    Exercise and manual physiotherapy arthritis research trial (EMPART) for osteoarthritis of the hip: a multicenter randomized controlled trial.

    OBJECTIVES: To determine the effectiveness of exercise therapy (ET) compared with ET with adjunctive manual therapy (MT) for people with hip osteoarthritis (OA); and to identify if immediate commencement of treatment (ET or ET+MT) was more beneficial than a 9-week waiting period for either intervention. DESIGN: Assessor-blind randomized controlled trial with a 9-week and 18-week follow-up. SETTING: Four academic teaching hospitals in Dublin, Ireland. PARTICIPANTS: Patients (N=131) with hip OA recruited from general practitioners, rheumatologists, orthopedic surgeons, and other hospital consultants were randomized to 1 of 3 groups: ET (n=45), ET+MT (n=43), and waitlist controls (n=43). INTERVENTIONS: Participants in both the ET and ET+MT groups received up to 8 treatments over 8 weeks. Control group participants were rerandomized into either ET or ET+MT groups after 9 week follow-up. Their data were pooled with original treatment group data: ET (n=66) and ET+MT (n=65). MAIN OUTCOME MEASURES: The primary outcome was the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) physical function (PF) subscale. Secondary outcomes included physical performance, pain severity, hip range of motion (ROM), anxiety/depression, quality of life, medication usage, patient-perceived change, and patient satisfaction. RESULTS: There was no significant difference in WOMAC PF between the ET (n=66) and ET+MT (n=65) groups at 9 weeks (mean difference, .09; 95% confidence interval [CI] -2.93 to 3.11) or 18 weeks (mean difference, .42; 95% CI, -4.41 to 5.25), or between other outcomes, except patient satisfaction with outcomes, which was higher in the ET+MT group (P=.02). Improvements in WOMAC, hip ROM, and patient-perceived change occurred in both treatment groups compared with the control group. CONCLUSIONS: Self-reported function, hip ROM, and patient-perceived improvement occurred after an 8-week program of ET for patients with OA of the hip. MT as an adjunct to exercise provided no further benefit, except for higher patient satisfaction with outcome

    A Framework For Detecting Noncoding Rare-Variant associations of Large-Scale Whole-Genome Sequencing Studies

    Large-scale whole-genome sequencing studies have enabled analysis of noncoding rare-variant (RV) associations with complex human diseases and traits. Variant-set analysis is a powerful approach to study RV association. However, existing methods have limited ability in analyzing the noncoding genome. We propose a computationally efficient and robust noncoding RV association detection framework, STAARpipeline, to automatically annotate a whole-genome sequencing study and perform flexible noncoding RV association analysis, including gene-centric analysis and fixed window-based and dynamic window-based non-gene-centric analysis by incorporating variant functional annotations. In gene-centric analysis, STAARpipeline uses STAAR to group noncoding variants based on functional categories of genes and incorporate multiple functional annotations. In non-gene-centric analysis, STAARpipeline uses SCANG-STAAR to incorporate dynamic window sizes and multiple functional annotations. We apply STAARpipeline to identify noncoding RV sets associated with four lipid traits in 21,015 discovery samples from the Trans-Omics for Precision Medicine (TOPMed) program and replicate several of them in an additional 9,123 toPMed samples.     Forest Health Monitoring in the Ngangao Forest, Taita Hills, Kenya: A Five Year Assessment of Change

    Forest Health Monitoring (FHM) provides a standardized detection-level survey of forest and tree characteristics for large forested areas. We have adopted FHM methods from this temperate-based program to tropical forests in the Eastern Arc Mountains (EAM) of Kenya and Tanzania. This paper reports the first assessment of trend data in the EAM over a period from 2001 to 2006. Growth and diversity statistics are presented for 11 forest plots in Ngangao Forest; only six (of 45) species constitute the predominance (68%) of basal area growth. Four endemic trees were tallied in our survey: Dasylepis integra, Leptonychia usambarensis, Macaranga conglomerata, and Polyscias stuhlmannii. We contrast diversity statistics with the nearby Chawia Forest and confirm the basic principal of species area relationships. Additionally, we examine visual crown assessments and tree damages. Tree crowns appeared healthy overall, although one species (Albizia gummifera) displayed increased dieback and foliage transparency and decreased crown density. Tree damages increased slightly over the survey period with stem decays constituting the most prominent symptom. Overall, we found no significant change in this first remeasurement of forest conditions at Ngangao Forest. The results reported here, however, may require more intense inspection of conditions. The Forest Health Monitoring approach constitutes a detection-level survey; it is dependent on follow-up by local experts to determine more precise causality where initial problems are found.     Monitoring the Health of Selected Eastern Arc Forests in Tanzania

    The eastern arc mountains (EAMs) are a chain of isolated mountains (534,000 ha) in Kenya and Tanzania surrounded by arid woodlands and influenced by the Indian Ocean. In 1900 there was three times the amount of forest cover there is today. Much of the original forests have been converted into agricultural crops. These mountains are recognized as a globally important \u27hot spot\u27 by World Wide Fund for Nature (WWF) and World Conservation of Union (IUCN) for forest biodiversity and are major national, regional and local sources of hydropower, water and wide array of forest-based benefits and agricultural production. In February 2000 we began a 3-year evaluation of forest health, land-use change and information sharing in the EAMs. Permanent sample plots were used to evaluate forest health. This paper summarizes the baseline data from eighteen permanent sample plots in the east Usambara\u27s and nine in Ulugurus. The eastern arc web site (http://www.easternarc.org) was established, continues to expand and contains some detailed information on the project. Information gathered will assist land managers, planners, policy makers, interested communities (villages) and individuals in decision-making. These data can also provide an early warning system of stresses on forest catchments

    Professional nursing governance in a large Australian health service

    Background: Professional nursing governance refers to the processes and structures that influence nursing practice within an organisation. This study measured the effect of structured meeting communication processes on nurses\u27 perceptions of professional governance.Method: The intervention was implemented in eight hospital wards. After three months, nurses on the intervention wards and eight matched-control wards completed the Index of Professional Nursing Governance (n = 225). Data were compared with a sample of Magnet&reg; (n = 3) and non-Magnet (n = 46) hospitals.Results: There was substantial variation in nurses\u27 perceptions of governance across the 16 wards, irrespective of the intervention. Compared to non-Magnet hospitals, the overall score and three of the six subscales scores were higher in this study. Magnet hospitals scores, however, were typically higher suggesting greater progress towards shared governance.Conclusions: Professional nursing governance can be highly variable across individual wards and tailored interventions should be considered.<br /

    Crystal structure of ÎŽ-chymotrypsin bound to a peptidyl chloromethyl ketone inhibitor

    Chymotrypsin is a member of the trypsin family of serine proteases and is one of the first proteins successfully studied by X-ray crystallography. It is secreted into the intestine as the inactive precursor chymotrypsinogen; four sequential cleavages of the peptide bonds following residues 13, 15, 146 and 148 occur to generate the active pi, delta, kappa and alpha forms of chymotrypsin. C-13 NMR has shown [O\u27Connell & Malthouse (1995). Biochem. J, 307, 353-359] that when the delta form of chymotrypsin is inhibited by 2-C-13-enriched benzyloxycarbonylglycylglycylphenylalanyl chloromethane, a tetrahedral adduct is formed which is thought to be analogous to the tetrahedral intermediate formed during catalysis, This inhibitor complex has bean crystallized as a dimer in space group P4(1)2(1)2. The structure has been refined at 2.14 Angstrom resolution to an R value of 21.2% (free R = 25.2%), Conformational differences between delta-chymotrypsin and chymotrypsinogen in the region of the flexible autolysis loop (residues 145-150) were observed. This is the first crystal structure of S-chymotrypsin and includes two residues which are disordered in previous crystal structures of active chymotrypsin. A difference of 11.3 Angstrom(2) between the average Zz values of the monomers within the asymmetric unit is caused by lattice-disordering effects approximating to rotation of the molecules about a crystallographic screw axis, The substrate-binding mode of the inhibitor was similar to other chymotrypsin peptidyl inhibitor complexes, but this is the first published chymotrypsin structure in which the tetrahedral chloromethyl ketone transition-state analogue is observed, This structure is compared with that of a similar tetrahedral transition-state analogue which does not alkylate the active-site histidine residue
