    A Spectrophotometric Method for the Determination of Ramipril in Solid Dosage Forms

    Purpose: To develop a simple and cost effective spectrophotometer method for the determination of ACE inhibitor ramipril in dosage forms. Methods: UV spectrophotometry was used to develop and validate a simple method for the assay of ramipril in solid dosage form at λmax of 210 nm, as per International Conference on Harmonization (ICH) guidelines. Aqueous methanol (5 %) was used as the blank solvent. The method was validated for linearity, recovery, accuracy, precision, specificity in the presence of excipients, and also for inter-day stability under laboratory conditions. Results: Validation results showed linearity in the range 1 – 38 µg/ml; recovery accuracy 101.55%; regression equation Y = 0.0256X + 0.0697, R2 of 0.9942; precision RSD <2.00 %; and negligible interference from common excipients and colorants. The method was accurate (95 % confidence limit) compared to standard liquid chromatography (LC) method, with comparable reproducibility when used to assay a commercial product (Ramitace®, 2 and 5 mg tablets).     Cervical Spine Injury: A ten‑year multicenter analysis of evolution of care and risk factors for poor outcome in southeast Nigeria

    Study Design: Retrospective study.Objective: To describe the evolution of care and risk factors for poor outcome in patients with cervical spine injury (CSI) treated at three centers in southeast Nigeria.Setting: Nigeria, southeast.Materials and Methods: A 10‑year retrospective multicenter analysis of patients with CSI, managed at three centers in southeast Nigeria, from January 2003 to December 2012.Results: Two hundred and seven patients (55%) had CSI out of 377 spinal injury cases in the three study centers, but 195 cases had complete records and were studied. There were 148 males and 47 females. The age range was 3-74 years with a mean of 32.6 (±1.9) years 95% CI. Most injuries (149 cases) resulted from motor vehicular accidents (MVA). The C5 spinal level was involved in 75 (38%) cases One hundred and seventeen patients (60%) presented with American Spinal Injury Association A (ASIA A) injury. CSI care evolved from the application of a Minerva jacket or cervical traction only to cervical traction and spinal fusion resulting in a reduction in hospital stay (F = 52.5, DF (2, 3) P &lt; 0.05). When compared to 51 patients with incomplete injuries, who improved in neurologic al status at discharge, only three patients with ASIA grade A experienced some improvement. The mortality rate from our series is 16% (32 patients). Those who died were more likely to have a complete injury (25 patients) or a high cervical injury (X2 = 61.2, P &lt; 0.05) among other factors.Conclusion: The cervical spine is the most commonly injured spinal segment in southeastNigeria. Although treatment evolution has resulted in reduction of hospital stay, the associated mortality risk still remains high.Key words: Cervical spine injury, pattern, southeast Nigeria, treatment outcom

    Lymphatic Filariasis in Nigeria; Micro-stratification Overlap Mapping (MOM) as a Prerequisite for Cost-Effective Resource Utilization in Control and Surveillance

    Background Nigeria has a significant burden of lymphatic filariasis (LF) caused by the parasite Wuchereria bancrofti. A major concern to the expansion of the LF elimination programme is the risk of serious adverse events (SAEs) associated with the use of ivermectin in areas co-endemic with Loa filariasis. To better understand this, as well as other factors that may impact on LF elimination, we used Micro-stratification Overlap Mapping (MOM) to highlight the distribution and potential impact of multiple disease interventions that geographically coincide in LF endemic areas and which will impact on LF and vice versa. Methodology/Principal findings LF data from the literature and Federal Ministry of Health (FMoH) were collated into a database. LF prevalence distributions; predicted prevalence of loiasis; ongoing onchocerciasis community-directed treatment with ivermectin (CDTi); and long-lasting insecticidal mosquito net (LLIN) distributions for malaria were incorporated into overlay maps using geographical information system (GIS) software. LF was prevalent across most regions of the country. The mean prevalence determined by circulating filarial antigen (CFA) was 14.0% (n = 134 locations), and by microfilaria (Mf) was 8.2% (n = 162 locations). Overall, LF endemic areas geographically coincided with CDTi priority areas, however, LLIN coverage was generally low (<50%) in areas where LF prevalence was high or co-endemic with L. loa. Conclusions/Significance The extensive database and series of maps produced in this study provide an important overview for the LF Programme and will assist to maximize existing interventions, ensuring cost effective use of resources as the programme scales up. Such information is a prerequisite for the LF programme, and will allow for other factors to be included into planning, as well as monitoring and evaluation activities given the broad spectrum impact of the drugs used
