    CAUSES OF DEVELOPMENT OF ACUTE AND CHRONIC THROMBOCYTOPENIA IN CHILDREN OF IRKUTSK REGION

    Background. Immune thrombocytopenia in children is not a rare disease. Currently, this diagnosis is found in 2 times more often than 5-10 years ago. Acute immune thrombocytopenia is increasingly taking on a chronic course. Aims. At present, it is impossible to predict the outcome of the disease. But the chronic course of the disease significantly reduces the quality of life of children and their parents. The purpose of the study is to determine the likely causes of the development and chronicity of the disease. Materials and Methods. A retrospective analysis of 433 case histories of the cancer department of the Regional Children’s Hospital in Irkutsk was conducted. Statistical processing of the received data was carried out with the help of the Matlab program and statistical package “Statistica 6.0”. Results. The main trigger factor is ARVI and vaccination against poliomyelitis. Sensitization to herpes viruses can be a provoking factor in the development of the disease when re-encountering the virus. The brighter the clinic, the more children receive immunoglobulin treatment and the disease often ends with complete recovery. Children with minimal clinical manifestations are treated with hormones and more often the disease passes into a chronic form. Conclusion. If the child is discharged from the hospital with only a clinical improvement, then there is a greater likelihood of a chronic process

    Palaeozoic carbonates and fossils of the Mendeleev Rise (eastern Arctic): A study of dredged seafloor material

    © 2018 Elsevier Ltd Fossiliferous carbonate rocks dredged during the “Arctic-2012” cruise on the Mendeleev Rise (eastern Arctic) provide proof of the presence of Upper Silurian(?)–Middle Devonian, Famennian–Tournaisian, Bashkirian–Kasimovian, Gshelian–lower Asselian(?) and Kungurian–Kazanian carbonate deposits. The wide spectrum of facies includes deposits of both photic zone (with fusulinids, algae, relicts of microbial and coral reefs) and deeper dysphotic areas (with trilobites, deep-water tentaculitids and ostracods). The results obtained suggest that there were at least three periods of carbonate platform sedimentation during the latest Silurian(?) to Permian. The Late Silurian?–Devonian biota do not show biogeographical differentiation, but rather are distributed globally. Shallow-water foraminifera and some algae of early Pennsylvanian–basal Cisuralian age belong to the warm-water province. These forms are unknown in the Moscovian–Permian of the Boreal Realm (Taimyr, New Siberian Islands, Verkhoyanie, Omolon Massif) but are typical for Alaska and Arctic Canada, Wrangel Island, Chukotka, Polar Urals and Svalbard. The disappearance of warm-water biota during late Artinskian-Kungurian times led to a subsequent predominance of smaller foraminifera: this assemblage with Protonodosaria is widely distributed in Permian deposits of Novaya Zemlya, Urals, Barents Sea and the eastern Arctic. The warm-water Bashkirian-Asselian biota suggests that the Mendeleev-Chukotka-Wrangel block was a low-latitude shallow basin with predominant carbonate sedimentation, being part of the Arctida supercontinent, connected temporarily with the eastern margin of Laurasia (Chukcha-Alaska block)

    Regularities of fracture pattern formation in alumina ceramics subjected to dynamic indentation

    In this paper the process of dynamic indentation, causing deformation and fracture of alumina ceramics, is investigated. The dynamic indentation experiments were carried out on the original setup based on the split Hopkinson bar technique. The regularities of structure evolution caused by indenter penetration are studied using the computer tomography data of the samples subjected to different loads. The investigation revealed the existence of comminuted area in the vicinity of the indenter and the formation of multiple cracks in the zone lying below. It was found that the higher is the applied indentation load, the denser is the crack pattern and larger are the cracks. A similarity of such a mechanical behavior between the examined material and dentin taken as a biocomposite is discussed

    Clinical features of post-COVID-19 period. Results of the international register “Dynamic analysis of comorbidities in SARS-CoV-2 survivors (AKTIV SARS-CoV-2)”. Data from 6-month follow-up

    Aim. To study the clinical course specifics of coronavirus disease 2019 (COVID-19) and comorbid conditions in COVID-19 survivors 3, 6, 12 months after recovery in the Eurasian region according to the AKTIV register. Material and methods.The AKTIV register was created at the initiative of the Eurasian Association of Therapists. The AKTIV register is divided into 2 parts: AKTIV 1 and AKTIV 2. The AKTIV 1 register currently includes 6300 patients, while in AKTIV 2 — 2770. Patients diagnosed with COVID-19 receiving in- and outpatient treatment have been anonymously included on the registry. The following 7 countries participated in the register: Russian Federation, Republic of Armenia, Republic of Belarus, Republic of Kazakhstan, Kyrgyz Republic, Republic of Moldova, Republic of Uzbekistan. This closed multicenter register with two nonoverlapping branches (in- and outpatient branch) provides 6 visits: 3 in-person visits during the acute period and 3 telephone calls after 3, 6, 12 months. Subject recruitment lasted from June 29, 2020 to October 29, 2020. Register will end on October 29, 2022. A total of 9 fragmentary analyzes of the registry data are planned. This fragment of the study presents the results of the post-hospitalization period in COVID-19 survivors after 3 and 6 months. Results. According to the AKTIV register, patients after COVID-19 are characterized by long-term persistent symptoms and frequent seeking for unscheduled medical care, including rehospitalizations. The most common causes of unplanned medical care are uncontrolled hypertension (HTN) and chronic coronary artery disease (CAD) and/or decompensated type 2 diabetes (T2D). During 3- and 6-month follow-up after hospitalization, 5,6% and 6,4% of patients were diagnosed with other diseases, which were more often presented by HTN, T2D, and CAD. The mortality rate of patients in the post-hospitalization period was 1,9% in the first 3 months and 0,2% for 4-6 months. The highest mortality rate was observed in the first 3 months in the group of patients with class II-IV heart failure, as well as in patients with cardiovascular diseases and cancer. In the pattern of death causes in the post-hospitalization period, following cardiovascular causes prevailed (31,8%): acute coronary syndrome, stroke, acute heart failure. Conclusion. According to the AKTIV register, the health status of patients after COVID-19 in a serious challenge for healthcare system, which requires planning adequate health system capacity to provide care to patients with COVID-19 in both acute and post-hospitalization period

    TREATMENT OF SACROCOCCYGEAL TERATOBLASTOMA IN CHILDREN

    The article presents the analysis of treatment of several cases of relatively rare malignant tumor in children - sacrococcygeal teratoblastoma. Over last fifteen years there were 8 children with this diagnosis treated in the Irkutsk State Regional Children's Clinical Hospital. Mean age was 1.5 years. All children were admitted to the hospital with stages III and IV of the disease. After the course of chemotherapy the surgical treatment was conducted from perineal section with obligatory coccygectomy. Four children recovered, three died, one child is having the course of postoperative chemotherapy
