N plus 3 Advanced Concept Studies for Supersonic Commercial Transport Aircraft Entering Service in the 2030-2035 Period
Boeing, with Pratt & Whitney, General Electric, Rolls-Royce, M4 Engineering, Wyle Laboratories and Georgia Institute of Technology, conducted a study of supersonic commercial aircraft concepts and enabling technologies for the year 2030-2035 timeframe. The work defined the market and environmental/regulatory conditions that could evolve by the 2030/35 time period, from which vehicle performance goals were derived. Relevant vehicle concepts and technologies are identified that are anticipated to meet these performance and environmental goals. A series of multidisciplinary analyses trade studies considering vehicle sizing, mission performance and environmental conformity determined the appropriate concepts. Combinations of enabling technologies and the required technology performance levels needed to meet the desired goals were identified. Several high priority technologies are described in detail, including roadmaps with risk assessments that outline objectives, key technology challenges, detailed tasks and schedules and demonstrations that need to be performed. A representative configuration is provided for reference purposes, along with associated performance estimates based on these key technologies
Spatial Time-Dependent Reliability Analysis of Reinforced Concrete Slab Bridges Subject to Realistic Traffic Loading
Resistance and loads are often correlated in time and space. The paper assesses the influence of these correlations on structural reliability/probability of failure for a typical two-lane RC slab bridge under realistic traffic loading. Spatial variables for structural resistance are cover and concrete compressive strength, which in turn affect the strength and chloride-induced corrosion of RC elements. Random variables include pit depth and model error. Correlation of weights between trucks in adjacent lanes and inter-vehicle gaps are also included and are calibrated against Weigh-In-Motion (WIM) data. Reliability analysis of deteriorating bridges needs to incorporate uncertainties associated with parameters governing the deterioration process and loading. One of the major unanswered questions in the work carried out to date is the influence of spatial variability of load and resistance on failure probability. Spatial variability research carried out to date has been mainly focused on predicting the remaining lifetime of a corroding structure and spatial variability of material, dimensional and environmental properties. A major shortcoming in the work carried out to date is the lack of an allowance for the spatial variability of applied traffic loads. In this paper, a 2-dimensional (2D) random field is developed where load effects and time-dependent structural resistance are calculated for each segment in the field. The 2D spatial time-dependent reliability analysis of an RC slab bridge found that a spatially correlated resistance results in only a small increase in probability of failure. Despite the fact that load effect at points along the length of a bridge are strongly correlated, the combined influence of correlation in load and resistance on probability of failure is small

Anthropogenic aerosols mask increases in US rainfall by greenhouse gases.
A comprehensive understanding of human-induced changes to rainfall is essential for water resource management and infrastructure design. However, at regional scales, existing detection and attribution studies are rarely able to conclusively identify human influence on precipitation. Here we show that anthropogenic aerosol and greenhouse gas (GHG) emissions are the primary drivers of precipitation change over the United States. GHG emissions increase mean and extreme precipitation from rain gauge measurements across all seasons, while the decadal-scale effect of global aerosol emissions decreases precipitation. Local aerosol emissions further offset GHG increases in the winter and spring but enhance rainfall during the summer and fall. Our results show that the conflicting literature on historical precipitation trends can be explained by offsetting aerosol and greenhouse gas signals. At the scale of the United States, individual climate models reproduce observed changes but cannot confidently determine whether a given anthropogenic agent has increased or decreased rainfall
Critical relationship between sodium valproate dose and human teratogenicity: results of the Australian register of anti-epileptic drugs in pregnancy
To compare the incidence of foetal malformations (FMs) in pregnant women with epilepsy treated with different anti-epileptic drugs (AED) and doses, and the influence of seizures, family and personal history, and environmental factors. A prospective, observational, community-based cohort study. Methods. A voluntary, Australia-wide, telephone-interview-based register prospectively enrolling three groups of pregnant women: taking AEDs for epilepsy; with epilepsy not taking AEDs; taking AEDs for a non-epileptic indication. Four hundred and fifty eligible women were enrolled over 40 months. Three hundred and ninety six pregnancies had been completed, with 7 sets of twins, for a total of 403 pregnancy outcomes. Results. 354 (87.8%) pregnancy outcomes resulted in a healthy live birth, 26 (6.5%) had a FM, 4 (1%) a death in utero, 1 (0.2%) a premature labour with stillbirth, 14 (3.5%) a spontaneous abortion and 4 lost to follow-up. The FM rate was greater in pregnancies exposed to sodium valproate (VPA) in the first trimester (116.0%) compared with those exposed to all other AEDs (16.0% vs. 2.4%, P < 0.01) or no AEDs (16.0% vs. 3.1 %, P < 0.01). The mean daily dose of VPA taken in pregnancy with FMs was significantly greater than in those without (11975 vs: 1128 mg, P < 0.01). The incidence of FM with VPA doses greater than or equal to 1100 mg was 30.2% vs. 3.2% with doses < 1100 mg (P < 0.01). Conclusions. There is a dose-effect relationship for FM and exposure to VPA during the first trimester of pregnancy, with higher doses of VPA associated with a significantly greater risk than with lower doses or with other AEDs. These results highlight the need to limit, where possible, the dose of VPA in pregnancy. (C) 2004 Elsevier Ltd. All rights reserved
Histologic and immunohistochemical responses after aortic valve allografts in the rat
Background. Human aortic valve allografts elicit a cellular and humoral immune response. It is not clear whether this is important in promoting valve damage. We investigated the changes in morphology, cell populations, and major histocompatibility complex antigen distribution in the rat aortic valve allograft. Methods. Fresh heart valves from Lewis rats were transplanted into the abdominal aorta of DA rats. Valves from allografted, isografted, and presensitized recipient rats were examined serially with standard morphologic and immunohistochemical techniques. Results. In comparison with isografts, the allografts were infiltrated and thickened by increased numbers of CD4(+) and CD8(+) lymphocytes, macrophages, and fibroblasts. Thickening of the valve wall and leaflet and the density of the cellular infiltrate was particularly evident after presensitization. Endothelial cells were frequently absent in presensitized allografts whereas isografts had intact endothelium. Cellular major histocompatibility complex class I and II antigens in the allograft were substantially increased. A long-term allograft showed dense fibrosis and disruption of the media with scattered persisting donor cells. Conclusions. The changes in these aortic valve allograft experiments are consistent with an allograft immune response and confirm that the response can damage aortic valve allograft tissue. The PCORnet Blood Pressure Control Laboratory: A Platform for Surveillance and Efficient Trials.
BACKGROUND: Uncontrolled blood pressure (BP) is a leading preventable cause of death that remains common in the US population despite the availability of effective medications. New technology and program innovation has high potential to improve BP but may be expensive and burdensome for patients, clinicians, health systems, and payers and may not produce desired results or reduce existing disparities in BP control. METHODS AND RESULTS: The PCORnet Blood Pressure Control Laboratory is a platform designed to enable national surveillance and facilitate quality improvement and comparative effectiveness research. The platform uses PCORnet, the National Patient-Centered Clinical Research Network, for engagement of health systems and collection of electronic health record data, and the Eureka Research Platform for eConsent and collection of patient-reported outcomes and mHealth data from wearable devices and smartphones. Three demonstration projects are underway: BP track will conduct national surveillance of BP control and related clinical processes by measuring theory-derived pragmatic BP control metrics using electronic health record data, with a focus on tracking disparities over time; BP MAP will conduct a cluster-randomized trial comparing effectiveness of 2 versions of a BP control quality improvement program; BP Home will conduct an individual patient-level randomized trial comparing effectiveness of smartphone-linked versus standard home BP monitoring. Thus far, BP Track has collected electronic health record data from over 826 000 eligible patients with hypertension who completed ≈3.1 million ambulatory visits. Preliminary results demonstrate substantial room for improvement in BP control (<140/90 mm Hg), which was 58% overall, and in the clinical processes relevant for BP control. For example, only 12% of patients with hypertension with a high BP measurement during an ambulatory visit received an order for a new antihypertensive medication. CONCLUSIONS: The PCORnet Blood Pressure Control Laboratory is designed to be a reusable platform for efficient surveillance and comparative effectiveness research; results from demonstration projects are forthcoming
