Spectra of Unsteady Wind Models of Gamma-Ray Bursts
We calculate the spectra expected from unsteady relativistic wind models of
gamma-ray bursts, suitable for events of arbitrary duration. The spectral
energy distribution of the burst is calculated over photon energies spanning
from eV to TeV, for a range of event durations and variability timescales. The
relative strength of the emission at different wavelengths can provide valuable
information on the particle acceleration, radiation mechanisms and the possible

Emission Spectra from Internal Shocks in Gamma-Ray-Burst Sources
Unsteady activity of gamma-ray burst sources leads to internal shocks in
their emergent relativistic wind. We study the emission spectra from such
shocks, assuming that they produce a power-law distribution of relativistic
electrons and posses strong magnetic fields. The synchrotron radiation emitted
by the accelerated electrons is Compton up-scattered multiple times by the same
electrons. A substantial component of the scattered photons acquires high
energies and produces e+e- pairs. The pairs transfer back their kinetic energy
to the radiation through Compton scattering. The generic spectral signature
from pair creation and multiple Compton scattering is highly sensitive to the
radius at which the shock dissipation takes place and to the Lorentz factor of
the wind. The entire emission spectrum extends over a wide range of photon
energies, from the optical regime up to TeV energies. For reasonable values of
the wind parameters, the calculated spectrum is found to be in good agreement

Tomorrow’s world: Current developments in the therapeutic use of technology for psychosis
There is now an established evidence base for the use of information and communication technology (ICT) to support mental healthcare (‘e-mental health’) for common mental health problems. Recently, there have been significant develop-ments in the therapeutic use of computers, mobile phones, gaming and virtual reality technologies for the assessment and treatment of psychosis. We provide an overview of the therapeutic use of ICT for psychosis, drawing on searches of the scientific literature and the internet and using interviews with experts in the field. We outline interventions that are already relevant to clinical practice, some that may become available in the foreseeable future and emerging challenges for their implementation
Early multi-wavelength emission from Gamma-ray Bursts: from Gamma-ray to X-ray
The study of the early high-energy emission from both long and short
Gamma-ray bursts has been revolutionized by the Swift mission. The rapid
response of Swift shows that the non-thermal X-ray emission transitions
smoothly from the prompt phase into a decaying phase whatever the details of
the light curve. The decay is often categorized by a steep-to-shallow
transition suggesting that the prompt emission and the afterglow are two
distinct emission components. In those GRBs with an initially steeply-decaying
X-ray light curve we are probably seeing off-axis emission due to termination
of intense central engine activity. This phase is usually followed, within the
first hour, by a shallow decay, giving the appearance of a late emission hump.
The late emission hump can last for up to a day, and hence, although faint, is
energetically very significant. The energy emitted during the late emission
hump is very likely due to the forward shock being constantly refreshed by
either late central engine activity or less relativistic material emitted
during the prompt phase. In other GRBs the early X-ray emission decays
gradually following the prompt emission with no evidence for early temporal
breaks, and in these bursts the emission may be dominated by classical
afterglow emission from the external shock as the relativistic jet is slowed by
interaction with the surrounding circum-burst medium. At least half of the GRBs
observed by Swift also show erratic X-ray flaring behaviour, usually within the
first few hours. The properties of the X-ray flares suggest that they are due
to central engine activity. Overall, the observed wide variety of early

Cost-effectiveness analysis of an intervention project engaging traditional and religious leaders to improve uptake of childhood immunization in southern Nigeria
Vaccination is a cost-effective public health intervention, yet evidence abounds that vaccination uptake is still poor in many low- and middle-income countries. Traditional and Religious Leaders play a substantial role in improving the uptake of health services such as immunization. However, there is paucity of evidence on the cost-effectiveness of using such strategies. This study aimed to assess the cost-effectiveness of using a multi-faceted intervention that included traditional and religious leaders for community engagement to improve uptake of routine immunisation services in communities in Cross River State, Southern Nigeria. The target population for the intervention was traditional and religious leaders in randomly selected communities in Cross River State. The impact of the intervention on the uptake of routine vaccination among children 0 to 23 months was assessed using a cluster randomized trials. Outcome assessments were performed at the end of the project (36 months).The cost of the intervention was obtained from the accounting records for expenditures incurred in the course of implementing the intervention. Costs were assessed from the health provider perspective. The cost-effectiveness analysis showed that the incremental cost of the initial implementation of the intervention was US60/measles case averted. However, for subsequent scale-up of the interventions to new areas not requiring a repeat expenditure of some of the initial capital expenditure the ICER was estimated to be US$34 per measles case averted. Involving the traditional and religious leaders in vaccination is a cost-effective strategy for improving the uptake of childhood routine vaccinations
Systems Medicine 2.0: Potential Benefits of Combining Electronic Health Care Records With Systems Science Models
Background: The global burden of disease is increasingly dominated by non-communicable diseases.These diseases are less amenable to curative and preventative interventions than communicable disease. This presents a challenge to medical practice and medical research, both of which are experiencing diminishing returns from increasing investment. / Objective: Our aim was to (1) review how medical knowledge is generated, and its limitations, (2) assess the potential for emerging technologies and ideas to improve medical research, and (3) suggest solutions and recommendations to increase medical research efficiency on non-communicable diseases. / Methods: We undertook an unsystematic review of peer-reviewed literature and technology websites. / Results: Our review generated the following conclusions and recommendations. (1) Medical knowledge continues to be generated in a reductionist paradigm. This oversimplifies our models of disease, rendering them ineffective to sufficiently understand the complex nature of non-communicable diseases. (2) Some of these failings may be overcome by adopting a “Systems Medicine” paradigm, where the human body is modeled as a complex adaptive system. That is, a system with multiple components and levels interacting in complex ways, wherein disease emerges from slow changes to the system set-up. Pursuing systems medicine research will require larger datasets. (3) Increased data sharing between researchers, patients, and clinicians could provide this unmet need for data. The recent emergence of electronic health care records (EHR) could potentially facilitate this in real-time and at a global level. (4) Efforts should continue to aggregate anonymous EHR data into large interoperable data silos and release this to researchers. However, international collaboration, data linkage, and obtaining additional information from patients will remain challenging. (5) Efforts should also continue towards “Medicine 2.0”. Patients should be given access to their personal EHR data. Subsequently, online communities can give researchers the opportunity to ask patients for direct access to the patient’s EHR data and request additional study-specific information. However, selection bias towards patients who use Web 2.0 technology may be difficult to overcome. / Conclusions: Systems medicine, when combined with large-scale data sharing, has the potential to raise our understanding of non-communicable diseases, foster personalized medicine, and make substantial progress towards halting, curing, and preventing non-communicable diseases. Large-scale data amalgamation remains a core challenge and needs to be supported. A synthesis of “Medicine 2.0” and “Systems Science” concepts into “Systems Medicine 2.0” could take decades to materialize but holds much promise
Vitamin D and subsequent all-age and premature mortality: a systematic review
<br>Background:
All-cause mortality in the population < 65 years is 30% higher in Glasgow than in equally deprived Liverpool and Manchester. We investigated a hypothesis that low vitamin D in this population may be associated with premature mortality via a systematic review and meta-analysis.</br>
<br>Methods:
Medline, EMBASE, Web of Science, the Cochrane Library and grey literature sources were searched until February 2012 for relevant studies. Summary statistics were combined in an age-stratified meta-analysis.</br>
<br>Results:
Nine studies were included in the meta-analysis, representing 24,297 participants, 5,324 of whom died during follow-up. The pooled hazard ratio for low compared to high vitamin D demonstrated a significant inverse association (HR 1.19, 95% CI 1.12-1.27) between vitamin D levels and all-cause mortality after adjustment for available confounders. In an age-stratified meta-analysis, the hazard ratio for older participants was 1.25 (95% CI 1.14-1.36) and for younger participants 1.12 (95% CI 1.01-1.24).</br>
<br>Conclusions:
Low vitamin D status is inversely associated with all-cause mortality but the risk is higher amongst older individuals and the relationship is prone to residual confounding. Further studies investigating the association between vitamin D deficiency and all-cause mortality in younger adults with adjustment for all important confounders (or using randomised trials of supplementation) are required to clarify this relationship.</br>
Light from the Cosmic Frontier: Gamma-Ray Bursts
Gamma-Ray Bursts (GRBs) are the most powerful cosmic explosions since the Big
Bang, and thus act as signposts throughout the distant Universe. Over the last
2 decades, these ultra-luminous cosmological explosions have been transformed
from a mere curiosity to essential tools for the study of high-redshift stars
and galaxies, early structure formation and the evolution of chemical elements.
In the future, GRBs will likely provide a powerful probe of the epoch of
reionisation of the Universe, constrain the properties of the first generation
of stars, and play an important role in the revolution of multi-messenger
astronomy by associating neutrinos or gravitational wave (GW) signals with
GRBs. Here, we describe the next steps needed to advance the GRB field, as well
as the potential of GRBs for studying the Early Universe and their role in the

On Deformations of n-Lie algebras
The aim of this paper is to review the deformation theory of -Lie
algebras. We summarize the 1-parameter formal deformation theory and provide a
generalized approach using any unital commutative associative algebra as a
deformation base. Moreover, we discuss degenerations and quantization of
