HIV in vertical programs since 2000 and over time integrated HIV treatment into general health care services using simplified protocols. We analyzed the survival probability among patients receiving antiretroviral therapy (ART) from 2003 to 2010 in integrated versus vertical programs in 9 countries in sub-Saharan Africa

The physical theory of one dimensional galactic cosmic-ray propagation in the atmosphere

    Get PDF
An analytical theory of atmospheric cosmic-ray propagation is developed based on a phenomenological model of hadron-nucleus collisions. This model correctly predicts the sea level cosmic-ray nucleon, pion and muon spectra, the cosmic-ray ionization profile in the atmosphere, and neutron flux and density profiles in the atmosphere. It is concluded that the large scale properties of atmospheric cosmic-rays can be accurately predicted on the basis of a nucleonic cascade with all secondaries mediated by pion production. Implications for energy independence of cross sections, the recent 70 GeV results from Serpukhov, and nucleonic relaxation rates in the atmosphere are discussed

Methane overtone absorption by intracavity laser spectroscopy

    Get PDF
Interpretation of planetary methane (CH4) visible-near IR spectra, used to develop models of planetary atmospheres, has been hampered by a lack of suitable laboratory spectroscopic data. The particular CH4 spectral bands are due to intrinsically weak, high overtone-combination transitions too complex for classical spectroscopic analysis. The traditional multipass cell approach to measuring spectra of weakly absorbing species is insufficiently sensitive to yield reliable results for some of the weakest CH4 absorption features and is difficult to apply at the temperatures of the planetary environments. A time modulated form of intracavity laser spectroscopy (ILS), has been shown to provide effective absorption pathlengths of 100 to 200 km with sample cells less than 1 m long. The optical physics governing this technique and the experimental parameters important for obtaining reliable, quantitative results are now well understood. Quantitative data for CH4 absorption obtained by ILS have been reported recently. Illustrative ILS data for CH4 absorption in the 619.7 nm and 681.9 nm bands are presented. New ILS facilities at UM-St. Louis will be used to measure CH4 absorption in the 700 to 1000 nm region under conditions appropriate to the planetary atmospheres

Calculations of cosmogenic nuclide production rates in the Earth's atmosphere and their inventories

    Get PDF
The production rates of cosmogenic isotopes in the Earth's atmosphere and their resulting terrestrial abundances have been calculated, taking into account both geomagnetic and solar-modulatory effects. The local interstellar flux was assumed to be that of Garcia-Munoz, et al. Solar modulation was accounted for using the heliocentric potential model and expressed in terms of the Deep River neutron monitor count rates. The geomagnetic field was presented by vertical cutoffs calculated by Shea and Smart and the non-vertical cutoffs calculated using ANGRI. The local interstellar particle flux was first modulated using the heliocentric potential field. The modulated cosmic-ray fluxes reaching the earth's orbit then interacted with the geomagnetic field as though it were a high-pass filter. The interaction of the cosmic radiation with the Earth's atmosphere was calculated utilizing the Bolztmann transport equation. Spallation cross sections for isotope production were calculated using the formalism of Silberberg and Tsao and other cross sections were taken from standard sources. Inventories were calculated by accounting from the variation in solar modulation and geomagnetic field strength with time. Results for many isotope, including C-14, Be-7 and Be-10 are in generally good agreement with existing data. The C-14 inventory, for instance, amounts to 1.75/sq cm(e)/s, in excellent agreement with direct estimates

An Investigative Redesign of the ECG and EMG Signal Conditioning Circuits for Two-fault Tolerance and Circuit Improvement

    Get PDF
An investigation was undertaken to make the elctrocardiography (ECG) and the electromyography (EMG) signal conditioning circuits two-fault tolerant and to update the circuitry. The present signal conditioning circuits provide at least one level of subject protection against electrical shock hazard but at a level of 100 micro-A (for voltages of up to 200 V). However, it is necessary to provide catastrophic fault tolerance protection for the astronauts and to provide protection at a current level of less that 100 micro-A. For this study, protection at the 10 micro-A level was sought. This is the generally accepted value below which no possibility of microshock exists. Only the possibility of macroshock exists in the case of the signal conditioners. However, this extra amount of protection is desirable. The initial part deals with current limiter circuits followed by an investigation into the signal conditioner specifications and circuit design

    Advanced rocket propulsion

    Get PDF
Existing NASA research contracts are supporting development of advanced reinforced polymer and metal matrix composites for use in liquid rocket engines of the future. Advanced rocket propulsion concepts, such as modular platelet engines, dual-fuel dual-expander engines, and variable mixture ratio engines, require advanced materials and structures to reduce overall vehicle weight as well as address specific propulsion system problems related to elevated operating temperatures, new engine components, and unique operating processes. High performance propulsion systems with improved manufacturability and maintainability are needed for single stage to orbit vehicles and other high performance mission applications. One way to satisfy these needs is to develop a small engine which can be clustered in modules to provide required levels of total thrust. This approach should reduce development schedule and cost requirements by lowering hardware lead times and permitting the use of existing test facilities. Modular engines should also reduce operational costs associated with maintenance and parts inventories

Investigation into the common mode rejection ratio of the physiological signal conditioner circuit

    Get PDF
The common mode rejection ratio (CMRR) of the single operational amplifier (op amp) differential amplifier and of the three operational amplifier differential amplifier was investigated. The three op amp differential amplifier circuit is used in the signal conditioner circuit which amplifies signals such as the electromyograph or electrocardiogram. The investigation confirmed via SPICE modeling what has been observed by others in the recent literature that the CMRR for the circuit can be maximized without precision resistor values or precisely matched op amps. This can be done if one resistor in the final stage can be adjusted either by a potentiometer or by laser trimming in the case of hybrid circuit fabrication
